What is included in the 5mg price?
The price shown is the price paid. The consultation is free, and there are no hidden fees. Needles, a sharps bin and injection guidance come with every order. Delivery is free, next-day, tracked and sent in temperature-controlled, discreet DPD packaging; Saturday delivery is £14.99. Orders placed by 3pm are dispatched the same day.
The pen is genuine Mounjaro supplied through official LillyDirect supply, direct from Eli Lilly's UK distribution. Ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team are part of the service, along with side-effect support. We do not auto-ship: each repeat order re-checks that treatment is still suitable for you.

How do you order a 5mg pen?
Ordering takes three steps. First, complete the free online consultation, a short health questionnaire with no charge and no obligation, via our free consultation. Second, a GPhC-registered prescriber reviews your answers. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Third, our registered pharmacy dispenses your pen and sends it by free next-day tracked delivery.

05 · Where 5mg sits
Where 5mg sits on the Mounjaro dose ladder
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 The 5mg pen is therefore the second strength on the ladder, one step above the 2.5mg starter pen. The 5mg dose is reached after at least 4 weeks on 2.5mg, depending on how you respond.
Each pen is designed and licensed to deliver exactly four weekly doses, so one pen is one month of treatment. The 5mg pen costs £218.99, or £54.75 a week.
The pen strength is 5mg, and the "mg" matters: a milligram is a thousandth of a gram. Always check the strength printed on the pen and your prescription label before you inject, and read the patient information leaflet that comes with the pen.13
Whether 5mg is the right dose for you depends on how you are getting on. Your prescriber reviews your progress and side effects and decides whether to stay at a dose or change it. Some people stay at 5mg, some move up, and some move down. 06 · Does 5mg work?
Does Mounjaro 5mg work?
Yes, 5mg is a clinically studied dose. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 Those are trial averages and individual results vary.2
To make that concrete with a worked example: for someone starting at 100 kg, 15% is about 15 kg. Your own starting weight, diet, activity and response will differ, and trial averages do not predict your own result.
The trial also reported how many people reached a threshold. At 72 weeks, 85% of participants on 5mg had lost at least 5% of their body weight.2 For comparison, the higher doses in the same trial showed larger average losses: 19.5% on 10mg and 20.9% on 15mg.2 That does not mean everyone needs the top dose. Your prescriber guides which dose suits you.
Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 That is also how the medicine is licensed.1
How it works
Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. Nothing needs doing while it clears, and there is no way to speed it up.1
07 · Eligibility
Who can be prescribed Mounjaro?
Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.1 Weight-related conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 It must be used alongside a reduced-calorie diet and increased physical activity.1
Meeting those criteria does not mean you will automatically be prescribed it. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.11 That is why the consultation matters and why it is reviewed by a GPhC-registered prescriber, not a scoring script.
The consultation is a short health questionnaire. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
Why eligibility affects what you pay
Because a declined order is refunded automatically in full, the price of the 5mg pen is not a price for "a chance of treatment". It is the price of a dispensed prescription medicine once you have been assessed. Dispensed prescription medicines cannot be returned except where an item is faulty, damaged or wrong; contact us within 3 days if that happens.
Repeat orders
Repeat orders re-check suitability. The prescribing team runs ongoing monthly reviews, dose-titration and follow-up assessments, and offers side-effect support. If your circumstances change, such as a new medicine or a new diagnosis, tell your prescriber at the next review. This is also why we do not auto-ship pens: each order is a fresh decision.

08 · NHS or private
NHS or private: which route suits you?
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially people with a BMI of 40 or above and four or more qualifying conditions.6 Full rollout is expected to take years.6
That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you are in a group the NHS is prioritising, ask your GP about your options first. The NHS also publishes its own information on the medicine.5
What private treatment involves
Private treatment means you pay for the pen: £218.99 at the standard 5mg price, with £40 off a first order and £25 off every later order. You also get a clinical pathway: a free consultation, prescriber review, dispensing by a registered pharmacy and monthly reviews.
The main practical differences are speed and certainty. The NHS route depends on local rollout and on whether you fall into a currently prioritised group. The private route depends on the prescriber's assessment and on what you are willing to pay. Neither route is risk-free or suited to everyone, and both require treatment alongside diet and activity.
How to decide
Ask yourself three questions. Do you meet the NHS criteria now? Can you comfortably fund the monthly cost for the length of treatment you and your prescriber expect? Would you rather wait for a local NHS pathway or start with a regulated private service? You can also move between routes. If you start privately and later become eligible on the NHS, speak to your GP and your private prescriber about handing over care.

09 · Comparing prices
How to compare Mounjaro 5mg prices fairly
The fairest comparison is like for like: the same dose, the same four-week pen, and the same inclusions. Across the UK pharmacies we track, Mounjaro pens range from £143.96 to £339.00 per month depending on dose and provider, according to our own price index checked 10 October 2026. That range covers all six doses, so it is not a 5mg price, and it should not be read as one.
For the same dose, providers differ. In our index, the gap between providers for the same dose ranges from £27 at the smallest to £41 at the largest. 10 · Spotting fakes
Buying safely: how to avoid fake Mounjaro pens
Only buy Mounjaro from a pharmacy you can verify on the GPhC register. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified.7 Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Never buy from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.

Where genuine stock comes from
Mounjaro is manufactured by Eli Lilly. Genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies, and every pen carries a batch number and expiry date that can be checked.1 Our pens come through official LillyDirect supply, direct from Eli Lilly's UK distribution.
Checking your pen when it arrives
A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless.13 Do not use a pen that:
- has been frozen;
- looks cloudy or discoloured;
- has particles in it;
- is past its expiry date; or
- has been out of the fridge for more than 30 days in use, or has been above 30°C.
If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.13 Store unopened pens in the fridge until first use, and they keep until the printed expiry date; for anything beyond that, read the patient information leaflet and ask the pharmacy.
If something goes wrong
Report suspected side effects or suspect products through the MHRA Yellow Card scheme.7 Our support team is available Monday to Friday, 9am to 5pm, by email, account portal chat or phone, and the prescribing team provides side-effect support as part of ongoing reviews.
Price should never override these checks. A pen offered far below the prices you see across reputable providers, from a seller who does not ask about your health, is the scenario the MHRA warnings describe.
11 · Using pens as designed
Using each pen as designed and managing cost safely
Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.1 Each KwikPen is designed and licensed to deliver exactly four weekly doses.1 Use the pen as designed, then dispose of it.
When costs feel high, it is tempting to look for ways to make a pen last longer. Making a pen last longer is not licensed; no regulated UK pharmacy or prescriber can endorse it, and we do not.
Safe ways to bring cost down
Practical things you can do within the licence:
- Use the first-order discount of £40 with code FIRST40 on your first order, and the automatic £25 reorder discount on every later order (code DOSE25).
- Use Klarna or pay-monthly options to spread the cost; there is no subscription lock-in.
- Plan the year in advance using the year totals.
Moving between doses
Questions such as "can I go from 5mg to 10mg?" are for your prescriber, who decides the next dose at your review based on your response and tolerability. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. Do not decide a jump yourself, and do not combine pens of different strengths.
Stopping also needs planning. After a final dose, tirzepatide takes roughly 4 to 5 weeks to clear the system, with nothing needing to be done while it clears.1 If you are thinking of stopping because of cost, tell your prescriber first, so a planned approach with diet and activity support can be put in place.
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
