Why prices differ between pharmacies

Eli Lilly announced a UK list-price increase for Mounjaro on 14 August 2025, effective 1 September 2025.10 Pharmacy sell prices still vary because of confidential discounts between suppliers and pharmacies, which is why two providers can charge quite different amounts for the same pen. That variation is the reason comparison matters more than timing.

How to read any "deal" headline

Ask four questions of every offer. Is it a first-order price or a repeat price? Which dose does it apply to? Does it include the consultation and delivery? And is the seller a pharmacy you can find on the GPhC register? A headline that fails any of these tells you less than it appears to. Our own first-order figures are labelled as first-order prices, and the reorder discount is stated as a reduction on the standard price, so you can work out the real monthly cost for yourself.

Avoid planning around urgency. Starting treatment is a clinical decision made with a prescriber, who may decide Mounjaro is not suitable for you at all. A discount is only useful once that decision has been made, so treat it as a second consideration, not the first.

Comparing the cost of Mounjaro

A fair comparison looks at the total cost at the dose your prescriber chooses, from a regulated pharmacy. That means looking at the first-order discount, the repeat-order discount, delivery charges and whether the price includes the consultation. Outside the UK

Genuine treatment always involves a prescription and a clinical consultation, so any seller offering to supply without a consultation or prescription is not operating legally.7

How to verify a pharmacy

Search for the pharmacy on the GPhC register. Our registration number is 9012878. The listing should match the website name, and the register is the same one used by high street pharmacies.12

Checking the pen when it arrives

Mounjaro is made by Eli Lilly, and genuine UK stock reaches licensed pharmacies through Lilly's official distribution. Every pen carries a batch number and expiry date that can be checked. A genuine pen arrives cold-chain packaged, and the solution inside is clear and colourless.

Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, has been above 30°C, or has been in use for more than 30 days. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

Red flags in a deal

  • No health questionnaire, or a questionnaire with no clinician reviewing it.
  • Payment requests by bank transfer to an individual, or via messaging apps.

07 · Who can have Mounjaro

Who can be prescribed Mounjaro

A discount only helps if a prescriber decides Mounjaro is right for you. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1

It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable. That assessment is part of our free consultation, and it is reviewed by a GPhC-registered prescriber rather than a scoring script.

What the main trial found

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. Trial averages do not predict your own result.

Why this matters for deal hunting

The dose that suits you is set by your prescriber, not by which pen is on offer. Buying a higher dose because it is discounted is not a regulated approach. The medicine's licensed dosing is described in the product information, and your prescriber builds your plan from it.13

08 · NHS versus private

NHS or private: where deals fit in

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46

Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6 If you may qualify through the NHS, your GP is the place to ask first. The NHS prescription charge in England is £9.90 per item, though not every NHS weight management route involves it.

When private treatment makes sense

Private treatment suits people who do not meet the NHS thresholds yet but do meet the licensed criteria, or who prefer not to wait. In that case the price you pay is the main variable, which is why discounts and monthly cost matter. Compare it with the NHS honestly: NHS treatment, where you qualify, is the lower-cost route, and private treatment is the faster one for many people.

Stopping and the 4–5 week tail

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That is useful to know if you are pausing treatment for cost reasons: discuss it with your prescriber first, because stopping and restarting changes how you feel and how the dose should be handled.

Payment flexibility

If the issue is cash flow rather than the total price, Klarna and pay-monthly options are available, and there are no subscriptions, so you are not locked in. That spreads the cost without changing the medicine or its schedule, which is a safer way to ease a budget than changing how you inject.

What you are paying for: how Mounjaro works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Dose scaling and pen strength

Higher is not automatically better: the right dose is the one your prescriber finds effective and tolerable.

Comparing with Wegovy

Wegovy (semaglutide) is a once-weekly alternative whose pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. Titration steps up after at least 4 weeks, reaching the 2.4mg maintenance dose from week 17. 11 · Medicines, food and cautions

What to avoid while taking Mounjaro

The safest answer is to give your prescriber a full list of your medicines and conditions before you start, then follow the patient information leaflet. No food is banned on Mounjaro. Treatment works alongside a reduced-calorie diet and increased physical activity, and your prescriber or a dietitian can help shape that.1

Which drugs should not be taken with Mounjaro?

Two interactions are worth knowing. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 And if you use insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar; this is a conversation to have with your prescriber, not a reason to change doses alone.13

Beyond these, tell the prescriber about every prescription, over-the-counter and supplement product you take. The consultation asks about current medicines for exactly this reason, and a prescriber reviews the answers before approving anything.

Supplements and "natural" alternatives

Low-cost supplements marketed as natural alternatives are available cheaply without assessment and have no clinical oversight. Their dosing is not standardised, and they are not equivalent to a licensed medicine. Treat dramatic weight-loss claims on any product with caution.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
