01 · The core answer

What are GLP-1 capsules, and are they the real thing?

GLP-1 capsules are almost always supplements. They are sold with names that borrow the language of prescription medicines, and the labels often imply a similar effect. They are not licensed GLP-1 medicines and they do not contain the active ingredients that were tested in the large weight-management trials.

A genuine GLP-1 medicine is a prescription-only product. It is supplied after an assessment and it carries a patient information leaflet, a Summary of Product Characteristics and a licence for a specific use. A capsule that you can add to a basket with no consultation has none of that behind it.

That does not mean every supplement is dangerous. It means that the marketing phrase 'GLP-1' on a capsule tells you very little about what is inside, how it was made or whether it does anything useful. Dosing for these products is not standardised, so you cannot compare one tub with another.

02 · Your decision

What should you do if you want real GLP-1 treatment?

The practical route is a prescriber-led one. Mounjaro is a once-weekly injectable prescription medicine made by Eli Lilly. It is licensed in the UK for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

03 · How GLP-1 medicines work

How do real GLP-1 medicines work?

Prescription GLP-1 medicines mimic natural gut hormones that regulate appetite. The mechanism is well described for Mounjaro. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).1

The point for anyone looking at capsules is that these effects belong to specific, tested molecules at specific, controlled doses. A capsule that merely uses the phrase 'GLP-1' in its name has not been shown to reproduce them.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1

04 · Do GLP-1 pills work?

Do GLP-1 pills actually work?

Licensed GLP-1 tablets are prescription medicines with their own product information, and they are the version of 'GLP-1 pills' that has regulatory backing. Supplement capsules sold under the same label are a different category. Read the product information for the specific tablet, or ask a prescriber.

For the injectable we do supply, the main trial evidence is SURMOUNT-1. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Those results came from a licensed medicine given as a controlled injection with medical oversight. They tell you nothing about a supplement capsule, and no supplement capsule can borrow them. If a seller quotes dramatic weight-loss claims for a capsule and points to GLP-1 trials, the trials were not of that product.

A fair reading is simple. Medicines tested in trials have measured results. Supplements without such trials have marketing.

A plant extract with a catchy nickname such as 'nature's Ozempic' is not the medicine it is named after.

Yes, licensed GLP-1 tablets exist in the UK, but they are prescription-only and they are tablets, not capsules. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) each have a Summary of Product Characteristics on the electronic medicines compendium.2021

Whichever format you choose, the decision should be made with a prescriber. The right option depends on your health history, other medicines and how you feel about injections versus tablets.

07 · Who should not take them

Who should not take GLP-1 pills or injections?

Suitability is decided case by case, and that is the main reason a prescriber reviews you first. Mounjaro is licensed for adults, and it is used alongside a reduced-calorie diet and increased physical activity.1 You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

  • Your full medical history, including any digestive, pancreatic, gallbladder or thyroid conditions you have been told about.
  • Every medicine you take, including diabetes treatments, and any supplements.
  • Whether you are pregnant, breastfeeding or planning a pregnancy. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. Switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.
  • Your height and weight, and any previous weight-loss medicines you have used.

For supplements marketed as GLP-1 capsules, the same caution applies. Ingredient lists can be unclear and no assessment takes place, so there is no one checking whether a product suits your health. If you already take prescribed medicines, speak to a pharmacist or prescriber before adding any supplement.

08 · Side effects and safety

What are the long-term side effects of taking GLP-1?

Like all medicines, Mounjaro can cause side effects, and most are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. The leaflet lists known side effects and what to do about them.13

For any new symptom, speak to your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme, which applies to medicines and supplements alike.19 A symptom that is not on the leaflet list is not a listed side effect, and your prescriber should still hear about it.

09 · Living with treatment

What can you not do, and what not to eat, on GLP-1?

Because these medicines slow how quickly the stomach empties, many people find that very large or very rich meals feel uncomfortable, and that smaller, simpler meals are easier. Your prescriber can give advice that fits your own symptoms.

Practical habits that people often discuss with their prescriber include:

  • Eating slowly and stopping when you feel full.
  • Keeping hydrated through the day.
  • Noticing which foods bring on discomfort and talking about them at your review.
  • Not treating the medicine as a replacement for diet and activity.

A related question is storage and use. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Do not use a pen that has been above 30°C or has been frozen. For anything beyond that, follow the pen's instructions and the leaflet.

10 · Losing weight too fast

What happens if I lose weight too fast on GLP-1?

If you feel your weight is dropping faster than expected, or you feel unwell, tell your prescriber rather than waiting. Trial figures are averages only, and trial averages do not predict your own result.2

What matters is how you feel and whether you are eating enough to stay well. Warning signs worth raising with your prescriber include feeling constantly weak or ongoing sickness. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away. A prescriber can review your dose, which is exactly what monthly reviews and titration are for.

Dose changes should only be made by the prescribing team.

Where capsule supplements are concerned, there is no titration, no review and no one watching your progress. That absence of oversight is one of the biggest differences between a supplement and a prescribed medicine.

11 · Medicines to mix with care

What medications should you not mix with GLP-1?

The honest answer is that it depends on the medicine, and the patient information leaflet is where interactions are listed.13 Because Mounjaro slows stomach emptying, it can affect how quickly some oral medicines are absorbed, particularly when you start treatment or increase the dose. Tell your prescriber about everything you take, especially warfarin or other narrow-margin medicines, insulin, or sulfonylureas (where doses may need reducing to avoid low blood sugar).

What you should do is declare everything you take when you consult a prescriber. That includes prescribed medicines, over-the-counter products, herbal remedies and supplements, including any capsule sold as a GLP-1 product. Some medicines, especially those used for diabetes, may need their doses reviewed when you start treatment, which is a decision for your prescriber.

Do not stop or change any existing medicine on your own because you have started something new. Speak to the prescriber or your pharmacist first.

12 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

13 · NHS and private access

Is GLP-1 treatment available on the NHS?

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

NHS access to Wegovy runs through specialist weight-management services; ask your GP.
