Stopping Mounjaro before trying to conceive

The instruction is clear: Mounjaro should be stopped at least one month before trying to conceive.1 Plan this with your prescriber rather than stopping on your own date and guessing.

Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 That is the reason the advice builds in a gap before conception.

It also helps to think about what happens after you stop. The medicine works alongside a reduced-calorie diet and increased physical activity, not instead of them.13 Ask your prescriber and your GP how to support your weight and nutrition during the gap and into pregnancy.

Contraception and the risk of an unplanned pregnancy

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7

The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 This applies to the pill only. Implants, coils and injections are unaffected.1

  • Do not inject another dose while you wait for an appointment.
  • Contact your GP or midwife so antenatal care starts early, and tell them which medicine you took and when your last dose was.
  • Tell the prescribing team that reviewed your treatment, so your record is updated and no further supply is arranged.
  • Consider reporting the exposure through the MHRA Yellow Card scheme, which collects reports about medicines.19

Mounjaro is not licensed specifically for PCOS, but many women with PCOS meet the licensed weight-management criteria, and weight loss itself is a first-line recommendation that can improve insulin resistance, cycles and symptoms.

Eligibility is assessed on the standard BMI criteria. Mention PCOS and any fertility plans in your consultation, because improving weight can restore ovulation.

08 · Who should not take it

Who else should not take Mounjaro

Mounjaro is not suitable for everyone. You should not take it if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.

Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Always share your full medical history and medicine list during consultation.

As a precaution, tell your prescriber if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2. They will weigh this in your assessment.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
