03 · What the trial shows at 5mg

What the SURMOUNT-1 trial shows at 5mg

SURMOUNT-1 was a trial of 2,539 adults who took tirzepatide alongside diet and exercise. People on 5mg lost on average 15% of their body weight over 72 weeks, compared with 3.1% with lifestyle changes alone.² That is a trial average, and individual results vary.

Beyond the average, the trial reports how many people reached a given threshold. At 72 weeks, 85% of participants on 5mg had lost at least 5% of their body weight.² For comparison, the same trial reports 89% on 10mg and 91% on 15mg reaching that 5% mark.²

The higher doses did produce larger averages: 19.5% on 10mg and 20.9% on 15mg.² Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² These are separate dose groups, and your prescriber can discuss which dose suits you.

A worked example makes the percentage concrete. For someone starting at 100 kg, 15% is about 15 kg; for someone starting at 120 kg, it is about 18 kg. It is not a prediction of what you will lose or how fast, because the trial measured results at 72 weeks, not at any shorter checkpoint.

Remember the design of the trial too. Participants were also following a reduced-calorie diet and increasing physical activity, and the 3.1% comparison group shows how much lifestyle change alone achieved in the same period.² The medicine supports those changes by helping you feel fuller sooner and less hungry between meals.

If you are comparing what you read elsewhere with these numbers, check whether a claim names the trial, the dose and the duration. 04 · 2.5mg versus 5mg

Do you lose more on 5mg than on 2.5mg?

The licensed route is built so that 2.5mg is a starting dose, not a weight-loss dose. Everyone begins on 2.5mg once a week, and this starting dose is about letting your body adjust rather than about weight loss itself.¹¹¹ After at least four weeks the dose increases to 5mg.

That is why many people see more change once they reach 5mg. Weight loss on Mounjaro is gradual by design: the first weeks on 2.5mg are an adjustment phase and losses are usually modest, while most measurable progress builds from 5mg onward, month by month.¹ Experiences differ widely, and some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal.

Early loss also includes water weight, so the trend over several weeks matters more than any single weigh-in. If your scales move a lot in the first fortnight and then slow, that does not mean the treatment has stopped working. Weigh on the same day each week under the same conditions, and look at the direction over a month rather than the daily number.

The minimum interval of at least four weeks on each dose exists so your body can adjust, and your prescriber, not the calendar alone, decides when you move up.

Do not shorten the interval or combine pens to reach 5mg sooner. If you feel progress is slow on the starter dose, the useful step is a conversation with your prescriber at your monthly review, not a change you make yourself. 05 · What to expect on 5mg

What to expect on 5mg Mounjaro

On 5mg expect a gradual, steady pace rather than rapid change. The trial measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.¹ Most measurable progress builds from 5mg onward.

In practical terms, the mechanism explains what you may notice. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1, which help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.¹ In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. Because the stomach empties more slowly, smaller meals often feel like enough.

The commonest early effects are mild nausea, altered appetite and tiredness in the first days after each injection, and they usually settle.¹³ Many people choose a weekend morning to inject so that early effects fall on days off.

Not everyone's experience at 5mg is the same. Some people feel a clear step up in appetite control when they move from 2.5mg. Others notice less difference and find the benefit accumulates gradually. Neither pattern means something is wrong. If you are worried about a symptom, particularly a new or persistent one, talk to your prescriber, and you can report suspected side effects through the Yellow Card scheme.¹⁹

A symptom that is not on the UK patient leaflet list is not a listed side effect; tell your prescriber about any new or lasting symptom. 06 · Is 5mg enough?

Is 5mg enough, and how long should you stay on it?

For many people 5mg is enough, and it is a recognised maintenance dose. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.¹¹¹ Your prescriber will guide the right pace based on how you respond and tolerate treatment.

The licensed ladder runs from 5mg up in steps of 2.5mg at a minimum of four-week intervals: 7.5mg, 10mg and 12.5mg, to a maximum of 15mg weekly.¹ So the minimum time at 5mg before stepping up is four weeks, but there is no required maximum. If you are tolerating 5mg and your trend is moving the right way, staying put can be a sensible decision rather than a failure to progress.

How do you tell? Look at the trend over several weeks, not one reading. Your prescriber reviews treatment response, and if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether continuing or stopping is right for you.¹ Note the wording: it is the highest tolerated dose, so a review does not simply push you up the ladder regardless of side effects.

If you stay at 5mg, nothing about that dose is unlicensed or temporary. The trial's 15% average and the 85% reaching at least 5% loss were measured on 5mg.²

07 · Speeding up and plateaus

How to speed up weight loss on 5mg, and what to avoid

There is no safe shortcut to faster loss on 5mg: the evidence supports taking the medicine as licensed, alongside a reduced-calorie diet and increased physical activity.² That is the honest answer, and it is also where the real levers are.

What helps in practice is within your control without changing the medicine. Eat in a way that suits a reduced-calorie diet, which the appetite effect makes easier. Build activity you can sustain. What to avoid is more specific, and most of it concerns the medicine itself. Do not take two doses within three days of each other.¹ Do not freeze or use a frozen pen.¹³

For food, drink and other medicines, the patient information leaflet and your prescriber are the right sources, because they know your history. Tell the prescriber about every medicine you take. Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows, so ask about contraception. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.¹ If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar.

What about a plateau? A slowdown is not unusual, and the answer is review rather than self-adjustment. If your trend flattens over several weeks, raise it at your monthly review. Your prescriber can discuss whether a dose step-up is appropriate if you are not yet at maximum.

Treat advice promising dramatic weight-loss claims with caution. A medicine's results come from the dose, the diet and the time, not from a trick.

08 · Injecting and storage

Injecting, missed doses and storing your 5mg pen

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.¹ There is no best time of day: it works the same in the morning or evening. What matters is consistency, so pick a day and time you will remember and stick to it.

Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Each KwikPen contains four weekly doses. Needles, a sharps bin and injection guidance come with every order, and you should follow the pen's instructions for the injection itself.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose and take the next one on your normal day. Never take two doses within 3 days of each other.¹

You can change your injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.¹ This is useful if work or holidays shift your routine.

For storage, keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded. Keep the cap on to protect it from light, and never use a pen that has been frozen.¹³ Unopened pens stay in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements, and for hot climates an insulated pouch helps. Check destination rules for prescription injectables before you fly.

09 · NHS and private access

NHS or private: who can get 5mg and how

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁴⁶ Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

The licence itself is broader than the first NHS groups. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity.¹ A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable.

If you are weighing the routes, ask your GP about NHS eligibility first, since it is the right place to check where you fall in the phased rollout. Buying safely, stopping and switching

Buy Mounjaro only from a UK pharmacy that is registered with the General Pharmaceutical Council and that supplies after a consultation and prescription. Anyone offering to supply without a consultation or prescription is not operating safely. 11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.¹
