Which is better for losing weight: Mounjaro or Ozempic?

For weight loss, the real comparison is Mounjaro against Wegovy. Tirzepatide averaged more weight loss than semaglutide 2.4mg in the direct comparison, SURMOUNT-5, and Ozempic is not a UK weight-loss medicine at all.15 That does not make Wegovy a poor treatment. Both are effective, licensed options, and the right one depends on your health profile, tolerance and prescriber advice rather than the headline number alone.15

Mounjaro may suit adults whose prescriber recommends a once-weekly pen that is stepped up over time. Wegovy (semaglutide 2.4mg) may suit people whose prescriber prefers a single-action GLP-1 medicine, or who have a history that points that way. Ozempic is for type 2 diabetes. A regulated UK pharmacy will not supply it for weight loss.18

03 · Deciding

The factors that should decide it

Five factors matter most when choosing between these medicines. The first is licensing: Ozempic is licensed for type 2 diabetes only, so for weight loss the real comparison is Mounjaro against Wegovy.18 The second is the size of average effect. SURMOUNT-5 favours tirzepatide, and the gap is not small.15

The third is tolerance. Gastrointestinal side effects are common with this class, and your own history matters more than a trial average. 04 · Ozempic's UK licence

Why Ozempic is not a UK weight-loss medicine

Ozempic is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label.18 For weight loss, the real comparison is Mounjaro with Wegovy, the semaglutide product licensed for weight management.

The licensed weight-management options in the UK are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).118 Those are the two medicines with trial programmes designed to measure weight loss and with product information written for it. A regulated UK pharmacy will not supply Ozempic for weight loss. If a seller offers it for that purpose, ask why it is not offering the licensed products, and be cautious about any service that skips a proper consultation or prescription.

Why does the distinction matter beyond paperwork? A licence means a regulator has reviewed the evidence for a specific use, the dose range for that use and the safety information that goes with it. Off-label prescribing can be legitimate in some medical settings, but it is a decision for a prescriber who knows your full history. It is not a shortcut for people who find the licensed route inconvenient. For weight management, both licensed routes are available and the evidence is strong.

The names also cause confusion because Ozempic and Wegovy contain the same active ingredient, semaglutide, made by the same company. They are different products with different licences, different pens and different dose ladders. Wegovy pens run through 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18

Mounjaro, by contrast, is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It is licensed in the UK for weight management in adults and for type 2 diabetes, and comes as a pre-filled multi-dose KwikPen in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 So one medicine carries both licences, and the semaglutide range splits them across two brands.

In practical terms, this means you should compare like with like. If your goal is weight loss, compare Mounjaro with Wegovy. Whichever route you take, the medicine should come from a registered pharmacy after a prescriber review, not from an unregulated seller.

05 · Ingredients and mechanism

Ingredients in Ozempic vs Mounjaro: how each works

The main ingredient in Ozempic is semaglutide, and the active ingredient in Mounjaro is tirzepatide, so the two do not have the same ingredients. Semaglutide is the same active ingredient used in Wegovy. Tirzepatide is a different molecule with a different action. The leaflets for each product list the full set of non-active ingredients, and the Mounjaro patient information leaflet is the place to check those.13

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic).1

What does that feel like in practice? Most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That is the point of the medicine. It makes a calorie-controlled eating pattern easier to hold, alongside increased physical activity. It does not replace either.

The trial result is what matters for your decision, not the mechanism alone. SURMOUNT-5 measured the outcome directly, and tirzepatide came out ahead.15

One mechanism detail is worth knowing before you start. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4 to 5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 If you stop treatment or switch, your prescriber will factor that in.

There is also a practical point about the pens. Mounjaro comes as a multi-dose KwikPen, giving four weekly doses per pen at one strength.1 Wegovy and Ozempic each use their own pens and dose steps, which are not interchangeable with Mounjaro's, and strengths cannot be compared milligram for milligram. A 5mg Mounjaro dose does not equal any particular semaglutide dose, which is why a switch is never done by simple conversion.

06 · Head-to-head evidence

What SURMOUNT-5 shows about Mounjaro and semaglutide

SURMOUNT-5 is the first large head-to-head trial of tirzepatide against semaglutide 2.4mg (Wegovy) for weight loss. Adults on tirzepatide lost on average 20.2% of body weight over 72 weeks, versus 13.7% on semaglutide 2.4mg.15 Both are effective, licensed treatments, so a 6.5 percentage point gap in averages should be read as a real difference, not a verdict that Wegovy does not work.

