Neither is better: the stomach area (at least 5cm from the belly button) and the front of the thigh are both licensed injection sites, and what matters is that you rotate the site with each dose rather than injecting the same spot every week.13 Rotating sites each week is the practical advice that reduces stinging.13
Many pages online claim one location is more effective, faster or gentler. Mounjaro is a once-weekly prescription medicine that activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties.1 That is a whole-body effect. It is not spot treatment, so injecting near your belly does not mean you lose weight from your belly.
Pick the site you can reach, see and keep still comfortably, and vary it. Comfort and consistency matter more than any claimed advantage of a particular patch of skin.

Why rotating injection sites matters
Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.13
A simple routine works for most people. Decide your pattern in advance, for example alternating between sides or between areas, and keep a note of last week's site. If you inject on the same day each week, the habit becomes easy to follow.
Avoid returning to a spot that is still sore, bruised or lumpy. Give that area time to settle and use a different place. If you are unsure which areas are suitable, check the patient information leaflet or ask the prescribing team.13

05 · Does the site change results?
Does the injection site change your results?
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 Those results are trial averages, and individual results vary. The figures describe the dose, not the site.
For someone starting at 100 kg, 20.9% is about 21 kg.
What drives results is the dose you reach, how consistently you take it, and the reduced-calorie diet and physical activity alongside it. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 Most people feel fuller sooner and less hungry between meals.1
So the fat under your skin at the injection site is simply where the medicine is delivered. If you see claims that a site gives faster or stronger results, treat them as unproven.
06 · Injection-site reactions
Injection-site reactions: what is normal
Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days.13 Injection-site reactions were reported by 8.0% of people taking tirzepatide in the weight-management trials, compared with 1.8% on placebo.1
Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.13 Those signs need a professional opinion rather than waiting it out.
Reactions do not depend on you choosing the wrong body part. They can happen at any site, which is another reason to rotate. If a reaction seems unusual, you can report it through the MHRA Yellow Card scheme.19
Do not mix up local reactions with whole-body symptoms. Severe, persistent stomach pain that may spread to your back, with or without vomiting, is a possible sign of pancreatitis — get medical help straight away. That pain comes from the digestive system, not from where you injected.
07 · Cold pens and storage
Should Mounjaro be injected cold?
Letting the pen warm briefly out of the fridge reduces stinging, so injecting straight from the fridge is more likely to sting.13 Follow the guidance in your leaflet.
Store unopened pens in the fridge at 2–8°C until first use. They keep until the printed expiry date.13 For how long an unopened pen can safely sit out of the fridge, check the patient information leaflet or ask the pharmacy rather than guessing.
Once you start using a KwikPen, it is licensed for up to 30 days (not above 30°C) out of the fridge in use.1 Do not apply that figure to a pen you have not yet started.

08 · Pinching, belly button and technique

In the stomach area, inject at least 5cm from the belly button; for whether to pinch the skin, follow the instructions for your pen.
If a spot is sore, bruised or lumpy afterwards, treat it as an injection-site reaction and use a different area next time. Mild reactions usually settle within a few days, and anything spreading, hot, very painful or lasting more than a week should go to your clinical team.13

09 · Stomach effects are not site effects
Stomach symptoms after injecting are not caused by the site
Digestive symptoms come from how the medicine works, not from injecting into the stomach area. The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13 You can have them whichever site you use.
Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts.13 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.1
Practical steps that help most people: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated, and avoid lying down straight after eating.13 If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.

10 · Leg-related questions
Leg injections and leg symptoms
If you inject into your thigh and later notice leg symptoms such as cramps, do not assume the injection site is the cause. Discuss new symptoms with your prescriber and report them through the Yellow Card scheme.19

11 · Tips and what to avoid
Tips for Mounjaro, and what to avoid
The most useful habits are simple. Inject on the same day each week, rotate sites, let the pen warm briefly, and eat smaller meals slowly. Keep meals balanced, with protein especially, stay hydrated and keep gentle activity going.13
- Lying down straight after eating, which can worsen digestive discomfort.13
- Pushing through severe symptoms. If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up.13
- Ignoring warning signs: get medical help straight away for severe, persistent stomach pain, pain in the upper right abdomen with fever, yellowing of the skin or eyes, or swelling of the face or throat.
- Buying from anyone offering to supply without a consultation or prescription. Mounjaro is prescription-only, and the MHRA publishes counterfeit alerts.7
If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, such as shakiness, sweating or confusion, because your prescriber may need to adjust those medicines.13

Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
