Mounjaro in Leg vs Stomach: Which Injection Site Should You Use? (October 2026)

For Mounjaro in leg vs stomach, both are licensed sites: the stomach area (at least 5cm from the belly button) or the front of the thigh, and you should rotate the site with each dose. Mounjaro works on appetite-related gut-hormone receptors, not on fat under the skin where you inject.1 Current as of October 2026.

A quick overview

What to know
before you start.

  • Leg (thigh) and stomach are both everyday choices; the leaflet and your prescriber have the final word on permitted sites.
  • Rotating the site each week is the habit that matters most for avoiding sore, bruised or lumpy skin.
  • Mounjaro acts on gut-hormone receptors that regulate appetite, so where you inject does not decide where you lose weight.
  • Injection-site reactions were reported by 8.0% of people on tirzepatide in the weight-management trials, versus 1.8% on placebo.
  • Spreading, hot or very painful reactions, or any lasting over a week, are a reason to contact your clinical team.
In this guide

01 · The verdict

Mounjaro in leg vs stomach: who each site suits

The stomach suits people who like to see the skin clearly and find the area easy to reach. The leg suits people who prefer to inject while seated with the pen in plain view, or who want a break from a tender abdomen.

Licensed injection sites for the Mounjaro KwikPen are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.13

Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days. Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.13 That advice applies equally to the leg and the stomach.

03 · How to choose

The decision factors that actually matter

The first factor is whether you can follow the leaflet's instructions confidently at that site. If you are unsure, ask the prescribing team before your first dose rather than guessing. Ongoing monthly clinical reviews are part of our aftercare, and injection technique is a normal thing to raise in them.

The second is your digestive state. Most Mounjaro side effects are digestive, and nausea, diarrhoea, vomiting, constipation and abdominal discomfort affect more than 1 in 10 people.1 If your abdomen feels tender or bloated, using the leg that week is a sensible, comfortable choice. Our guide to bubbling stomach on Mounjaro explains what that sensation usually means.

The third is the skin itself. If a spot is sore, bruised or lumpy from a previous dose, use a different area. The fourth is routine. A site you can reach in the same place and position each week makes it easier to remember which spot comes next. A simple note on your phone or in the GLPTrackr companion app is enough.

If you want a deeper comparison of one specific pairing, see our pages on thigh versus stomach and arm versus stomach. Our shorter leg or stomach overview covers the same question in brief.

04 · Does site change results?

Does the injection site change how well Mounjaro works?

Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals. That is a whole-body, appetite-driven effect.

This also answers a common worry: injecting into the stomach does not mean the fat is lost from the stomach, and injecting into the leg does not mean leg fat is targeted. The medicine supports a reduced-calorie diet by changing appetite. It is not a local fat treatment.

The trial results that do exist are about the dose and the person, not the site. In SURMOUNT-1, 2,539 adults taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

For someone starting at 100 kg, 20.9% is about 21 kg. That is simple arithmetic on the trial average, not a prediction for any one person, and it says nothing about injection site.

05 · Rotating your sites

How to rotate injection sites week to week

Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.13 Licensed sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection, and you should rotate the site with each dose. Because Mounjaro is a once-weekly injection, rotation is easy to build into a routine.

A few simple patterns work well. You can alternate between the left and right side of the stomach and the left and right leg, giving a four-week cycle that lines up neatly with one pen. Or you can move to a new spot within the same area each week and switch areas monthly. What matters is that you do not return to exactly the same spot two weeks running.

Keep a short log. Note the date, dose and site, for example "week 6, 7.5mg, left leg". That makes it easy to spot a pattern if one area keeps becoming sore, and it gives your prescriber useful detail at review. The free GLPTrackr companion app (iOS and Android) is designed for tracking a GLP-1 journey if you prefer not to use a notes app.

Always follow the pen's own instructions for the injection itself, including how long to hold it in place. Needles, a sharps bin and injection guidance are included with every order, so you have what you need for the first dose.

If you want to read more about injecting into the leg specifically, our Mounjaro leg injection guide goes through the practical side step by step.

06 · Site reactions explained

Injection-site reactions: what is normal and what is not

Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days.13 In the weight-management trials, injection-site reactions were reported by 8.0% of people taking tirzepatide, compared with 1.8% on placebo.1 Injection-site reactions are also listed among the common side effects, affecting up to 1 in 10 people.1

That means most people will never have a significant problem, but a minority will notice some soreness. If you do, the practical response is the same whether you used the leg or the stomach: rotate to a different area next time and let the pen warm briefly before you inject.

Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.13 Those signs are different from a small bruise and should be looked at rather than waited out. You can also report any suspected side effect through the MHRA Yellow Card scheme.19

It is worth separating a site reaction from the general digestive effects of the medicine. Soreness where the needle went in is local. Nausea, diarrhoea or an unsettled stomach are digestive effects of the medicine and are not caused by injecting into the abdomen.1 If your abdomen feels uncomfortable, our page on whether Mounjaro can paralyse your stomach explains what is and is not happening.

07 · Stomach side effects

Stomach symptoms and why they are not caused by the site

People sometimes assume that injecting into the stomach causes stomach upset, and that switching to the leg will cure it. Most Mounjaro side effects are digestive, and the most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.1 These follow from how the medicine works, including slowing how quickly the stomach empties.

In the weight-management trials, nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo. Diarrhoea was reported by 18.7% on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.1 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time.

So a rough week after a dose increase is more likely to be about the new dose than the spot you chose. Side effects appear in the first days after starting or increasing a dose, peak early and settle as your body adapts, which is one reason Mounjaro uses a gradual dose ladder.1

Using the leg on a bad-stomach week is still a reasonable comfort choice, as nobody enjoys pressing a pen against a tender abdomen. It just should not be expected to prevent nausea.

Practical steps that help most people: eat smaller meals slowly, stop at the first feeling of fullness, favour plain lower-fat foods while adjusting, stay well hydrated and avoid lying down straight after eating. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.1

08 · Serious symptoms

Symptoms that are not about injection site at all

Serious side effects are uncommon but need urgent attention, and they have nothing to do with whether you injected into the leg or the stomach. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis. Pain in the upper right abdomen, fever or yellowing of the skin or eyes could be a gallbladder problem, so get medical help straight away.1

Signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing, need immediate medical help, so get medical help straight away.1 If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.

The distinction matters. A sore, slightly bruised patch on your thigh is a local reaction. Severe pain in the abdomen that travels to the back is a different thing and should not be put down to the injection site. If you are unsure which you have, treat the more serious possibility as real and get medical help straight away.

Leg symptoms that are not at the injection spot, such as aching or cramping, are a separate question again. Our guide to Mounjaro leg cramps covers that.

Your questions, answered

Mounjaro price UK — frequently asked questions

Answers to common questions about Mounjaro prices, NHS access and ongoing treatment.

Ask our pharmacy team
Is it better to inject Mounjaro in the leg or the stomach?
Mounjaro works on appetite-related gut-hormone receptors rather than on fat where you inject. If your abdomen feels tender or bloated, using the leg that week is a sensible comfort choice. Both are licensed sites: the stomach area (at least 5cm from the belly button) or the front of the thigh. Rotate the site with each dose.
How do I give a Mounjaro shot?
Follow the instructions in the patient information leaflet and the guidance supplied with your pen. Let the pen warm briefly out of the fridge, which reduces stinging, choose a different site from last week, and dispose of the needle in your sharps bin. Needles, a sharps bin and injection guidance come with every EverydayMeds order. Ask your prescriber if unsure.
Can you inject Ozempic in your thigh?
Ozempic is semaglutide licensed for type 2 diabetes, and its injection sites are set out in its own leaflet, so follow that. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. The licensed weight-management options are Mounjaro and Wegovy, and a regulated UK pharmacy will not supply Ozempic for weight loss.
What is Mounjaro used to treat?
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.
What happens if you don't drink enough water on Mounjaro?
Low fluid intake can make digestive effects harder, because many people already eat and drink less. Staying well hydrated is part of the standard advice for managing nausea, and fluid intake is the first step for constipation. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
What do GLP-1 medicines do to your bowels?
For Mounjaro, diarrhoea and constipation are both common; constipation happens because food moves through the gut more slowly and many people eat and drink less. Both are very common, affecting more than 1 in 10 people, and tend to ease as you adjust. For other medicines such as Ozempic, check that product's leaflet.
Can I inject Mounjaro in the same spot every week?
It is better not to. Rotating sites each week reduces the chance of redness, itching, bruising or a tender lump, which are common and normally settle within a few days. Contact your clinical team if a reaction is spreading, hot, very painful or lasts more than a week. A simple log of dose and site makes rotation easy.
Does injecting in the leg lose leg fat?
No. Tirzepatide activates GIP and GLP-1 receptors that help regulate appetite, so most people feel fuller sooner and less hungry between meals. That supports a reduced-calorie diet across the whole body. It is not a local fat treatment, so the site you inject does not decide where you lose weight.