The trial enrolled 751 adults. Average loss was 22.8 kg on tirzepatide against 15.0 kg on semaglutide.15 As a worked example, for someone starting at 100 kg, 20.2% is about 20 kg and 13.7% is about 14 kg. Your starting weight, dose reached and how closely you follow the diet and activity plan all change your own outcome, and trial averages do not predict your own result.

The threshold figures show where the gap is widest. The share of participants reaching each level of weight loss was:

  • At least 10% of body weight: 81.6% on tirzepatide vs 60.5% on semaglutide.
  • At least 15%: 64.6% vs 40.1%.
  • At least 20%: 48.4% vs 27.3%.
  • At least 25%: 31.6% vs 16.1%.

These percentages come from the SURMOUNT-5 results.15 The gap grows at higher thresholds. That matters if your prescriber's goal is substantial weight loss, for example because of weight-related health conditions. It matters less if a modest, steady reduction would meet your goal and tolerability is your main concern.

Where does Ozempic fit? It does not. SURMOUNT-5 compared tirzepatide with the 2.4mg weight-management dose of semaglutide, which is Wegovy, not Ozempic.15 That is why we say the useful question is Mounjaro versus Wegovy.

One further caution: SURMOUNT-5 is the first large head-to-head trial, so it is the one that supports a tirzepatide-versus-semaglutide statement. Other trials, such as SURMOUNT-1 for tirzepatide and STEP 1 for semaglutide, tested each medicine against placebo or lifestyle changes in different groups of people. Their figures are useful for understanding each medicine on its own, but they should not be set side by side as if they were a contest.

The trial results also assume treatment alongside a reduced-calorie diet and increased physical activity. They are not what a medicine does on its own. Individual results vary, and a prescriber should review your progress and tolerance as you go.1

07 · Wegovy's own results

How well does semaglutide work on its own terms?

Semaglutide 2.4mg (Wegovy) produces substantial weight loss in its own right. In the STEP 1 trial, adults on semaglutide 2.4mg lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 That is a meaningful result, and it is why Wegovy is a licensed UK weight-management medicine. It also explains why choosing Mounjaro is not the same as dismissing semaglutide.

The STEP 1 detail shows how the results were spread. 86.4% of adults on semaglutide 2.4mg lost at least 5% of body weight over 68 weeks, compared with 31.5% on placebo. 69.1% lost at least 10% (vs 12.0% on placebo) and 50.5% lost at least 15% (vs 4.9%).9 As a worked example, for someone starting at 100 kg, 14.9% is about 15 kg.

Those numbers are not directly comparable with the tirzepatide figures from SURMOUNT-1, because the trials ran for different lengths with different participants and designs. The fair comparison is SURMOUNT-5. STEP 1 simply tells you what to expect from semaglutide 2.4mg against placebo.

Tolerability is the other half of the picture. In STEP 1, gastrointestinal side effects were reported by 74.2% of people on semaglutide 2.4mg, compared with 47.9% on placebo. 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.9 If you are weighing the two medicines, these figures show that side effects are a real consideration with semaglutide at the weight-management dose. Your prescriber and the product leaflets list the side effects for each pen.13

What should you do if you are worried about side effects? Tell your prescriber, who can review your dose and plan. You can also report any suspected side effect through the MHRA Yellow Card scheme.19 Our monthly reviews and side-effect support exist so you are not working through that alone.

Wegovy's dose ladder is gradual by design. Pens run 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg once weekly, with a step up after at least four weeks, reaching the 2.4mg maintenance dose from week 17.18 Mounjaro also starts low at 2.5mg and steps up through its six strengths. 08 · Mounjaro dose results

What Mounjaro achieved at each dose in SURMOUNT-1

In SURMOUNT-1, adults taking tirzepatide alongside diet and exercise lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 The trial involved 2,539 adults. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The pattern is dose-related. Higher maintenance doses produced higher average loss in the trial. But a higher dose is not automatically better for you. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.

The threshold figures give a fuller picture. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

As a worked example, for someone starting at 100 kg, 20.9% is about 21 kg and 15% is about 15 kg. Starting weight changes the kilogram figure, but the percentage is the more useful measure for comparing across people.

Higher doses averaged higher loss, but the figures also show something less often said: even the 5mg dose delivered an average 15% loss in SURMOUNT-1.

