A quick overview
What to know
before you start.
-
Leg cramps are not a listed side effect in the Mounjaro SmPC or patient leaflet, so the medicine is not an established cause.
-
Cramps that appear during treatment may have indirect, everyday explanations, such as drinking less, eating less or gut upset. Your prescriber can help work out which.
-
Staying well hydrated and eating a varied diet are sensible first steps, and they support a reduced-calorie plan anyway.
-
Painful swelling, redness or warmth in a calf, chest pain or breathlessness needs medical help straight away, not waiting.
-
You can report any suspected side effect through the Yellow Card scheme, and our prescribing team offers side-effect support.

01 · The core answer
Does Mounjaro cause leg cramps?
Leg cramps are not listed as a side effect of Mounjaro in the SmPC or the patient information leaflet, so they are not an established effect of the medicine.113 That does not mean your cramps are imaginary or unimportant. It means that if you are getting them, the more useful question is what else is going on, and your prescriber is the right person to help answer it.
Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties. In practice most people feel fuller sooner and less hungry between meals.1 Eating and drinking less is the intended effect on appetite, but it can have knock-on effects on how you feel day to day. Those knock-on effects, rather than a direct action on the muscles, are the most reasonable places to look first.
02 · Your next step
What should you do if you are getting cramps?
Start by noting when the cramps happen, how often, and whether anything changed around the time they began, such as a new dose, a stomach upset, a new medicine or a big change in diet. This gives your prescriber something concrete to work with.
Then check the basics. Are you drinking regularly through the day, even when you do not feel thirsty? Are you eating a varied diet rather than very little? Have you had sickness or loose stools recently? Small corrections to these often settle things without any other change.
If the cramps are frequent, severe, disturb your sleep or keep coming back, contact your prescribing team or your GP rather than waiting. If you are one of our customers, our team offers ongoing monthly reviews and side-effect support, so you do not have to work this out alone.
Painful swelling, redness or warmth in a calf, chest pain or sudden breathlessness is not a simple cramp: get medical help straight away.
03 · Why cramps may appear
Why might you get leg cramps while on Mounjaro?
Leg cramps are common in adults generally, and they have many everyday causes. Starting a weight-management medicine does not protect you from them, and it coincides with changes that can plausibly make them more noticeable.
The changes most worth considering are these:
- Drinking less. Feeling full sooner can mean you sip less through the day without noticing.
- Eating less variety. A smaller appetite can narrow what you eat, and a reduced-calorie diet works best when it is still balanced.
- Gut symptoms. Sickness or loose stools can leave you short of fluid.
- Changes in activity. Starting to walk or exercise more, which is part of the plan alongside treatment, can make muscles more tired than usual.
- Other medicines or conditions. Cramps have causes entirely unrelated to weight management.
Because several of these can overlap, it is rarely helpful to blame a single factor. A review of the whole picture with a clinician is more reliable than guessing.
Mounjaro is used alongside a reduced-calorie diet and increased physical activity, and individual experience varies.1
04 · Hydration and eating less
What happens if you do not drink enough on Mounjaro?
Not drinking enough fluid can leave you feeling tired, light-headed or generally unwell, and it is a common, avoidable reason for muscle discomfort in anyone. Because Mounjaro helps you feel fuller sooner, it is easy to drink less than you used to, particularly if you used to drink with meals or snacks.1
A few practical habits help:
- Keep a water bottle where you work and where you sit in the evening.
- Drink steadily through the day rather than catching up late at night.
- Have a drink with each meal, even a small meal.
- Take extra care in hot weather, after exercise, or if you have had sickness or loose stools.
Eating very little is a separate issue. A reduced-calorie diet should still be varied, with fruit, vegetables, protein and wholegrains. If you struggle to eat enough, tell your prescriber at your review rather than pushing through. They can look at whether your dose or pace of change suits you.
Needs differ by person, and supplement dosing is not standardised, so ask your pharmacist or prescriber what suits you before taking anything for cramps.
05 · Gut symptoms and cramps
Can stomach upset make cramps more likely?
Sickness or loose stools can leave you short of fluid, and that is a plausible, indirect route to muscle cramps in anyone. The medicine slows how quickly the stomach empties, which is part of how it works, and gut-related effects are the area where the patient leaflet should be your first reference.113
If you are having ongoing sickness, loose stools or constipation, do not simply wait it out. Tell your prescriber, because they can look at how you are coping with your current dose and whether anything should change. Never change your dose on your own.
Do not confuse leg cramps with abdominal pain. Cramping in the stomach is a different symptom with different considerations, and we cover it in our guide to belly cramps on Mounjaro and the broader question of whether Mounjaro can cause cramps.
