MHRA and GLP-1 Medicines: What UK Patients Need to Know (October 2026)
The MHRA is the UK regulator that licenses GLP-1 medicines, monitors their safety and publishes alerts on them. Licensed GLP-1 and GIP/GLP-1 medicines such as Mounjaro and Wegovy are legal in the UK on prescription only, and the MHRA also runs the Yellow Card scheme for reporting side effects and counterfeits.7 Information current as of October 2026.

A quick overview
What to know
before you start.
-
The MHRA licenses, monitors and issues safety communications for GLP-1 medicines; it does not approve supplements or unlicensed sellers.
-
Mounjaro and Wegovy are prescription-only, so legal supply needs a consultation and a prescription from a registered prescriber.
-
MHRA contraception advice matters if you use the pill: Mounjaro can reduce how well oral contraceptives are absorbed.
-
Report any suspected side effect or counterfeit product through the Yellow Card scheme; it takes a few minutes and helps the regulator.
In this guide
01 · The regulator
What the MHRA does for GLP-1 medicines
The Medicines and Healthcare products Regulatory Agency is the UK body that licenses medicines, inspects supply routes and watches safety after launch. For GLP-1 treatments, that covers three jobs: deciding whether a product is licensed, publishing safety advice, and running the Yellow Card scheme where patients and professionals report problems.7
It helps to separate the MHRA from other bodies. NICE decides whether the NHS should fund a treatment. The GPhC regulates pharmacies and pharmacists. The MHRA regulates the medicine itself. A product can be licensed by the MHRA and still not yet be widely funded on the NHS, which is the situation many people face with weight-management injections.
If you see the phrase "MHRA approved" on a website, check what it is attached to. A licensed medicine has a Summary of Product Characteristics and a patient leaflet. A supplement marketed with dramatic weight-loss claims does not have either, whatever the page says.
02 · Legality
Are GLP-1 medicines legal in the UK?
Yes, licensed GLP-1 medicines are legal in the UK, but only as prescription-only medicines supplied after a proper clinical assessment. Mounjaro is the brand name for tirzepatide, a once-weekly injectable made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1
What is not legal is supply without a consultation or prescription, or the sale of unlicensed products dressed up as the real thing. A seller offering injections with no clinical review is outside the rules, and the product may be counterfeit. Our own route is a free online consultation reviewed by a GPhC-registered prescriber, dispensing by our registered pharmacy (GPhC 9012878) and Mounjaro supplied through official LillyDirect supply from Eli Lilly's UK distribution.
You can check any UK pharmacy on the GPhC public register. That check takes a minute and is the single most useful thing you can do before buying online.
03 · Next step
What to do with this information
If you are only researching, the practical takeaway is simple: use a regulated route, read the leaflet, and know how to report a problem.
If you want the Mounjaro-specific version of this topic, see our guide to MHRA guidance on Mounjaro.
04 · Licensing and labels
How licensing shapes what you are told
Every licensed GLP-1 medicine has two core documents: the Summary of Product Characteristics, written for prescribers, and the patient information leaflet, written for you. Both are reviewed by the regulator and updated when new safety information emerges. They are the authoritative source for side effects, interactions and how to use the pen.113
When an MHRA safety communication is published about the class, the safest approach is to read the notice itself on the government website and then talk to your prescriber about whether it affects you.
Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Wegovy is a separate licensed semaglutide product with its own label and dose ladder of 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg. Each is regulated on its own evidence, so a warning for one product is not automatically a statement about another unless the regulator says it covers the class.
Unlicensed or "research" versions of these molecules have no such label, no batch oversight and no accountable prescriber. That gap is the main reason the regulator warns the public about them.
05 · MHRA and contraception
MHRA advice on contraception with GLP-1 medicines
The contraception point is one of the clearest practical items in the guidance for this class, and it matters because many adults using these medicines are of childbearing age. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only — implants, coils and injections are unaffected.17
In plain terms, the risk is tied to the pill, and it is tied to the moments when the dose changes. Because Mounjaro is stepped up in stages, those windows recur during titration, so it is worth planning your method with your prescriber before you begin rather than after a dose change.
Pregnancy is a separate point. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.113
On breastfeeding, current prescribing information indicates minimal transfer of tirzepatide into breast milk; discuss breastfeeding with your prescriber so the decision reflects your circumstances.1
If you have polycystic ovary syndrome, Mounjaro is not licensed specifically for it, but many women with it meet the licensed weight-management criteria. Mention PCOS and any fertility plans in your consultation, because improving weight can restore ovulation.1
06 · Yellow Card reporting
The Yellow Card scheme: how to report a problem
The Yellow Card scheme is the MHRA's system for collecting reports of suspected side effects and of faulty or fake medicines, and anyone can use it: patients, carers and healthcare professionals.19 You do not need to be certain the medicine caused the problem. A suspicion is enough.
Reports are not a verdict on a single person's case. They are pooled so the regulator can spot patterns that individual trials were too small to show. That is how the safety wording on labels changes after a medicine has been in wide use. A report you file today can contribute to tomorrow's label.
