Why Mounjaro is a once-weekly injection
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
The weekly pattern is not a convenience choice. It reflects how long the medicine stays active in the body. A daily injection medicine, such as liraglutide, is built around daily dosing. Mounjaro is built around a weekly rhythm, so a regular gap between shots is part of how the treatment is designed to work.
Each pen holds several doses at the strength printed on it. You use the pen on your chosen day each week, and the pen's own instructions tell you how to set up and deliver each dose13.
Choosing your day is worth a moment's thought. Pick a day that suits your week, not one that clashes with regular travel or work. Some people prefer a day when they can rest afterwards if they feel a little queasy. Others prefer an evening so any mild side effects fall overnight. There is no clinically better day; consistency matters more than which day you choose.

04 · Why spacing matters
Why the gap between shots matters
Spacing matters because tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.
That long half-life has two practical consequences. First, each weekly injection adds to medicine that is still present from earlier doses, so levels build up gradually before settling. Second, an extra shot is not a clean 'top-up'. It lands on top of what is already there, which is why we advise against bringing doses closer together.
It also explains why a single slightly late dose is usually a matter of following the leaflet, not an emergency. Because the medicine clears slowly, levels do not drop to nothing overnight. That is reassuring, but it is not a licence to stretch intervals.
How Mounjaro works also relates to timing. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

If you have severe, persistent stomach pain, chest pain, swelling of the face, lips, tongue or throat, or difficulty breathing, get medical help straight away. 05 · Changing injection day
How to change your Mounjaro injection day
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.
Here is a sensible way to approach a change of day.
- Decide whether you really need to move. A holiday may only need your usual day, adjusted once.
- Check that at least 3 days (72 hours) have passed since your last dose, or ask your prescriber to confirm.
- Move to a day that keeps at least 3 days (72 hours) between doses, never closer.
- Once you move, treat the new day as your fixed day and stop adjusting.
- Note the change in your tracker or app so your reviewer sees it.
Do not move your day in several small steps over successive weeks. Each small shift changes the gap, and the effect adds up. A single, deliberate move is easier to reason about, easier to explain to your prescriber, and easier to follow.
If you want to move the day for a particular event, consider whether it can simply be handled by injecting on your usual day. Many people find the usual day works even when travelling, because the pen is portable once handled correctly.
People who take Mounjaro for type 2 diabetes alongside insulin or sulfonylureas should be especially cautious. Doses of those medicines may need adjusting because of low blood sugar risk, so tell your prescriber before you change any routine around them.
If you take an oral contraceptive, remember Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

06 · Late or missed doses
What to do if you are late with a shot
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day13. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.
The principles are straightforward and apply to almost everyone.
- Never double up. Two doses close together are not a substitute for a missed dose.
- Do not guess. Check the leaflet or ring the prescribing team.
- Record what happened, so your next review reflects reality.
- Return to your normal day afterwards unless the leaflet or prescriber advises otherwise.
Slow clearance helps explain why a late dose is rarely dramatic. Because the medicine lingers, appetite control does not vanish at once.

07 · Minimum time on treatment
Is there a minimum time on Mounjaro?
There is no fixed minimum time you must stay on Mounjaro, but weight-loss results are measured over many months. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
For a worked example, for someone starting at 100 kg, 20.9% is about 21 kg. That is a trial average, and trial averages do not predict your own result.
Your prescriber decides your maintenance strength.
Many people ask how soon they will see a change. Weight loss on Mounjaro is gradual by design, and the first weeks on 2.5mg are an adjustment phase. Do not judge treatment on the first few injections.
Stopping is also a decision for you and your prescriber. After a final dose it takes about four to five weeks for the medicine to clear, and nothing needs doing while it clears. Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year. 08 · Staying on 2.5mg
Appetite changes, 2.5mg and starting again
Many readers ask whether you can stay on 2.5mg and still lose weight, why appetite control sometimes fades, and whether treatment works the second time around.
Trial results are quoted for 5mg, 10mg and 15mg2. Some people feel an effect on appetite at the starting strength. Whether it is enough is a conversation for your prescriber, who can step you up if it is not and you are tolerating treatment well.
If appetite suppression seems to fade, consider the common, practical explanations before assuming the medicine has stopped working.
- Irregular spacing or missed doses changing the level in your body.
- Staying at one strength after your body has adapted, when a review might suggest a change.
- Eating patterns drifting back to old habits, because treatment supports a reduced-calorie diet rather than replacing it.
- Stress, sleep or illness altering hunger signals.
Raise it at your monthly review rather than changing anything on your own. Never bring shots closer together to recapture the effect.

09 · Pen storage and 30 days
Pen storage and the 30-day in-use limit
Unopened pens belong in the fridge until first use, and they keep until the printed expiry date. The temperature range for the fridge is 2–8°C. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit13.
Once you start using a pen, UK KwikPens are licensed for up to 30 days (not above 30°C) out of the fridge. After that, the pen must be thrown away.
Some tips for handling pens well:
- Keep the pen in its carton, away from heat and direct sunlight.
- Never use a pen that has been frozen or that you suspect has been mishandled.
- Take a new pen from the fridge only when you need it.
- Keep a note of the date you first used each pen.
- Dispose of used needles in the sharps bin supplied with your order; needles, sharps bin and injection guidance are included with every order.
When travelling, plan carefully so the pen stays protected from heat. A small insulated accessory is a low-cost way to help. Do not leave a pen in a hot car or in checked luggage where temperatures cannot be controlled.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
11 · NHS and private routes
NHS or private: who sets your schedule?
Whichever route you use, the dosing schedule is set by the licence and your prescriber, and the weekly spacing is the same. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46
