Mounjaro and Vitamin B12: What UK Patients Need to Know (October 2026)
Yes, you can take vitamin B12 alongside Mounjaro, but no supplement is required with Mounjaro. Eating much less can leave nutritional gaps, so a standard daily multivitamin is a reasonable safety net, and anything beyond that is worth discussing with a pharmacist.13 Mounjaro is tirzepatide only; it contains no B12. Current as of October 2026.

A quick overview
What to know
before you start.
-
Mounjaro contains tirzepatide only, so it does not supply vitamin B12 and no supplement is required with it.
-
Eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight.
-
Oral supplements may be absorbed more slowly when you start treatment, so mention what you take to your pharmacist.
-
B12 is not a weight-loss treatment and not a substitute for prescribed tirzepatide.
-
Symptoms you are worried about deserve a conversation with your prescriber or GP, not guesswork or self-dosing.
In this guide
01 · The core answer
Can you take vitamin B12 with Mounjaro?
Yes. The medicine and the vitamin do different jobs: tirzepatide activates the GIP and GLP-1 receptors to help regulate appetite, while B12 is a nutrient you get from food or supplements.1
The sensible question is therefore not "is it allowed?" but "do I actually need it, and in what form?". That depends on what you eat and what other medicines you take, so ask your pharmacist.
If you want a deeper look at how B12 fits with this class of medicine, our guide to B12 and GLP-1 medicines goes through it in more detail.
02 · Is this relevant to you?
Do you need extra B12 while on Mounjaro?
Not automatically. No supplement is required with Mounjaro, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight.13 Anything beyond that should be discussed with a pharmacist first.
The practical risk is indirect: a smaller appetite means a smaller variety and volume of food, so fewer B12-containing foods may end up on your plate. Our separate page on whether Mounjaro can cause B12 deficiency covers that question directly.
| Your situation | What it suggests | Sensible next step |
|---|---|---|
| Eating a varied diet with regular protein | Little reason to add anything specific | A standard multivitamin is optional, not required |
| Appetite is very low and meals are small or repetitive | Gaps are more likely | Consider a standard multivitamin and review your meals |
| Taking other long-term medicines | Other medicines may matter more than Mounjaro | Ask your pharmacist whether any of them affect vitamins |
| Symptoms such as unusual tiredness that worry you | Many causes are possible | Speak to your prescriber or GP about testing |
| Considering B12 shots for weight loss | B12 is not a weight-loss treatment | Do not substitute it for prescribed treatment |
03 · Why eating less matters
Why eating less matters for nutrients
Mounjaro works by helping you feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is the intended effect, and it is why the medicine works. The side effect of that success is that the food you do eat has to work harder.
When portions shrink, it is easy to fall into a narrow routine: the same few foods, skipped meals, or lots of liquid calories. A narrow routine is how nutrient gaps usually appear, whichever vitamin you are thinking about.
Because the stomach also empties more slowly, large meals can sit heavily. Smaller, slower meals tend to work better, and that is the moment to make each one count. Prioritise protein at each meal, plus vegetables, fibre and regular fluids.13 No food is banned, and eating a varied mix is the best first-line approach to covering your vitamins before reaching for a pill.
Our guide to vitamin deficiency on Mounjaro lays out the wider picture, including which nutrients people ask about most.
04 · Tablets, injections and absorption
B12 tablets, sprays and injections
Oral supplements, like other oral products, may be absorbed more slowly when you start treatment.13 The reason is that tirzepatide slows how quickly the stomach empties.1
That has a practical consequence for anything where timing matters. If you take an oral medicine with a strict schedule, tell your pharmacist or prescriber that you have started Mounjaro. They can advise on whether anything needs adjusting. For a general multivitamin, a small timing change is rarely a concern.
Our explainer on B12 injections and weight loss covers the claims in more detail.
Supplement dosing is not standardised across products, so read labels carefully and ask a pharmacist before combining several products at once.
05 · Mixed or compounded products
Beware tirzepatide "mixed with B12" products
Genuine Mounjaro is a pre-filled KwikPen containing tirzepatide, made by Eli Lilly.1 Some sellers advertise tirzepatide blended with vitamin B12, or vials you are told to mix yourself. Those are not the licensed UK medicine, and they are not what you receive when you order Mounjaro from a registered pharmacy.