09 · NHS and private costs

NHS access and what private Mounjaro costs

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. On the private side, the price depends on the dose. The standard price of one 4-week pen is £169.99 at 2.5mg, £218.99 at 5mg, £279.99 at 7.5mg, £290.99 at 10mg, £319.99 at 12.5mg and £329.99 at 15mg. 10 · Switching and safety

Switching from Ozempic or Wegovy, and what not to combine

There is no licensed direct conversion between semaglutide and tirzepatide, so a switch is always prescriber-led. Typically the previous medicine is stopped, a washout interval is observed, and Mounjaro is started at a dose your prescriber judges appropriate. That is often the 2.5mg starting dose, sometimes adjusted based on your history.11 Never run two GLP-1 medicines together.

This matters if you currently take Ozempic for diabetes and are wondering whether Mounjaro would suit you better. Do not stop or change a diabetes medicine on your own. Speak to the clinician who manages your diabetes first. Where a person also uses insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar, and that is a conversation for the prescriber.

Mounjaro must also not be combined with other GLP-1 medicines, unlicensed "research" peptides (such as retatrutide, which has no UK licence) or compounded copies. These combinations have no safety data and no regulated supply chain.7 If something promises faster results outside a licensed pathway, that is the warning sign. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme, and we recommend reporting anything suspicious.7

Contraception is another point to raise. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

What about tablets and "cheaper alternatives"? There is no tablet form of Mounjaro, because tirzepatide is injection-only in the UK. Two GLP-1 weight-loss tablets are now licensed: Wegovy tablets (oral semaglutide, made by Novo Nordisk), approved by the MHRA in June 2026, and Foundayo (orforglipron, made by Eli Lilly), approved in August 2026.2021 They are different medicines from Mounjaro, with their own dosing, eligibility checks and trial results, and both are prescription-only. Other licensed options work differently. Orlistat (tablet) blocks some dietary fat absorption and produces smaller average losses. Wegovy and Saxenda (liraglutide) are other licensed injectables, and liraglutide is a daily injection, unlike once-weekly Mounjaro and Wegovy. "Natural GLP-1" supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost supplements with no clinical oversight, and dosing is not standardised.

Pick one licensed medicine, take it under a prescriber's review and never layer treatments to chase speed.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Is Mounjaro or Ozempic better for losing weight?
Ozempic is not licensed for weight loss in the UK, so the comparison is Mounjaro against Wegovy. In the head-to-head SURMOUNT-5 trial, tirzepatide users lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg (Wegovy). Both are licensed, effective treatments, and individual results vary with your health, tolerance and prescriber advice.
Do people lose more weight on Ozempic or Mounjaro?
In the first large head-to-head trial, average weight loss was higher on tirzepatide (Mounjaro). SURMOUNT-5 followed 751 adults for 72 weeks: average loss was 22.8 kg on tirzepatide against 15.0 kg on semaglutide 2.4mg, and 48.4% reached at least 20% weight loss versus 27.3%. That trial used Wegovy's dose, not Ozempic. Your own result depends on dose reached, diet, activity and tolerance, and trial averages do not predict it.
Does Mounjaro have the same ingredients as Ozempic?
No. Mounjaro's active ingredient is tirzepatide, while Ozempic's main ingredient is semaglutide, the same one in Wegovy. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, whereas semaglutide acts on GLP-1 alone. The two pens also list different non-active ingredients, which you can check in each product's patient information leaflet. Never use both together, as two GLP-1 medicines must not be combined.
Which weight loss injection is best?
No licensed medicine is best for everyone; your prescriber decides what suits you. Tolerance, health conditions and your prescriber's judgement matter. Anything promising faster results outside a licensed pathway, including unlicensed research peptides, is a warning sign.
Is Mounjaro safer than Ozempic?
Both are prescription medicines with known side effects, mostly gastrointestinal, and both need prescriber supervision. Your prescriber should review your history before either medicine.
Can I just stop taking Ozempic?
Do not stop a diabetes medicine on your own; speak to the clinician who manages it first, because stopping can affect blood sugar control. Switching between semaglutide and tirzepatide is prescriber-led, with a washout interval, and never involves taking both together.
What is known as the "poor man's Ozempic"?
The phrase is usually applied to 'natural GLP-1' supplements such as berberine, but they are not equivalent. They are not medicines, are not regulated for this use and have no comparable trial evidence, and dosing is not standardised. They are low-cost supplements with no clinical oversight. If cost is a worry, talk to a prescriber about licensed options.
Which is better for weight loss, Mounjaro or Ozempic, in the UK?
In the UK, Mounjaro is licensed for weight management and Ozempic is not, so the licensed comparison is Mounjaro against Wegovy. In SURMOUNT-5, average loss was 20.2% on Mounjaro against 13.7% on Wegovy over 72 weeks. A regulated UK pharmacy will not supply Ozempic for weight loss.
