Which is better for inflammation: Ozempic or Mounjaro?
Neither can be called better for inflammation, because neither is licensed or approved for it.
If you have an inflammatory condition, treating it is a job for your GP or specialist, not for a weight-loss injection.
Mounjaro is a licensed weight-management treatment; Ozempic is not.1 Mounjaro activates two gut-hormone receptors, GIP and GLP-1, whereas semaglutide is a single-action GLP-1 medicine.1

The SURMOUNT-5 comparison was between tirzepatide and semaglutide 2.4mg (Wegovy), not Ozempic. That is the right like-for-like comparison for weight loss, because Wegovy is the licensed weight-management version of semaglutide.
03 · Deciding
How to decide between them
Start with your reason for treatment, not a side benefit. If you have type 2 diabetes, your diabetes team will choose between diabetes medicines on your overall health. If you want help with weight, the licensed options are Mounjaro and Wegovy. Our comparison of Ozempic and Mounjaro for type 2 diabetes covers the diabetes side.
For weight management, the main decision factors are:
- Licence: Mounjaro and Wegovy are licensed for weight management; Ozempic is not.
- Trial evidence: SURMOUNT-5 is the first large head-to-head trial, and it showed greater average weight loss with tirzepatide.15
- Tolerance: both cause gastrointestinal effects in some people; your prescriber can discuss which suits your history. See whether Mounjaro is better tolerated than Ozempic.

04 · Inflammation evidence gap
Inflammation and these medicines
Inflammation is not a listed use for either medicine.
If you notice new or lasting symptoms after starting any medicine, such as joint pain, swelling or skin changes, tell your prescriber. If you have swelling of the face, lips, tongue or throat or difficulty breathing, get medical help straight away. Check the patient information leaflet for the listed side effects.13 You can also report a suspected side effect through the Yellow Card scheme.19
If you live with a diagnosed inflammatory condition, do not start, stop or change any medicine in the hope of treating it. Your GP or specialist is the right person to weigh up treatment options. They can tell you whether a GLP-1 or GIP/GLP-1 medicine fits alongside your other medicines.
05 · Ozempic's licence
Why Ozempic is not the comparison for weight loss
Ozempic is semaglutide licensed for type 2 diabetes, not for weight loss in the UK. Prescribing it privately for weight loss is off-label.1 The licensed weight-management options are Mounjaro (tirzepatide) and Wegovy (semaglutide 2.4mg).
So when someone asks 'Ozempic or Mounjaro?' for any weight-related goal, the practical question is really Mounjaro versus Wegovy. A regulated UK pharmacy will not supply Ozempic for weight loss. If a seller offers it without a consultation or prescription, treat that as a warning sign.

06 · Head-to-head weight loss
What the weight-loss trials show
In SURMOUNT-5, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks, versus 13.7% on semaglutide 2.4mg (Wegovy).15 Both are effective, licensed treatments. Which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.
The detailed SURMOUNT-5 results involved 751 adults over 72 weeks:15

The average loss was 22.8 kg on tirzepatide versus 15.0 kg on semaglutide. These are trial averages, and individual results vary.
Each medicine also has its own trial. In SURMOUNT-1, which included 2,539 adults, people on tirzepatide lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 For someone starting at 100 kg, 20.9% is about 21 kg. In STEP 1, adults on semaglutide 2.4mg lost on average 14.9% of body weight over 68 weeks, compared with 2.4% on placebo.9 These two trials are separate studies and should not be set side by side as a comparison; only SURMOUNT-5 compares the medicines directly.
Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
07 · Side effects and tolerance
Side effects and tolerance
Gastrointestinal side effects are the main tolerance issue with semaglutide 2.4mg. In STEP 1 they were reported by 74.2% of people on semaglutide, compared with 47.9% on placebo, and 7.0% stopped treatment because of side effects, compared with 3.1% on placebo.9

Because tirzepatide has a half-life of about five days, it takes roughly 4 to 5 weeks to clear the system after a final dose.1 There is no way to speed this up, and nothing needs doing while it clears. Report any suspected side effect through the Yellow Card scheme.19

08 · Switching and safety
Switching between them, and what never to combine
There is no licensed direct conversion between semaglutide and tirzepatide. Switching is done under prescriber supervision: typically the previous medicine is stopped, a washout interval is observed, and Mounjaro is started at a dose your prescriber judges appropriate, often the 2.5mg starting dose, sometimes adjusted based on your history.1 Never run two GLP-1 medicines together.
Mounjaro must not be combined with other GLP-1 medicines, with unlicensed 'research' peptides such as retatrutide (which has no UK licence), or with compounded copies. These combinations have no safety data and no regulated supply chain. If something promises faster results outside a licensed pathway, that is the warning sign.
Counterfeit and unregulated products are a real risk when buying injections online. The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.7 Check that a pharmacy appears on the GPhC register. Natural 'GLP-1' supplements such as berberine are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost supplements with no clinical oversight, and dosing is not standardised. 09 · NHS or private
NHS or private access
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.

If you are on an NHS pathway already, stay in touch with that team. Do not run an NHS and a private GLP-1 treatment at the same time, and tell every prescriber what you take.
10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
