The Mounjaro pen at 5mg costs £189.99 at the standard price for one pen, which works out at £47.50 per week because a pen holds four weekly doses. That is what every repeat order costs before the reorder discount. What a year on Mounjaro costs if you settle at 5mg

Nobody starts on 5mg. Everyone begins on 2.5mg, and if you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £879.96 at standard prices. A first year (13 pens) costs £2,399.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,279.88 if you step up through every dose to 15mg. These totals are at standard prices: a new customer's first order is £40 less, and every later order is £25 less. If you would settle at 7.5mg or 10mg instead, plan on £244.99 or £274.99 per month at standard prices.

Ordering starts with a short, free health questionnaire through our free online consultation. There is no charge and no obligation to continue.

A GPhC-registered prescriber then reviews it. This is a clinical review, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.

04 · What the 5mg pen is

What the Mounjaro KwikPen 5mg/0.6ml actually is

The Mounjaro KwikPen 5mg/0.6ml is the 5mg strength of Mounjaro (tirzepatide), a once-weekly injectable prescription medicine made by Eli Lilly. It comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.¹ The 5mg strength is the first step up from the starter pen. It is also a dose many people stay on long term.¹

The "5mg/0.6ml" wording is simply how the strength appears in the product name. It identifies the pen on the box and in the leaflet. What matters in daily use is that each strength is its own pen, and that each pen contains four weekly doses.

In the UK, Mounjaro is licensed for weight management in adults and for type 2 diabetes.¹ Which one applies to you depends on your prescriber's assessment. How it works

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.¹ In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).¹

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4–5 weeks to clear the system.¹ There is no way to speed this up, and nothing needs doing while it clears. That matters if you stop treatment or are managing side effects: the effects of the last 5mg dose do not disappear overnight.

A prescription-only medicine

Mounjaro is prescription-only and is supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. 05 · Where 5mg fits

Where 5mg sits on the Mounjaro dose ladder

5mg is the second step. Everyone starts Mounjaro on 2.5mg once a week, and this starting dose is about letting your body adjust, not about weight loss itself.¹ After at least 4 weeks the dose increases to 5mg. From there it can step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.¹

A single 2.5mg pen gives four weeks at the starting dose, and your first 5mg pen follows. If you are tolerating 5mg and your prescriber is happy, you can stay there. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.¹

When to review

Weight loss on Mounjaro is gradual by design. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.¹ That is a review point, not a verdict, and a reasonable moment to talk through diet, activity, side effects and whether the dose is right.

Ordering a higher-dose pen yourself without a prescriber's agreement is not something we support. Dose changes are made through the consultation and review process.

06 · Doses, clicks and half pens

How many doses are in a 5mg pen, and what about clicks and half pens?

A 5mg Mounjaro pen contains four weekly doses. Each KwikPen contains four weekly doses, injected once a week, so a pen lasts four weeks.¹ If your pen is the 5mg strength and your dose is 5mg, each injection uses one quarter of the pen's contents. After four injections the pen is finished, even if you think a little medicine remains visible. Dispose of it in your sharps bin and start the next pen on the usual day.

Clicks: how the pen is actually set

Do not count clicks. The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

The same applies if you wonder about getting 5mg from a 7.5mg or 15mg pen, or 2.5mg from a 5mg pen. Each strength is its own pen, designed to deliver its labelled weekly dose. If you have the wrong strength, contact your prescriber or us for guidance rather than improvising.

Can you take half a 5mg pen?

No. A pen holds four weekly doses of its labelled strength, and the weekly dose is the strength on the box. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.¹ Altering doses outside the licence means unknown dosing accuracy and no safety data.

Also note that sharing a pen between people, or trying to get extra injections from one pen, is unsafe. Pens are for one person, with their own needles and their own prescription.

07 · Injecting and missed doses

How to inject the 5mg pen, and what to do about missed doses

Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.¹ Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.¹ Rotating helps you avoid injecting repeatedly into the same spot. Follow the pen's instructions for the injection steps and for how long to keep the pen in place, rather than relying on a number from elsewhere.

Choosing a day and time

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week. Pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off, then stick to it.¹ A phone reminder, or the free GLPTrackr companion app for iOS and Android, can help you keep track of injection days and pen numbers.

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

In a four-dose pen, a skipped dose simply means the pen lasts a little longer in calendar terms. You do not need to "catch up" by injecting more. If you miss more than one dose, speak to your prescriber, who can advise whether to restart at the same dose or a lower one. That decision belongs to the prescriber, not to a rule of thumb.

