Licensed pharmacies then buy through Lilly's official distribution, and the terms of those purchases are confidential. That is why you will never see a single "trade price" quoted by every pharmacy, and why published retail prices vary so much.

For you, the number that matters is the retail price on the checkout page. Across the UK pharmacies we track, the cheapest and dearest Mounjaro pens at any dose are £143.96 and £339.00 (checked 10 October 2026). How to compare prices without being misled

Compare like with like: the same dose, one pen of four weekly doses, and the same total at checkout. A low headline can hide a separate fee or a different dose. 04 · What trade price means

What "trade price" actually means

A trade price is the price a manufacturer or wholesaler charges a pharmacy for a medicine. It sits one step before the retail price a patient sees. For Mounjaro, the manufacturer is Eli Lilly, and genuine UK stock is supplied through Lilly's official distribution to licensed pharmacies. Every pen carries a batch number and expiry date that can be checked.

There are really three numbers in the chain, and people often mix them up. The first is the list price, which is public and which Lilly sets by dose. The second is the net price an individual pharmacy pays after confidential discounts and terms. That figure is not public. The third is the retail price, which is what you see on the product page and pay at checkout.

That is why a phrase like "trade price" can mislead. Some websites quote the list price as if it were what every pharmacy pays. Others quote a retail price as if it were wholesale. Neither is a precise measure of what a pharmacy spends. When a page claims to know the exact wholesale cost of a dose for a specific pharmacy, treat it with caution: that figure is not publicly knowable.

The trade price does not include anything beyond the pen itself. A pharmacy still has to pay for a prescriber's review of your consultation, pharmacist dispensing, cold-chain packaging, a sharps bin and injection guidance, delivery, and customer support. A fair retail price has to cover those, so a direct comparison of list price and retail price is not a fair test of whether a pharmacy is "marking up".

Who can buy at trade price? Only licensed pharmacies and wholesalers dealing through authorised supply routes. A private individual cannot order a pen at list price, and a seller offering to supply without a consultation or prescription is not operating legally. Mounjaro is prescription-only, supplied after an online consultation reviewed by a GPhC-registered prescriber.

05 · The September 2025 increase

The September 2025 list-price increase

Eli Lilly announced a UK list-price increase for Mounjaro on 14 August 2025, effective 1 September 2025.10

Two things follow from that. First, the rise was not uniform: the biggest rise fell on the highest dose. Second, the list price is the manufacturer's price, so retail prices did not all move by the same amount, because pharmacies buy on confidential terms.

You may see headlines about prices "spiking" or "doubling". Those usually describe the list change, not what each patient was charged. Each pharmacy's own purchase terms are confidential, so the live retail price on the day you order is the figure to rely on, rather than any estimate of what a pharmacy pays for a particular dose.

It also helps to separate the price rise from the medicine. Mounjaro is tirzepatide, a once-weekly injectable made by Eli Lilly, licensed in the UK for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

06 · Why retail prices differ

Why retail prices differ between pharmacies

Retail prices differ because pharmacies buy on confidential terms and choose how to price and bundle their service. Across the UK pharmacies we track, the same dose can differ by between £27 and £41 from one provider to another (checked 10 October 2026). That gap is the clearest evidence that list price alone does not set the retail price.

Several things sit behind the spread. Purchasing terms are one. Another is what is included: some providers may charge separately for consultation or delivery, or require a paid plan. We make the consultation free and delivery free next-day. A third factor is how discounts are structured. Price transparency is another reason for the spread. Some sites advertise a low "from" price for the 2.5mg pen that applies only to the first pen. If you are modelling cost, look at the dose you are likely to be on for most of the year, not the headline dose. Location is a smaller factor than people expect. 07 · Planning a year of cost

Planning a year of Mounjaro cost

The honest way to think about cost is across the whole treatment, because the dose changes. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A new customer's first order is £40 less, and every later order is £25 less.

A first year is 13 pens. At standard prices, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. If you settle somewhere in between, use that dose's monthly price: £279.99 at 7.5mg, £290.99 at 10mg or £319.99 at 12.5mg. These are monthly prices, not year totals. Discounts reduce each figure in the same way as above.

Do higher doses mean more weight loss? In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages and individual results vary.2 For someone starting at 100 kg, 20.9% is about 21 kg. Your prescriber decides the dose that suits you.

08 · Cheap can be dangerous

When a low price is a warning sign

A price far below the market range should make you stop and check, because fake pens exist. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation.

When someone offers a pen at a fraction of the range across the UK pharmacies we track, ask how it was sourced. A seller offering to supply without a consultation or prescription is not a legitimate pharmacy, however attractive the price looks.

A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C. In use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, then it must be thrown away. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.13

If you notice a side effect after any pen, report it through the MHRA Yellow Card scheme and tell your prescriber. Reporting helps regulators spot problems with falsified products. Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.

09 · NHS versus private

NHS and private: where trade price fits

Trade price only affects private buyers directly. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6

If you qualify for NHS treatment, your NHS prescription charge in England is £9.90 per item unless you are exempt. If you do not, or you do not want to wait, private treatment is the alternative.

Private eligibility is set by the licence rather than the NICE criteria. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1

So how does the list price relate to the NHS? It does not change what an NHS patient pays: the NHS route has its own arrangements, separate from the retail prices private patients see. A private price and the NHS route are not directly comparable, so ask your GP about NHS access rather than weighing it against a pharmacy price.

A practical way to decide: ask yourself whether you meet a current NHS group and whether you can wait. If yes to both, speak to your GP. If you meet the licensed criteria but not an NHS group, or the wait is a concern, a private consultation is a legitimate route. It is also worth knowing that moving between the two routes is possible. If you start privately and later become eligible on the NHS, speak to your GP about it. Keep your dosing history and your clinical review notes handy so any new prescriber understands where you are.

Tirzepatide activates two gut-hormone receptors at once, GIP and GLP-1, which together help regulate appetite, slow how quickly the stomach empties and improve how the body responds to insulin. Is there a direct comparison with semaglutide? SURMOUNT-5 is the first large head-to-head trial of tirzepatide and semaglutide for weight loss.15 Trial averages do not predict your own result.

After the last dose, tirzepatide has a half-life of about five days, so it takes roughly 4 to 5 weeks to clear the system. Nothing needs doing while it clears. The decision about when to stop belongs with your prescriber.
