Can I leave Mounjaro out overnight? It depends on the pen

Leaving Mounjaro out overnight is fine for a pen you are already using, and needs a quick check for a pen you have not yet started. The UK product information says an in-use KwikPen can be kept out of the fridge for up to 30 days (not above 30°C).1 One night on a bedside table or kitchen counter sits comfortably inside that window.

An unopened pen is different. The instruction is to store it in the fridge, between 2–8°C, until first use, and it then keeps until the printed expiry date.13

02 · What to do next

The two decisions you are actually facing

The first decision is whether the pen was in use. If you had already taken at least one dose from it, treat the 30 days as your clock. Put it back in the fridge or keep it somewhere cool and dark, and carry on as normal.

The second decision is whether anything about the night was unusual. A warm room, a car, a hot bag or a sunny windowsill matters more than the hours themselves. If the pen was somewhere hot, or you are unsure, do not inject until you have checked the leaflet and spoken to your pharmacy. 03 · In-use versus unopened

In-use versus unopened pens

The storage rules for Mounjaro depend on where the pen is in its life. Mixing the two up is the most common reason people worry unnecessarily, or relax when they should not.

Before first use, the pen lives in the fridge at 2–8°C. It then keeps until the printed expiry date.13 That is the whole rule for unopened pens, and it is deliberately simple. If an unopened pen spends time outside the fridge, the leaflet and your pharmacy are the right place to check, not a rule of thumb.

Once you have started using a KwikPen, the UK licence allows it to be kept out of the fridge for up to 30 days (not above 30°C).1 That is why many people simply keep the pen in a cool drawer or on a shelf during the month it takes to finish. A pen contains several weekly doses, so it stays in use across a whole month of injections.

One important point: the 30 days is a UK figure from the UK product information. You may see a shorter figure in material written for other countries. Ignore that, because those labels belong to a different market and do not apply to a UK-supplied KwikPen.

Another point worth being clear on: the 30 days is a limit, not a target. Keeping an in-use pen in the fridge is still perfectly acceptable, and many people prefer it. The out-of-fridge allowance is there for convenience, travel and mistakes, not as a reason to stop refrigerating.

When the in-use period ends, the pen should be discarded, even if some liquid seems to remain. Do not try to extend it.

If you are not sure whether your pen counts as in use, the simple test is this: have you used it to give yourself an injection? If yes, it is in use. If you have only attached nothing and taken nothing from it, it is unopened and should stay chilled until you begin.

04 · What overnight really means

What "overnight" looks like in real life

The word overnight covers very different situations, and the right response depends on which one you are in.

You forgot to put the pen away after injecting. This is the commonest scenario and the least worrying. A pen you have started, left on a counter or bedside table in an ordinary home overnight, is inside the licensed in-use window.1 Put it back where it lives and carry on.

You came home and the delivery sat out. A new pen that arrived and was left out is an unopened pen, so the fridge rule applies.13 Our pens travel in temperature-controlled packaging with free next-day tracked delivery, designed to keep the contents cold in transit. Even so, get the pen into the fridge as soon as you can, and tell your pharmacy if it waited a long time or the weather was hot.

You were away overnight and the pen was in a bag. A bag in a cool hotel room is a different proposition from a bag in a hot car. Consider where it was, whether it was in direct sun, and whether it was next to a heater or radiator.

You left the pen in the car. Cars can get far hotter than the outside air, and quickly. If a pen was in a parked car in warm weather, treat it with caution. Do not inject until your pharmacy has advised you.

You left it by a window. Sunlight adds both heat and light. The pen should be kept in its carton, away from direct sun, whenever it is not in use.

Notice that none of these questions is really about the number of hours. Most of the risk is about heat and exposure. If you cannot tell how warm the place got, that uncertainty is itself a reason to ask rather than assume.

05 · Heat, light and potency

Why heat and light matter more than time

Mounjaro is a medicine in solution, and like most injectable medicines it is designed to be kept within a stated range of conditions. The storage instructions are written so that the pen stays safe and effective for as long as the label says. Step outside the instructions and you are outside the conditions it was tested in.

Outside the stated storage conditions, sterility and potency are no longer assured. It does not mean a pen has necessarily failed. It means nobody can vouch for it any longer. For a medicine you inject, that is a good reason to be cautious.

The in-use allowance of up to 30 days (not above 30°C) is the tested window for a pen at room conditions.1 Within that window, an ordinary room is expected. A hot car, a sunny sill or a spot beside a radiator is not an ordinary room.