There is no reason to add B12 into a pen or to draw anything out of one. The licensed product should be used exactly as supplied and as the pen's instructions describe. If a seller is offering to supply without a consultation or prescription, treat that as a warning sign in itself. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting problems.7
Our supply is official LillyDirect supply: Mounjaro direct from Eli Lilly's UK distribution, in genuine KwikPens. We are a GPhC-registered pharmacy (registration 9012878), and every order is reviewed by a GPhC-registered prescriber. If you want B12 alongside treatment, take it as a separate, sensibly chosen supplement rather than a blended injectable.
06 · Low B12 symptoms and testing
What about the signs of low B12?
Common complaints such as tiredness can have many causes, including simply eating and sleeping differently while losing weight.
If you are worried about a symptom, the useful step is a conversation with your prescriber or GP and, if they advise it, a blood test. A test tells you what your level actually is, which a symptom checklist cannot.
Cravings are not a reliable test of any vitamin level. Either way, ask your GP or pharmacist, and do not self-treat on the basis of an online list.
If you develop a new symptom on treatment, tell your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19
07 · Anaemia and other medicines
Anaemia, Ozempic and other medicines
People often ask whether Mounjaro can cause anaemia, or whether Ozempic can cause B12 deficiency. Anaemia is not listed as a side effect in the UK Mounjaro patient leaflet, and Ozempic is semaglutide licensed for type 2 diabetes only; tell your prescriber about any new or lasting symptom.13
New symptoms are worth raising with the prescriber and reporting through the Yellow Card scheme, which is how safety signals are picked up in the UK.19
What can matter more than tirzepatide is anything else you already take. Some medicines are known to interact with vitamin status, and that is a question for your pharmacist, who can see your full list. If you take insulin or a sulfonylurea alongside Mounjaro, doses may need adjusting because of low blood sugar risk, which is a different issue but a good reminder to tell every clinician what you take.
For wider supplement questions, such as whether vitamin D is worth considering, see our pages on Mounjaro and vitamin D and on taking vitamin D with treatment.
08 · B12 and weight loss claims
Does B12 help with weight or belly fat?
Claims that B12 shots melt fat are marketing, not a substitute for prescribed treatment. Supplements sold as "natural GLP-1 boosters" are not equivalents and are not substitutes for prescribed treatment.13
Where the weight-loss evidence does exist, it belongs to tirzepatide used with diet and exercise. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.
You cannot target belly fat with a vitamin. If you are interested in how B12 and weight come up together, our page on B12 injections and weight loss discusses why the idea persists.
09 · Protecting muscle and energy
Muscle, hydration and everyday habits
Vitamins are a small part of staying well on treatment. The bigger levers are protein, movement and fluids, and they do more for how you feel than any single supplement.
Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein, with a protein source at every meal, and resistance exercise on a regular weekly basis. This matters more, not less, as the dose rises and appetite falls.13
Increased physical activity is part of the licence, not an optional extra. Regular moderate activity such as brisk walking, plus resistance work, is the evidence-backed pattern.
Drinking enough matters more on Mounjaro than usual, because reduced appetite means less fluid from food. Inability to keep fluids down is a contact-your-clinical-team issue, not something to push through.
10 · Fibre and other supplements
Fibre and other supplements
Many people ask about the best fibre supplement to take on Mounjaro. Supplement choice is a pharmacist conversation, and dosing is not standardised.
Low-cost supplements are available widely without any assessment, and that cheapness is exactly why it is easy to pile several together. A longer list is not a better plan. Start with food, add a standard multivitamin if your appetite is small, and take anything further to a pharmacist.
If you would rather see how ready-made options are grouped, our vitamin bundle guide explains what such bundles typically include and what to check before buying.
11 · Getting started safely
Starting Mounjaro with your nutrition in mind
Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity, and individual results vary.13 Nothing here is a substitute for medical advice.
At the consultation, mention any supplements and medicines you already take. Our free online consultation is a short health questionnaire with no charge and no obligation, and orders the prescriber decides are not suitable are refunded automatically in full. Ongoing monthly reviews are part of our aftercare, so you can raise nutrition questions as your appetite changes.
Mounjaro is available on the NHS, but access is being phased in over several years. If you want to explore private treatment, our weight loss clinic page is the best starting point.
Once on treatment, a few habits keep nutrition on track:
- Include a protein source at every meal, even small ones.
- Keep a standard multivitamin in mind if your appetite is very low.
- Tell your pharmacist about any supplement before adding another.
- Raise new symptoms with your prescriber rather than self-dosing.
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