Changing your injection day

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ For example, if you injected on a Sunday and want to move to Thursday, the gap is four days, so that works. Moving from Sunday to Tuesday would leave only two days, so you would wait. Once you move, keep the new day consistent. If you are unsure, ask the prescribing team before you inject.

Side effects to expect

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.¹ They can return briefly after a dose increase, so the first 5mg injection is worth noting in your tracker. 08 · Storage and travel

Storing and travelling with a 5mg KwikPen

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.¹ Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.¹

A 5mg pen lasts four weeks in two senses. In dosing terms, a pen holds four weekly doses, so it covers four weeks. In storage terms, an in-use pen is licensed for up to 30 days (not above 30°C) out of the fridge.

Unopened pens

Unopened pens should be kept in the fridge and keep until the expiry date printed on the box.¹ If an unopened pen has been out of the fridge, for example because of a delayed delivery or a power cut, check with the pharmacy before using it rather than relying on a remembered day limit. Flying and holidays

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.¹ In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates, use an insulated pouch. Check destination rules for travelling with prescription injectables before you fly.

Practical habits

Put new pens in the fridge as soon as they arrive. Our deliveries use discreet, temperature-controlled packaging with free next-day tracked delivery, so you should not leave a parcel sitting for long. Label the date you start each pen, so you can see at a glance when its 30 days out of the fridge (not above 30°C) end. Keep the cap on between injections. If a pen has frozen, do not use it, even if it looks normal once thawed. Contact the pharmacy for advice and a replacement decision.

Finally, store pens and needles out of reach of children, and put used needles in the sharps bin that comes with your order.

09 · What to expect at 5mg

What results look like on 5mg

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, over 72 weeks, compared with 3.1% with lifestyle changes alone.² At 72 weeks, 85% of participants on 5mg had lost at least 5% of their body weight.² These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

As a worked example only: for someone starting at 100 kg, 15% is about 15 kg. Your own result will depend on your starting weight, diet, activity, dose and how you tolerate treatment, and trial averages do not predict your own result. The same trial reported 19.5% on 10mg and 20.9% on 15mg, but these are separate dose groups within the trial and not a promise that moving up will always add loss.²

A realistic pace

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.¹ Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.

Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.¹ Weighing once a week at the same time of day, or using a tracker such as GLPTrackr, gives you a clearer picture than daily checks.

Is 5mg enough?

For many people, yes. Many settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.¹ If you are losing weight steadily, tolerating the medicine and your prescriber is content, there is no need to climb the ladder. If progress stalls, the review conversation looks at the whole picture: eating pattern, activity, sleep, other medicines, and then dose.

What the review checks

10 · Eligibility and the NHS

Who can use Mounjaro, and the NHS versus private picture

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.¹

That assessment is the point of the consultation. It is a clinical review by a GPhC-registered prescriber, and an order the prescriber judges unsuitable is refunded automatically. If you take other medicines, mention every one of them, including contraception and anything for diabetes. The prescriber will then advise on whether changes are needed.

Mounjaro on the NHS

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

11 · Safety and genuine supply

Safety, counterfeits and modified pens

Mounjaro is a prescription-only medicine, and the safest way to get a pen is through a registered pharmacy after a consultation. Buying from social media sellers, marketplaces or anyone offering to supply without a consultation or prescription carries real risk, because you cannot know what is inside the pen, how it has been stored or whether it is genuine. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting suspected problems.⁷

Check any pharmacy on the GPhC register before ordering.¹² Our Mounjaro comes via official LillyDirect supply, sourced directly from Eli Lilly's UK distribution, and the pens are genuine KwikPens in all six strengths. Pens are not to be stretched

Because a pen is designed for four weekly doses, any attempt to get more injections from one, divide it or use a different pen strength to approximate a dose is outside the licence. There is no safety data on it and no assurance of dosing accuracy.¹ If you feel the dose is too strong or too weak, the safe route is to talk to the prescriber, who can hold, step down or adjust within the licence.

Common effects and when to get advice

Regulators have looked specifically at whether GLP-1 medicines are linked to low mood or suicidal thoughts: the EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review. Any big change in weight, appetite and routine can affect how you feel, so tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.¹⁷

In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

Contraception, diabetes medicines and your prescriber

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.⁷ The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar. This is a conversation for your prescriber, not a reason to stop or alter anything yourself.¹

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