Light is a smaller concern than heat, but it is still worth managing. The sensible habit is to keep the pen in its carton whenever it is not in your hand. That also protects the pen from knocks, and keeps the label and pen details together, which helps if you ever need to talk to a pharmacist about it.

What about freezing? Cold is a risk too. The fridge range is 2–8°C for a reason: the pen should be chilled, not frozen. Keep it away from the back wall or the cooling element of the fridge, and do not use a pen that has been frozen. Check the leaflet for the exact instruction and contact your pharmacy if this has happened.

In short: treat temperature extremes in either direction as a reason to pause and ask, and treat ordinary room conditions during the in-use period as normal.

06 · Checking a pen before use

How to tell if a pen may have gone bad

You cannot always tell by looking whether a pen has been affected by heat, but you can check for obvious warning signs, and you should do so before every injection.

Start with the label and the expiry date. A pen past its printed expiry date is not to be used. For an in-use pen, work out whether it has been in use for longer than 30 days, or above 30°C.1 If it has, discard it.

Next, look at the contents through the pen's window. The solution inside a genuine pen is clear and colourless. If it looks cloudy or discoloured, or has particles in it, do not inject it.

Then look at the pen itself. A cracked body, a damaged dose window or a pen that has clearly been dropped or crushed should not be used. Check that the needle attaches properly and that the pen behaves as the instructions describe, following the pen's own instructions for use.

Finally, think about the history. If the pen was somewhere hot, if it was frozen, or if you simply do not know what happened to it, treat that as a failed check even if the pen looks normal. A pen that looks fine can still have been exposed to conditions outside its tested range.

If any check fails, the right steps are straightforward. Do not inject. Keep the pen and its carton. Contact your pharmacy, and if you ordered from us, our support team can advise on next steps. For dispensed prescription medicines, our policy is that returns are accepted only for faulty, damaged or wrong items, and we need you to contact us within 3 days. So if something looks wrong on arrival, tell us promptly.

Do not try to fix a doubtful pen by injecting more or less than usual. That is not a safe workaround, and your prescriber is the right person to decide what to do about a missed or replaced dose.

07 · Travel and holidays

Travelling, holidays and nights away

A night or two away from home is the most common reason Mounjaro ends up out of the fridge, so a little planning goes a long way.

If your pen is already in use, you have the licensed in-use window of up to 30 days (not above 30°C) to work with.1 That makes ordinary trips manageable. Keep the pen in its carton, in hand luggage rather than a hold, and out of direct sun. A small insulated pouch is a sensible accessory and does not need to be expensive.

If you are carrying a pen you have not started, keep it cool, and put it in a fridge at your destination as soon as you can. Many hotels and holiday rentals have one, and it is reasonable to ask. The unopened-pen rule is the fridge until first use, so a long journey with an unopened pen is worth discussing with your pharmacy beforehand.13

Do not leave a pen in a parked car, a beach bag or a coat pocket in the sun. Those are the situations that create real problems, far more than a night in a cool room.

Many people ask whether they can pause Mounjaro for a holiday. That is a prescriber decision, not a storage one. The pen is a once-weekly medicine, and tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up. A planned break therefore does not behave like switching off a light. If you are thinking about one, speak to your prescriber before you act, because restarting may need a different approach from simply carrying on.

Carrying injectables abroad can also involve airline or border requirements. Check with your airline and destination, and carry your pharmacy label and prescription details with you.

08 · Missed doses and breaks

Skipping weeks, breaks and alternate-week dosing

Storage worries sometimes lead people to wonder whether they should skip a dose or change their schedule. The short answer is that you should not improvise, and your prescriber should make any change.

Mounjaro is licensed as a once-weekly injection.1

If you miss a week, do not double up to catch up. Take a missed dose as soon as you can within 4 days (96 hours) of the scheduled time; if more than 4 days have passed, skip it and take the next one on your normal day, and never take two doses within 3 days of each other. If you stop altogether, remember that tirzepatide stays in the body for some time. After a final dose it takes roughly 4–5 weeks to clear the system, and nothing needs doing while it clears.

What about keeping weight off afterwards? The clinical trials describe treatment working alongside a reduced-calorie diet and increased physical activity, not instead of them.2 In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Whatever habits you have built while on treatment are what you carry forward, so keep them going, and talk to your prescriber about a plan before you stop rather than after.

In SURMOUNT-1, people lost on average 15% of body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, and individual results vary.2