If you have severe or persistent abdominal pain, or pain that spreads to your back, get medical help straight away.
06 · Weight loss and cramps
Can weight loss itself cause leg cramps?
Weight loss is not an established cause of leg cramps, but a big change in how you eat, drink and move can make you notice your body differently. The question is better framed around what changed in your routine than around the weight itself.
In the SURMOUNT-1 trial of adults taking tirzepatide alongside diet and exercise, people lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.2
Muscle strength matters when you are losing weight. Keeping up gentle, regular activity, including some resistance work if your doctor agrees, supports how your legs feel and function. If you are unsure what is safe for you, ask your GP or prescriber before starting a new exercise programme.
07 · Leg pain and restless legs
Is leg pain or restless legs a Mounjaro side effect?
Cramps, aching and restless legs are different experiences, and describing yours precisely helps a clinician.
- A cramp is a sudden, tight, often painful tightening of a muscle that eases after a short time.
- Aching or heaviness is a more constant discomfort, sometimes after activity.
- Restless legs is an urge to move the legs, often in the evening or at night, that eases when you move.
If you notice a new symptom after starting treatment, tell your prescriber even when you are unsure it is connected. You can also report any suspected side effect yourself through the MHRA Yellow Card scheme.19 Reports help regulators keep the safety picture up to date, which is exactly how effects get added to product information over time.
Our page on GLP-1 medicines and leg cramps looks at the same question across the wider medicine class.
08 · What can help at night
How do you stop leg cramps while on Mounjaro?
The most dependable steps are to keep drinking regularly, eat a varied diet and keep moving, then ask a clinician if cramps continue. These are general self-care measures, not treatments for a Mounjaro side effect, because leg cramps are not an established one.
Things many people find sensible to try:
- Drink steadily through the day, and have a drink in the evening.
- Eat regular meals that include fruit, vegetables, protein and wholegrains.
- Stretch your calves gently in the evening and when a cramp starts.
- Walk the cramp off when it happens, if it is safe to stand.
- Keep up regular, gentle activity.
- Avoid sudden big increases in exercise.
Be cautious about supplements. Low-cost supplements are widely available without any assessment, and dosing is not standardised. Do not add products to your routine without asking a pharmacist or your prescriber, particularly if you take other medicines or have kidney, heart or other health conditions.
Do not change or skip your Mounjaro dose to try to deal with cramps without speaking to your prescriber. After a final dose the medicine takes roughly four to five weeks to clear the system, and there is no way to speed this up, so stopping is not a quick fix for a symptom.1
09 · Why so many cramps suddenly
Why am I suddenly getting so many leg cramps?
A sudden run of cramps usually has a recent trigger, and finding it is the quickest route to relief. Common triggers include a change in fluid intake, heat, a new exercise routine, illness, long periods sitting or standing, or a new medicine.
Ask yourself what changed in the weeks before the cramps began. If the answer is a dose increase, say so at your next review, even if you think the link is unlikely. Your prescriber can consider whether anything else about your health or other medicines is relevant.
It is also worth knowing that many causes of cramps have nothing to do with weight management.
If cramps are accompanied by weakness, numbness, swelling of a leg or skin changes, or if they affect only a calf with pain and warmth, get medical help straight away.
10 · Long-term effects and reviews
What are the long-term side effects of Mounjaro?
Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions. Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.113
Treatment is supplied only after an online consultation reviewed by a GPhC-registered prescriber, and repeat orders re-check suitability, with no auto-shipping. Ongoing monthly reviews, dose titration and follow-up assessments are carried out by the prescribing team, with side-effect support throughout. We are a GPhC-registered pharmacy, registration 9012878, which you can check on the GPhC register.12
Mounjaro is a prescription-only medicine used alongside a reduced-calorie diet and increased physical activity. Individual results vary, and this page is information, not a substitute for medical advice.1
11 · Where to get more help
Who can help with persistent cramps?
Your GP is the right first contact for cramps that are frequent, severe or unexplained, and your prescribing team can advise on whether your current dose and routine need a review. Pharmacists can also advise on over-the-counter options and on whether they suit your other medicines.
If you take medicines for other conditions, bring the list. Some medicines are associated with muscle symptoms, and a clinician can check this in a way a general web page cannot. If you use insulin or sulfonylureas, your doses may need adjusting while you are on Mounjaro because of low blood sugar risk, so tell your prescriber about them.1
Questions about where the injection goes are covered in our guide to injecting Mounjaro in the leg or stomach.
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
