The honest decision at this dose is whether to stay put or move up, and that depends on three things you can describe to your prescriber: how your appetite feels between meals, how you are tolerating the medicine, and whether your weight and measurements are moving in the right direction over several weeks. A single quiet week is not a reason to change dose.
Tirzepatide helps regulate appetite and slows how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 If that effect is still clearly present and you are losing weight steadily, there may be no reason to change. If appetite has crept back and side effects are mild, your prescriber may discuss a higher strength. If side effects are troublesome, staying on the current strength for longer is a legitimate choice.

Why 7.5g is really 7.5mg
Mounjaro is never dosed in grams, so "7.5g Mounjaro" almost always means the 7.5mg KwikPen. The mistake is easy to make because phone keyboards, forums and social posts blur the units, and because a gram is a thousand times larger than a milligram. Always check the strength printed on your pen and dispensing label, and make sure you use the unit milligrams (mg) in your own notes.
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Those milligram figures are the strength of the pen, which is what your prescriber chooses.
Millilitres cause a second confusion. Some injectable medicines are described by concentration, such as liraglutide, which is supplied at 6mg/ml and is a daily injection, unlike once-weekly Mounjaro. For Mounjaro, the thing you set and check is the strength printed on the pen and on your dispensing label, not a volume you draw up yourself. The KwikPen is a pre-filled device, so there is no syringe to fill and nothing to measure out.
A practical habit helps. When you log your injections, write the strength as milligrams, the date, and the injection site, and keep the same format every week. Where 7.5mg sits on the dose ladder
The 7.5mg pen is the third of six strengths, so it is a mid-ladder step rather than a starting or a ceiling dose. The ladder runs 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
The 2.5mg pen is the starting strength, and your prescriber steps you up from there. If you step up after at least 4 weeks on each of 2.5mg, 5mg, 7.5mg and 10mg, your first four pens cost £959.96 at standard prices; a new customer's first order is £40 less and every later order £25 less. It is a framework, not a promise: some people stay on a strength for longer, and some find a lower dose is enough.
Why pause at 7.5mg? The reasons are usually practical. Perhaps appetite is well controlled and weight is moving, so there is no need to go higher. Perhaps the previous step caused side effects that have only just settled. Perhaps you are managing other health conditions and your prescriber wants a steadier approach. All of these are valid, and nobody is obliged to reach the top strength; the right dose is the one that works for you with tolerable side effects.
Not every person ends up at 7.5mg. Others continue to 10mg or beyond. The maintenance dose is individual.

06 · How it works
How tirzepatide works at this dose
Tirzepatide works by acting on two gut-hormone pathways at once, and that action is present at every strength, including 7.5mg. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1 That is the whole mechanism in plain terms: the medicine changes appetite and fullness, and you use that change to eat less in a sustainable way.
What the mechanism does not do is remove the need for the other half of treatment. Mounjaro is used alongside a reduced-calorie diet and increased physical activity. If the diet does not change, there is less for the medicine to support. This is the single most useful idea for anyone wondering why the same dose seems to work well for one person and less well for another.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 If you stop treatment, the effect on appetite fades gradually over several weeks rather than ending overnight, so changes in hunger after stopping will build slowly.
Because the medicine works through appetite and stomach emptying, the sensations you notice are often related to meals: feeling full after small portions, or finding rich or large meals uncomfortable. Pay attention to these signals. They are information about how your body is responding at this dose, and worth passing on at your monthly review.
None of this means more is always better. The point of titration is to find the lowest dose that gives the effect you need with side effects you can live with. For many people that is a mid-range strength, and for others it is higher. Your prescriber will weigh up your progress rather than aiming for a particular number on the ladder.
07 · What trials show
What the trial results say
Published obesity trial results are reported for 5mg, 10mg and 15mg, not for 7.5mg. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result, and they should not be read as a forecast for 7.5mg specifically, since that strength sits between the figures reported. What the pattern does show is that the strengths either side of 7.5mg were associated with substantial average weight loss, and that higher strengths were associated with larger average losses in that trial.
The spread of outcomes matters as much as the average. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Most people in the trial lost a meaningful amount at every strength studied, but a smaller proportion lost a great deal, and some lost far less than the average.
To make the 20.9% figure concrete: for someone starting at 100 kg, 20.9% is about 21 kg.
The trial also shows why time on treatment matters. Results built up across the full 72 weeks, which means that judging a dose after a few weeks is judging it too early. A dose should be assessed over several weeks of steady use, alongside your diet and activity, rather than on the scales from one week to the next.
A separate maintenance trial, SURMOUNT-4, looked at continued treatment for maintaining weight reduction, which is why people are often advised to think of treatment as a longer-term plan rather than a short course.14 Your prescriber can talk you through what that means for your situation.
08 · If weight loss stalls
If weight loss stalls on 7.5mg
If weight loss slows on 7.5mg, the first step is to review the basics with your prescriber, not to change dose on your own. A plateau is common in any weight-loss effort, and the honest explanation is usually a mix of factors rather than a single cause. Start with a calm audit of the last few weeks.
Consider these questions in turn:
- Time on dose: have you been on 7.5mg long enough for it to show its effect? Judging after only a short spell is premature.
- Injection routine: are you injecting on the same day each week, following the pen's instructions, and storing the pen correctly?
- Appetite signals: do you still feel fuller sooner and less hungry between meals? If that has faded, tell your prescriber.
- Diet: is the reduced-calorie diet still in place? Liquid calories, snacking and larger portions can quietly absorb the effect of reduced appetite.
- Activity: has movement dropped away, for example because of a busy period or low energy?
- Side effects: are symptoms making it harder to eat well or stay active?
- Other medicines and conditions: has anything changed in your health or prescriptions?
Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 That is why the review starts with habits. If the audit finds nothing obvious and appetite control has genuinely weakened, your prescriber may discuss whether a higher strength is appropriate. If it has not, patience and routine often matter more than a bigger dose.
The safer levers are the ones already named: consistent routine, a sustainable calorie reduction, regular activity, sleep and good communication with your prescribing team.
Scales are a blunt tool. Clothes fit, measurements and how you feel can improve even when the number on the scales sits still for a time. 09 · NHS or private
NHS or private access
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
If you are already on the NHS pathway, your dose is set by the NHS prescriber and you should follow their plan. If you are considering a private route, the dose ladder is the same: the same pens and the same strengths, with 7.5mg a mid-ladder step. What differs is how you get there and who reviews you.

11 · Safety and storage
Safety, storage and what to report
Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary.
Side effects are listed in the patient information leaflet supplied with every pen, which also says which symptoms need urgent medical attention.13 Read it before your first injection and again when you move to a new strength. If you notice a new symptom that you are unsure about, speak to your prescriber, and you can report suspected side effects through the MHRA Yellow Card scheme.19
Follow the leaflet and keep eating and drinking normally. If nausea or vomiting is persistent or you cannot keep fluids down, contact your clinical team — dehydration needs prompt attention.
Oral contraceptives deserve a specific mention. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows.7 If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so tell your prescriber about every medicine you use.
For storage, keep unopened pens in the fridge at 2–8°C until first use, and they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. If you have an unopened pen that has been out of the fridge and you are unsure, check the patient information leaflet and ask the pharmacy rather than guessing.13 Follow the pen's instructions for injecting, and dispose of used needles in the sharps bin supplied; needles, a sharps bin and injection guidance come with every order.

12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