When you report, it helps to have to hand the brand name, the strength, the batch number printed on the pen box, when you started and when the symptom began. Tell your prescriber as well. Reporting to the MHRA does not replace medical care, so get medical help straight away for severe or worrying symptoms.
The scheme also takes reports of suspected counterfeit products. If a pen looks different from what you expected, or the packaging, labelling or supply route seems wrong, do not use it. Keep the packaging, contact the seller and report it.7
For a symptom that is not on the full list in the UK patient leaflet, tell your prescriber about any new or lasting symptom, and report suspected side effects through the MHRA Yellow Card scheme.
07 · Counterfeits and alerts
Counterfeit alerts and how to avoid them
The MHRA publishes counterfeit medicine alerts, and GLP-1 injections are an obvious target because demand is high and prices vary between sellers.7 The risk is not only that a fake does nothing. It may contain the wrong substance, the wrong amount, or nothing safe to inject.
These are the practical warning signs, none of which depends on knowing a specific alert:
- The seller offers to supply without a consultation or prescription.
- The price is far below what regulated pharmacies charge, or there is pressure to buy quickly.
- The seller cannot show a GPhC registration you can verify on the public register.
- The product is sold as a vial, powder or "research peptide" rather than as a licensed pen in its original packaging.
- The website promises specific results or uses dramatic weight-loss claims.
Compare that with a regulated route. The seller is on the GPhC register, a prescriber reviews your answers, the medicine comes from the manufacturer's distribution chain in sealed packaging, and you receive the patient leaflet. Our Mounjaro is supplied through official LillyDirect supply, as genuine KwikPens, and ships in temperature-controlled packaging with free next-day tracked delivery.
Price comparison is sensible, but price alone is a weak safety signal.
08 · NHS and private access
NHS and private access to licensed GLP-1 treatment
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions). Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.46
That answers a common question about qualifying. Your GP or local NHS service decides NHS eligibility against the local criteria, so the first step is to ask them. NHS access to Wegovy runs through specialist weight-management services; ask your GP.
Private treatment follows the licence rather than local funding rules, but it is not a shortcut around clinical judgement. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Repeat orders re-check suitability, and there is no auto-shipping.
Whichever route you choose, the medicine is the same regulated product, and the same MHRA safety communications and Yellow Card scheme apply.
09 · Evidence behind the licence
The evidence the MHRA weighs
A licence rests on trial evidence for benefit and for safety. For Mounjaro's weight-management use, the main pivotal study is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 A trial average does not predict your own result, and it says nothing about unlicensed products that were never tested.
The mechanism helps explain both the benefit and several label warnings. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Slower stomach emptying is the reason behind the contraception advice.
If you are curious about the biology, our guide to GLP-1 physiology goes deeper. For how people stay on treatment in practice, see GLP-1 adherence.
10 · Supplements and look-alikes
Supplements, capsules and "natural GLP-1" claims
Products sold as natural GLP-1 boosters are not licensed GLP-1 medicines, and the MHRA licence for a prescription injection does not extend to them. They are low-cost supplements with no clinical oversight, and dosing is not standardised. Marketing that borrows the name of a prescription medicine is a red flag, not a reassurance.
Capsules and powders described as GLP-1 products raise the same question: is there a licence, a leaflet and a prescriber behind it? GLP-1 smoothies covers food-based approaches. Food can support appetite control, but it is not a substitute for a medicine, and it is not regulated as one.
The same applies to research areas. Scientific interest in GLP-1 biology, for example in GLP-1 and remyelination, concerns laboratory and early research. It is not a licensed use, and nobody should self-treat on the strength of it.
If an advert says a product works "like" a prescription injection, treat that as a prompt to ask who is accountable if something goes wrong. With a licensed medicine, the answer is a named manufacturer, a registered pharmacy and a prescriber. With many online products, there is no answer.
11 · Choosing a safe provider
Checking a provider against MHRA and GPhC standards
A safe provider is one you can verify, that asks real clinical questions and that explains what happens after the box arrives. Use this checklist before you pay anyone.
- The pharmacy is on the GPhC register, with a registration number you can look up.
- The consultation is reviewed by a registered prescriber, not scored by a script.
- The medicine comes from the manufacturer's supply chain, in original sealed packaging.
- You get the patient leaflet, needles, sharps bin and injection guidance.
- There is aftercare: monthly reviews, dose-titration support and a way to reach a person.
- Prices are shown clearly and the price shown is the price paid.
On our side, the consultation is free with no obligation, delivery is free and next-day tracked, with same-day dispatch when you order by 3pm, and support is available Monday to Friday, 9am to 5pm, by email, portal chat or phone on 0333 207 0413. You can also use the free GLPTrackr app to log your journey.
Dispensed prescription medicines cannot be returned except where faulty, damaged or the wrong item, so contact us within 3 days if something is wrong. And if you are weighing another licensed option, the Wegovy pen is a separate semaglutide product with its own label.
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
