Which Mounjaro injection site should you choose?

Choose the licensed site you can reach comfortably and reach the same way every week. Mounjaro is injected subcutaneously, meaning under the skin, and the licensed sites are the stomach area, the front of the thigh, or the back of the upper arm when someone else gives the injection.¹ All three are licensed options, so the choice is mostly practical: where you can see the area, keep the pen steady and feel at ease.

For most people living alone, that means the stomach area or the thigh. The upper arm is awkward to reach yourself, which is why the licence restricts it to injections given by another person. Does the injection site change your results?

Your results are tied to the dose you reach and the way you use the medicine, not to a particular injection site. In SURMOUNT-1, a trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.²

Those figures are reported by dose, alongside a reduced-calorie diet and increased physical activity. So the most useful question is not which site is magic, but which site lets you inject calmly every week without skipping. 03 · Stomach area

Injecting into the stomach area

The stomach area is licensed for Mounjaro, provided the injection is at least 5cm from your belly button.¹ People often choose it because it is easy to see and reach with one hand while the other holds the pen steady.

If you are asking what happens when you inject too close to the belly button, the answer for planning purposes is simple: do not. The 5cm margin is the licensed distance, so measure it by eye before you start and move further out if you are unsure. If you have a concern about a particular spot, ask your pharmacist or prescriber to look at it with you.

Treat the stomach area as a broad zone rather than one point. Splitting it into a left side and a right side gives you two separate rotation positions, which helps you avoid returning to the same spot on consecutive weeks.

Some people feel more tender in this area while their body adjusts to a new dose. If an area is very sore, leave it for that week and choose another licensed site.

04 · Thigh and upper arm

Injecting into the thigh or upper arm

The front of the thigh is a licensed site and is a practical alternative if you would rather not inject into your stomach area.¹ Many people sit down, rest the pen against the front of the thigh and inject there, which suits people who like to be seated and steady.

The question of whether Mounjaro works better in the thigh or the stomach is a common one. The trial results are reported by dose rather than by site, and the three licensed sites are used for the same weekly dose.² So if one site is easier for you to use consistently, that is the sensible one to rely on.

The back of the upper arm is also licensed, but only if someone else gives the injection.¹ That makes it a good choice when a partner, relative or carer helps, and a poor one if you are injecting alone. If someone else injects, ask your pharmacist to show them how the pen works first.

05 · Rotating sites

How to rotate Mounjaro injection sites

Rotate the site with every dose: the product information says to change the injection site each time you inject.¹

A simple routine is to make a four-week plan that matches one pen, which holds four weekly doses. This is one way to organise it, not a clinical rule:

06 · Timing and routine

Time of day, missed doses and changing your day

You can inject Mounjaro at any time of day, with or without food, as long as it is on the same day each week.¹ So morning or night is a matter of what you will remember, not of what works better.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.¹

You can change your weekly injection day if you need to, as long as at least 3 days (72 hours) have passed since your last dose.¹

Dose steps are separate from site choice. Everyone starts on 2.5mg once a week, which is about letting your body adjust rather than weight loss itself. After at least 4 weeks, the dose can increase to 5mg and then step up by 2.5mg at a minimum of 4-week intervals, up to 15mg weekly, depending on how you respond. Higher is not automatically better, and your prescriber guides the pace.¹

07 · Pen storage and warming

Storing the pen and reducing stinging

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Keep the cap on to protect it from light, and never use a pen that has been frozen.¹

Letting the pen warm briefly out of the fridge before you inject can reduce stinging, and so can rotating your sites each week.¹

Travelling is straightforward. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Unopened pens stay in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it.

08 · Site reactions

Redness, bruising and injection-site reactions

Mild redness, itching, a small bruise or a tender lump at the injection site is common and normally settles within a few days. Rotating sites each week and letting the pen warm briefly out of the fridge reduces stinging.¹

In the weight-management trials, injection-site reactions were reported by 8.0% of people taking tirzepatide, compared with 1.8% on placebo.¹ Contact your clinical team if a reaction is spreading, hot, very painful, or lasts more than a week.

09 · When to get urgent help

Serious side effects: when to get urgent help

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.¹

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.¹

Most side effects are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. They are most likely when starting treatment or moving up a dose, and for most people ease within days to a few weeks.¹ If you notice any new symptom, tell your prescriber, and you can report suspected side effects through the Yellow Card scheme.¹⁹

If you take the contraceptive pill, note that Mounjaro slows stomach emptying, which can reduce how well it is absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only: implants, coils and injections are unaffected.¹
