What Mounjaro is and what 5mg means

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Each KwikPen contains four weekly doses, so one pen lasts four weeks.13 The strength on the box is the dose you inject each week, not the total in the pen.

The pen is not a tablet or a shop-shelf product. It is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. How Mounjaro works

Mounjaro works by activating two natural gut-hormone receptors at once: GIP and GLP-1. Tirzepatide is the first treatment of its kind to do this.1 Together these actions help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals. That supports a reduced-calorie diet, which is the part of the plan that drives weight change. The medicine does not replace the diet or the activity; it makes sticking to them easier for many people. That is why the licence and the trials both describe treatment alongside lifestyle changes rather than instead of them.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, sold as Wegovy and Ozempic.1 That is a difference in mechanism rather than a verdict on which suits you. The slower stomach emptying matters for practical reasons. It explains the fullness many people feel. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.

Tirzepatide has a half-life of about five days. After a final dose, it takes roughly 4 to 5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. This is useful if you wonder what happens when you pause.

06 · Where 5mg fits

Where 5mg and 7.5mg sit on the dose ladder

Everyone starts Mounjaro on 2.5mg once a week, and 5mg is the second step. The starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks, the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1

Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1 If 5mg is working and you tolerate it, staying there can be entirely appropriate. If you need more, the next step is 7.5mg at least four weeks later.

The 2.5mg starting weeks are about adjustment, and experiences differ widely. Some people notice appetite change and early loss in week one, while others feel little until a higher dose. Both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

Weight loss on Mounjaro is gradual by design. Most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend. Treatment response is reviewed by your prescriber. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

07 · Efficacy at 5mg

How effective is Mounjaro at 5mg?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The same trial reported results at other doses. Participants lost on average 19.5% of their body weight on 10mg and 20.9% on 15mg over 72 weeks.2 At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

A worked example makes the percentages concrete. For someone starting at 100 kg, 15% is about 15 kg, and 20.9% is about 21 kg. Your own starting weight, diet, activity and response will change that. A trial average does not predict your own result, and the figures are not a promise about timing. Please do not read them as a number of kilograms per week; the trial reports an average at 72 weeks, not a weekly rate.

The pace matters as much as the endpoint. The first weeks on 2.5mg are an adjustment phase, and losses are usually modest. Most measurable progress builds from 5mg onward, month by month. Your prescriber reviews your response at intervals, and if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, they will discuss whether to continue, adjust or stop.1

It is worth being clear about what these numbers cannot do. They cannot tell you whether 5mg is enough for you, whether you will need 7.5mg, or how you will tolerate each step. They come from a clinical trial with structured support. They also do not compare Mounjaro with other medicines.

08 · Eligibility and NHS

Who can get Mounjaro 5mg: eligibility and the NHS

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Examples include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.11

That assessment is why a free consultation matters. It is not a box-ticking exercise: your answers are reviewed by a GPhC-registered prescriber, not a scoring script. The NHS picture is genuinely different. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40 or above with four or more qualifying conditions.6 Full rollout is expected to take years.

The practical upshot is that many people who meet the licensed criteria choose regulated private treatment rather than waiting. That is a personal decision. One other point: you do not need to be at your goal weight to be reassessed. If you are already on treatment elsewhere, a prescriber will want to know your current dose and how long you have taken it. Being open about that makes the review quicker and safer.

For people with type 2 diabetes, Mounjaro is also licensed for that condition, and NICE has separate guidance on it.8 Whether you can access it through the NHS depends on your local diabetes pathway, so ask your GP or diabetes team. Tell the consultation about any diabetes medicines you take; some, such as insulin and sulfonylureas, may need their doses adjusting alongside Mounjaro because of the risk of low blood sugar.

09 · Injecting safely

Injecting 5mg: technique, missed doses and changing day

Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.13 Licensed sites are the stomach area, at least 5cm from the belly button, the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose. Needles, a sharps bin and injection guidance are included with every order.

There is no "best" time of day. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off, but the right choice is the one you will remember.13

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1

You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That helps when holidays, shift patterns or a busy weekend interrupt your routine.

The most common side effects are digestive, such as nausea, and mild nausea, altered appetite and tiredness in the first days after each injection usually settle. If you notice any new symptom, including anything not listed in the patient information leaflet, discuss it with your prescriber. 10 · Storage and travel

Storing and travelling with your 5mg pen

Keep Mounjaro pens in a fridge at 2 to 8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, which is useful for short trips, after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.

Unopened pens belong in the fridge until first use, and they keep until the expiry date printed on the box.13 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit. Figures circulating online often come from other countries' labels, and the UK label is the one that applies to a UK pen.

Your delivery arrives in temperature-controlled packaging by tracked next-day DPD. Put the pen in the fridge when it arrives. Plan to be in, or to have a safe place for the parcel, so it does not sit in warm conditions for long. If a parcel is delayed or looks damaged, contact support straight away; faulty, damaged or wrong items should be reported within 3 days.

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.13 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. A pen holds four weekly doses, so one pen covers a four-week trip with some margin. If your trip is longer, or you are changing dose around then, order ahead of time so the next pen arrives before you need it. Remember that repeat orders re-check suitability and there is no auto-shipping, so allow time for that review.

Why does this matter if you want a pen nearby? A pen in your fridge is always nearer than a pharmacy across town. Many people order a pen shortly before the previous one runs out, and the monthly review gives you a natural prompt to do so. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. What drugs should not be taken with Mounjaro?
Tell the prescriber about every medicine you take, including over-the-counter products, during your consultation. Insulin and sulfonylureas may need their doses adjusting because of the risk of low blood sugar; they are not simply forbidden. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Your prescriber and the patient information leaflet are the right guides for your own combination of medicines, so do not stop or change any medicine yourself.

Can diabetics get Mounjaro on the NHS?
Mounjaro is licensed for type 2 diabetes, and NICE has separate guidance on its use for that condition. Whether you can get it on the NHS depends on your local diabetes pathway and your own treatment history, so ask your GP or diabetes team.
