Prescription weight loss drugs fall into a few families, and knowing the family tells you how a drug is taken and what it targets.

The dual-action family has one member here: Mounjaro (tirzepatide). It activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 That dual action is what distinguishes it from single-action GLP-1 medicines such as semaglutide.1

The single-action weekly family is Wegovy (semaglutide). Its pens come in 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, and the dose steps up after at least four weeks to reach the 2.4mg maintenance dose from week 17.18 Liraglutide, sold as Saxenda, belongs to the GLP-1 group too, but it is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy.

Why does the family matter? Practical reasons come first. A weekly pen suits people who dislike daily routines, a daily injection gives more frequent adjustment points, and a tablet suits those who prefer to avoid needles. Your prescriber weighs these preferences against safety.

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1

Tirzepatide is the first treatment of its kind to activate both GIP and GLP-1 receptors. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Treatment begins at the lowest strength and your prescriber moves you up. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices, and your first order is £40 less with code FIRST40. Not everyone needs the highest strength, and the right maintenance dose is a decision for you and your prescriber.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 This is worth knowing if you ever plan to stop.

06 · Wegovy and semaglutide

Wegovy (semaglutide) and Wegovy tablets

Wegovy is the brand name for semaglutide, a once-weekly injection for weight management. The pens come in 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, and treatment steps up after at least four weeks.18 You reach the 2.4mg maintenance dose from week 17, which is the licensed maintenance dose for weight management.18

The ladder runs 0.25mg, 0.5mg, 1mg, 1.7mg, 2.4mg, with at least four weeks at each step before 2.4mg. It gives the body time to adjust before the maintenance dose, and a prescriber supervises each step.

Semaglutide is a single-action GLP-1 medicine, whereas tirzepatide activates two receptors. That is a difference in mechanism, not a verdict on which suits you. Both are licensed treatments, and which suits you depends on your health profile, tolerance and prescriber advice.

07 · Saxenda, Foundayo, orlistat

Saxenda, Foundayo, orlistat and other options

Beyond the two weekly injections, UK prescribers can use liraglutide, orforglipron and orlistat.

Liraglutide, sold as Saxenda, is supplied at a concentration of 6mg per millilitre. It is a daily injection, unlike once-weekly Mounjaro and Wegovy. For some people a daily routine is a drawback; for others the frequent dosing is simply what they are used to from other treatments. Efficacy figures vary, and your prescriber can advise.

Orlistat is an older oral option. Your prescriber can tell you whether it suits your situation.

What about unlicensed or still-in-development medicines? Medicines that are not licensed for UK weight management should not be combined with Mounjaro or used in place of prescribed treatment. Equally, be wary of supplements marketed as natural versions of prescription drugs. They are low-cost supplements with no clinical oversight, and dosing is not standardised. Dramatic weight-loss claims on those products do not make them medicines.

The rule for every item here is the same: use a regulated UK pharmacy, accept an assessment by a prescriber, and expect a plan that includes diet and activity. If a source promises specific results or offers to supply without a consultation or prescription, walk away.

08 · Who is eligible

Who can be prescribed Mounjaro for weight loss?

Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition.1 Qualifying conditions include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes.1

It must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1 This assessment is the reason you cannot simply buy it.

Let us make that concrete. A BMI of 31 with no other conditions sits in the licensed range. A BMI of 28 with high blood pressure also qualifies. A BMI of 28 with no weight-related condition falls outside it. These are the licence thresholds, and the prescriber still decides whether treatment is appropriate in your case.

The consultation asks about your health, other medicines and history. Some information can change the answer, so be accurate. If a medicine could interact, the prescriber will say so. For example, Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, and if you use insulin or sulfonylureas, your doses may need adjusting because of the risk of low blood sugar. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Report any new symptom to your prescriber, and you can report suspected side effects through the Yellow Card scheme.19

09 · NHS versus private

NHS versus private prescribing

Mounjaro is available on the NHS, but access is being phased in over several years.4 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6

Full rollout is expected to take years.6 That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. The licence is broader than the NHS's current thresholds: a BMI of 30 or above, or 27 to 30 with a weight-related condition.

Which route suits you? Start with your GP. If you fit a highest-need group, NHS care may be available to you. If you do not, or you would rather not wait, a regulated online pharmacy is the alternative. Neither route is wrong, and the medicine is the same.

10 · What trials show

What the trial results show for Mounjaro

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Averages hide a spread. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

Here is a worked example with a starting weight named. For someone starting at 100 kg, 15% is about 15 kg, 19.5% is about 19.5 kg and 20.9% is about 21 kg. The lifestyle-only figure of 3.1% would be about 3 kg. These are straight arithmetic from the trial percentages. They are not a forecast for you, and trial averages do not predict your own result.

They do not tell you how fast you will lose weight. They do not apply to people outside the trial's profile. They do not mean the highest dose is always right: your prescriber balances benefit against tolerance.

Without the diet and activity component, you should not expect the trial result.

What about other drugs? Each has its own trials, with different people, durations and doses, so their results should not be set against these as if head to head. Trial designs differ in population, length and dose, so such comparisons mislead. Where you want a like-for-like view, ask your prescriber. 12 · Stopping and aftercare

Stopping, switching and aftercare

Stopping a medicine is a decision to make with your prescriber, and for Mounjaro there is one fact worth knowing in advance. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears.1 In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber.

That has practical consequences. If you miss a few weeks or decide to stop, the drug does not vanish overnight, and if you restart, tell the prescriber how long the gap was so they can advise on the right dose. Do not restart at a high dose on your own.

Switching between drugs is also a prescriber decision. People often move for reasons of tolerance, preference, supply or cost. Whatever the reason, share your current dose and how you have felt on it. 13 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1 Do not use Mounjaro if you are pregnant, and stop it at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.
