01 · The core answer

The standard gap between Mounjaro injections

The standard gap is a full week. Mounjaro (tirzepatide) is a once-weekly injectable medicine, and the schedule is built around that rhythm from your very first pen.1 You inject on the same day each week, at any time of day, with or without food.13

Each KwikPen contains four weekly doses. That means one pen lasts four weekly injections, and the pen itself will not let you dial more medicine than is left inside.13

02 · What if the gap is different?

Can the gap be shorter or longer than a week?

Only within the limits the licence sets. There is a firm floor on how close two doses can be, and a defined window for doses that run late. Anything outside those limits is not licensed.

On the short side, the product information says you must never take two doses within 3 days of each other. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.113

So can you go a longer gap, such as an extra few days beyond the usual week? If you have run late, you are in the late-dose situation, not a new schedule. Provided you are still inside the missed-dose window, you take the injection and carry on with your usual day. 04 · Minimum gap rule

What is the minimum gap between Mounjaro injections?

The minimum gap is the point at which two doses are too close together. The licence is clear: never take two doses within 3 days of each other.1 That is why a missed dose cannot be made up by injecting again soon after.

This floor exists because tirzepatide stays in the body for a long time between injections. It is not a number to test.

The same logic answers questions like whether you can inject on a much shorter cycle than weekly. The licensed schedule does not allow it. A consistent weekly day is the safest route to steady blood levels and predictable side effects.

05 · Changing injection day

How to change your Mounjaro injection day

You can change your injection day, as long as enough time has passed since your last dose. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

Put simply, you can bring your injection forward or move it later. If you bring it forward, there must still be at least 3 days between the two doses.13 After the change, you settle back into your normal once-weekly schedule on the new day.

Why might you want to change? Common reasons include:

  • You want side effects to fall on days off, so you move to a weekend.
  • A work rota, shift pattern or travel clashes with your current day.
  • You were late once and the new day suits you better than the old one.

Make the change once and then hold the new day. Repeatedly hopping the day around makes it harder to keep a consistent routine. A note in your phone or the free GLPTrackr companion app can help you keep the day consistent.

06 · Late or missed doses

What if you are late with your Mounjaro dose?

If your dose is only a little late, you will usually still take it. The rule is that you take a missed dose as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 That window is generous, which is why a modest delay is rarely a problem.

Beyond that window the answer flips. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day.13 You do not add the skipped dose to the next one, and you do not try to catch up.

A simple way to think about it:

  • Inside the window: inject as soon as you remember, then go back to your usual day next time.
  • Outside the window: skip it and inject on your normal day.
  • Never: inject extra to compensate, or inject again within the minimum gap.

07 · Why weekly spacing works

Why Mounjaro is injected once a week

Mounjaro is weekly because tirzepatide is cleared slowly. After a final dose it takes roughly four to five weeks to clear the system, and there is no way to speed this up. Nothing needs doing while it clears.1

That slow clearance is what lets a single injection keep working across the whole week.

Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

08 · Dose steps and spacing

How spacing relates to the dose ladder

Spacing between dose increases is a separate rule from spacing between injections. Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1

After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.

In practice that means you inject weekly the whole way, and each pen holds four weekly doses.

You cannot speed titration up by shortening the gap between injections. Doing so is outside the licence. Pace matters more than speed: losses are gradual by design, and trials measured results over 72 weeks, so think in months, not days.1

09 · Travel and storage

Keeping to your injection day when you travel

Travel is the commonest reason for an awkward injection day, and the storage rules make it easier than most people expect. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded.13

You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Most holidays need no special arrangements, though hot climates call for an insulated pouch. Check destination rules for travelling with prescription injectables before you go.13

If your trip will fall across your injection day, plan ahead rather than skipping. Inject before you leave, or on the day wherever you are, then keep your normal day. If travel changes the day, the day-change rule applies: leave at least the minimum gap since your last dose.

Unopened pens stay in the fridge until first use and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, as figures circulating online often come from other countries' labels.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
