Metformin vs Mounjaro: which one is for you?

Neither medicine is simply "better"; they are different treatments and your prescriber decides which suits your health profile. Mounjaro is a weekly injection licensed for weight management in adults and for type 2 diabetes.¹ Metformin is a separate medicine with its own patient leaflet and its own prescribing route, usually through your GP or diabetes team.

If you already take metformin, do not change or stop it yourself. Raise it with whoever prescribes it, and list it during your Mounjaro consultation so the prescriber reviewing your answers has the full picture.

Online, nicknames such as "poor man's Ozempic" are used for cheap alternatives. Marketing nicknames are not clinical evidence, and they do not tell you whether a medicine suits you. Supplements sold as "natural GLP-1" are not medicines and have no comparable trial evidence.²⁰

03 · How to decide

Decision factors to work through

The first factor is your goal. If your aim is licensed weight management, Mounjaro and Wegovy are the licensed injectable options, and a prescriber will check whether you meet the criteria. If your aim is managing type 2 diabetes, your GP or diabetes team leads, and Mounjaro is licensed for type 2 diabetes.

The second factor is what you already take. Mounjaro slows stomach emptying, so it can change how quickly some oral medicines are absorbed.¹ That is why a full medicine list matters at consultation.

The third factor is your history. A history of pancreatitis, gallbladder problems, diabetic retinopathy or severe digestive disease needs careful review at consultation, and so do pregnancy plans.¹³ If you ever develop severe, persistent stomach pain, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or swelling of the face or throat, get medical help straight away.¹³

04 · Taking both together

Can you take metformin with Mounjaro?

Whether you can take metformin and Mounjaro together is a decision for your prescriber, based on your full medicine list and health history. Mounjaro is licensed for type 2 diabetes as well as weight management, so prescribers do consider it for people with diabetes.¹ Whatever you are prescribed, do not change or stop a medicine you already take just because you are starting another.

At consultation, list everything: prescription medicines, over-the-counter products and supplements. Mounjaro can affect how quickly some oral medicines are absorbed, particularly when you start or increase the dose.¹ Severe vomiting or diarrhoea can cause dehydration, so keep fluids up and get medical help straight away if you cannot keep fluids down.

05 · Insulin and low blood sugar

Insulin, sulfonylureas and low blood sugar

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar. Typical signs are shakiness, sweating and confusion, and your prescriber may need to adjust those medicines.¹ This does not mean the medicines cannot be combined; it means the doses need supervision.

If you use a glucose monitor such as a Freestyle Libre, share the readings with your diabetic nurse or prescriber, especially in the weeks after a dose increase. Do not adjust diabetes medicines yourself.

Narrow-margin medicines such as warfarin also need a mention at consultation. No adjustment is needed for paracetamol.¹

06 · How Mounjaro works

How Mounjaro works for weight and blood sugar

Mounjaro activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.¹ In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

That dual action distinguishes it from single-action GLP-1 medicines such as semaglutide. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. Nothing needs doing while it clears.¹

Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

07 · What the trials show

What the weight-loss trials show

In SURMOUNT-1, 2,539 adults taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.² For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, and they do not predict your own result.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20%.²

The first large head-to-head trial of tirzepatide and semaglutide for weight loss was SURMOUNT-5. Adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy). Both are effective, licensed treatments, and which suits you depends on your health profile, tolerance and prescriber advice.¹⁵

08 · Beyond the scales

Blood sugar, blood pressure and other changes

In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.² These are trial-average secondary outcomes, not promises.

They are why prescribers talk about weight management as health management rather than appearance. Your own results will depend on your health, your dose and your lifestyle, and your prescriber reviews progress with you at follow-up.

09 · Side effects and safety

Side effects and when to seek help

Most Mounjaro side effects are digestive. The most common, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort. Common effects (up to 1 in 10) include indigestion, reduced appetite, dizziness, reflux, burping, hair shedding and injection-site reactions.¹ They are most likely when starting or moving up a dose, and for most people they ease within days to a few weeks.

Serious side effects are uncommon but need urgent attention:

• Get medical help straight away for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis).
• Get medical help straight away for pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems).
• Get medical help straight away for signs of a severe allergic reaction, such as swelling of the face or throat and difficulty breathing.

10 · Contraception and pregnancy

Contraception, pregnancy and PCOS

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method such as condoms, for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. This applies to the pill only; implants, coils and injections are unaffected.¹⁷

Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.¹³ If you are planning a pregnancy, speak to your prescriber before starting. 11 · Who should not take it

Who should not take Mounjaro

You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, or during pregnancy or while trying to conceive. Your prescriber will take extra care, or may advise against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy or gallbladder problems.¹³

In rodent studies tirzepatide caused thyroid C-cell tumours. It is not known whether this applies to humans, and no increase has been shown in human trials. Tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.¹

Mounjaro is not known to damage the kidneys or liver directly, but dehydration from severe vomiting or diarrhoea can strain the kidneys. Keep fluids up and contact your clinical team if you cannot.¹ Tell your prescriber about any history of depression too. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link with GLP-1 medicines, but big changes in weight and routine can affect how you feel. Seek help promptly if your mood drops or you have distressing thoughts during treatment.¹⁷

Tell your prescriber about any change in your mood while taking Mounjaro.

12 · Storing your pen

Storing Mounjaro pens

Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away.¹³

Do not use a pen that has been above 30°C or has been frozen. Keep the pen cap on to protect it from light. Needles, a sharps bin and injection guidance come with every order.

13 · Stacking and alternatives

Combinations to avoid and other options

Mounjaro must not be combined with other GLP-1 medicines, unlicensed "research" peptides such as retatrutide (which has no UK licence), or compounded copies. These combinations have no safety data and no regulated supply chain.¹ Anything promising faster results outside a licensed pathway is a warning sign.

There is no pill version of Mounjaro; tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed: Wegovy tablets, approved by the MHRA in June 2026, and Foundayo (orforglipron), approved in August 2026. They are different medicines with their own dosing and eligibility checks.²⁰²¹ Orlistat is a tablet that blocks some dietary fat absorption and produces smaller average losses. Ozempic is semaglutide licensed for type 2 diabetes, not a UK weight-loss medicine, and a regulated UK pharmacy will not supply it for weight loss.

14 · NHS or private

NHS or private access to Mounjaro

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).⁴⁶ Full rollout is expected to take years, so many people who meet the licensed criteria choose regulated private treatment.
