Is the NHS a lower-cost route?

Mounjaro is available on the NHS, but access is being phased in over several years.⁴ NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.⁶ If you qualify and your area offers it, the NHS prescription charge in England is £9.90 where it applies. Many people who meet the licensed criteria choose regulated private treatment rather than wait.

What are you actually paying for?

You are paying for a licensed once-weekly injectable medicine, plus the clinical review that makes it safe to use. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.¹ It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.

Each pen is designed and licensed to deliver exactly four weekly doses, so one pen covers four weeks. Our weekly equivalents are £42.50 at 2.5mg, £47.50 at 5mg, £61.25 at 7.5mg, £68.75 at 10mg, £72.50 at 12.5mg and £75.00 at 15mg, all at standard prices.

06 · How it works

How does Mounjaro work?

Mounjaro works by activating two natural gut-hormone receptors at once, GIP and GLP-1. Tirzepatide is the first treatment of its kind to do this. Together these receptors help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.¹

In practice, most people feel fuller sooner and less hungry between meals. That supports a reduced-calorie diet, which is the point of the medicine: it is used alongside a reduced-calorie diet and increased physical activity, not instead of them. This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide, which is sold as Wegovy and Ozempic.

Why does the mechanism matter on a price page? Because it explains what you are buying and why the medicine is dosed in steps. Treatment starts at 2.5mg and moves up through 5mg, 7.5mg, 10mg, 12.5mg and 15mg as your prescriber judges appropriate.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly 4–5 weeks to clear the system.¹ There is no way to speed this up, and nothing needs doing while it clears.

If you are comparing semaglutide, Wegovy pens are 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly. The dose steps up after 4 weeks on each of 0.25mg, 0.5mg, 1mg and 1.7mg, so the 2.4mg maintenance dose starts from week 17, and 2.4mg is the licensed maintenance dose for weight management.

Liraglutide, sold as Saxenda and in products such as Erly, is a daily injection supplied at 6mg per millilitre, unlike the once-weekly Mounjaro and Wegovy. A daily injection and a weekly one are a lifestyle difference as well as a cost one, so think about what you can keep up. Your prescriber can advise on which is suitable for you.

07 · Eligibility

Who can get Mounjaro for weight management?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition. Those conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.¹ It must be used alongside a reduced-calorie diet and increased physical activity.

That eligibility is a clinical judgement, not a price tier. A prescriber assesses your BMI, medical history and current medicines before deciding whether it is suitable for you.¹¹ That is why a price on its own is never the whole story: a low price for a medicine that is not right for you is not good value. What results could the price buy?

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks. The comparison group, on lifestyle changes alone, lost 3.1%.² These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.² Trial averages do not predict your own result, so treat them as a description of what the study found rather than a forecast.

As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg and 15% is about 15 kg.² That is straight arithmetic on the trial percentages, not a promise and not a timeline.

What does this mean for cost?

NHS or private: which fits you?

The NHS is the lowest-cost route if you qualify and can access it, and private treatment is the route if you meet the licensed criteria but cannot wait. Mounjaro is available on the NHS, but access is being phased in over several years.⁵ NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.⁴

Full rollout is expected to take years.⁶ That is the honest picture: some people can get it on the NHS now, others will wait, and many who meet the licensed criteria choose regulated private treatment.

The licence and the NHS criteria are not the same. The licence for weight management covers adults with a BMI of 30 or above, or 27 to 30 with a weight-related condition. The NHS criteria are narrower, and your GP or local service will tell you where you stand. If you are not sure, asking your GP is a sensible first step and costs nothing.

What about people with type 2 diabetes? Mounjaro is also licensed for type 2 diabetes, and there is NICE guidance for that use. Whether it is offered to you depends on your clinical team and local services, so ask your diabetes team or GP.

10 · Unsafe shortcuts

Which money-saving shortcuts should you avoid?

Cheap supplements are another route people consider. Products nicknamed "nature's Ozempic" or the "poor man's Ozempic" are low-cost supplements with no clinical oversight, and dosing is not standardised. Promises of dramatic weight-loss claims from sellers are a reason for caution, not a reason to buy.

11 · Genuine pens

How do you know a low-price pen is genuine?

You know by checking who is selling it and what arrives. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.⁷ That is why the buying channel matters more than a few pounds saved.

Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.¹² Genuine treatment always involves a prescription and a clinical consultation.
