How to inject Mounjaro: the correct way

The correct way to inject Mounjaro is a subcutaneous injection, meaning just under the skin, once a week on the same day each week, at any time of day, with or without food.1 It is a pre-filled multi-dose pen (KwikPen), so there is no drawing up, mixing or measuring syrup-style doses.1 Each pen contains four weekly doses.1

Your pen comes with the patient information leaflet and the pen's own instructions for use. Those are the authoritative step-by-step guide, and you should follow them for every injection, including how to attach the needle, remove the caps and complete the dose. Needles, a sharps bin and injection guidance are included with every order from us.

  • Pick your day. Choose a weekday or weekend day you will remember and keep it.
  • Check the pen. Look at the medicine through the window and check the pen has not been frozen, as a frozen pen must never be used.1
  • Choose a site. Stomach area, front of the thigh, or back of the upper arm if someone else injects. Pick a different spot from last time.
  • Dial and confirm. Turn the dose knob until the dose indicator shows your correct dose in the window.1
  • Inject as the pen instructions describe. Complete the injection exactly as the instructions for use say, then dispose of the needle in a sharps bin.

Can you inject Mounjaro wrong?

Yes, you can inject Mounjaro wrong, but the pen is designed to make the serious mistakes hard. The mistakes that actually happen are usually about timing, site choice, storage and improvising.

Where to inject Mounjaro: belly, thigh or arm

You can inject Mounjaro into the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection. Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection.1

Injecting Mounjaro in the belly, abdomen or tummy. These are all the same site under different names. Most people choose it because it is easy to see and reach. Keep at least 5cm from the belly button, and choose a patch of skin that is not sore, bruised or scarred. The back of the upper arm. This site is licensed only when someone else gives the injection, because you cannot reliably reach or see it yourself. If you live alone, stick to the stomach or thigh.

Why rotation matters

The leaflet and prescribing information say to rotate the site with each dose.1 Rotating means choosing a different spot each week, whether that is a different part of the stomach or alternating between stomach and thigh. A simple habit is to note the site in your tracker app each time so you are not relying on memory.

Pick skin that looks healthy. If a spot is lumpy, tender or discoloured, avoid it and mention it to your prescriber at your next review.

05 · Pen handling

Preparing the pen: dial, window and storage

The KwikPen is set by turning the dose knob until the dose indicator shows the correct dose in the window.

Mounjaro not injecting or the dial will not turn

Do not top up with a second pen or try to combine remnants. Contact our pharmacy team for guidance on the next steps. If the pen looks damaged or seems faulty, tell us promptly: faulty, damaged or wrong items must be reported within 3 days.

If the injection itself seems not to have gone in, for example the pen did not complete as the instructions describe, do not repeat the dose on impulse. Check the instructions for use and speak to your prescriber or pharmacist.

Injecting cold Mounjaro

Pens are kept in the fridge until first use, so the first injection from a new pen will usually be of a cold pen. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them.1 Many people simply find a very cold injection slightly more noticeable. If cold injections are uncomfortable for you, ask your pharmacist for advice rather than warming the pen with heat sources.

In-use storage

Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days — useful for short trips — after which it should be discarded.1 Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, because figures circulating online often come from other countries' labels.1

A spot of blood when injecting Mounjaro

A tiny spot of blood at the injection site after a subcutaneous injection is a common thing to notice with any pen injection, and by itself it is not a sign that the dose was wasted. The patient information leaflet is the reference for what to do and when to seek advice. If bleeding is more than a spot, keeps happening, or you are on a blood-thinning medicine, tell your prescriber or pharmacist.

Bruising when injecting Mounjaro

Speak to your prescriber if bruising is frequent, large, painful or spreading, or if you see increasing redness, warmth or a lump that does not settle. If you notice swelling of the face, lips, tongue or throat or have difficulty breathing, get medical help straight away. New symptoms should always be discussed with the prescriber, and you can report any suspected side effect through the MHRA Yellow Card scheme.19

Timing is the part of injecting Mounjaro where mistakes carry the most risk, so follow the licence closely.

What is the best time of day to inject?

There is no "best" time of day — Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1 Pick a day and time you will remember (many people choose a weekend morning so early side effects fall on days off) and stick to it.

Can you eat straight after an injection?

Yes. Mounjaro is injected at any time of day, with or without food, so there is no required gap before or after eating.1 If you feel queasy afterwards, that is a matter of side effects rather than a rule about meals. Mild nausea in the first days after each injection is among the commonest early effects and usually settles.1

Injecting Mounjaro early

Injecting early is only possible within limits. You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 Never take two doses within 3 days of each other.1 If you are thinking of moving your day permanently, for example because of a work change or holiday, move it once and then keep the new day.

If you miss a dose

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day.1 If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 If you have missed several doses in a row, speak to your prescriber before restarting, because the right dose to resume at is a clinical decision.

Doses, dose ladder and the 2.5mg pen

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 If you are injecting 2.5mg, you use the same technique as at any higher strength; only the pen differs. Your prescriber guides the pace of each step.

Why you should not change how the pen delivers the dose

Do not try to split, stretch or spread doses. Altering doses outside the licence means unknown dosing accuracy and no safety data.1

If you have a severe or unusual reaction, get medical help straight away. If the pen seems damaged, or you have injected something that did not go to plan, stop and seek advice. What to expect after you start injecting

Early weeks are an adjustment phase, and experiences differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose — both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

How long does Mounjaro take to kick in?

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Think in months, not days, and judge progress by trend.

Early signs Mounjaro is working

The earliest sign most people describe is feeling fuller sooner and less hungry between meals. That fits how the medicine works: it activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties.1 Do not judge it by one week of the scales.

How quickly will you feel sick?

Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Many people choose a weekend morning to inject so these effects fall on days off.

Trial results, put in context

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

10 · Travel and storage

Injecting while travelling

You can fly with Mounjaro, and most holidays need no special arrangements. Carry pens in hand luggage (hold luggage can fall below freezing), ideally in a cool bag with the prescription label or a copy of your prescription.1 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements; for hot climates use an insulated pouch.1 Check destination rules for travelling with prescription injectables before you fly.

Plan your injection day around the trip. If travel crosses your usual day, you can change the day as long as at least 3 days (72 hours) have passed since your last dose.1 Unopened pens should stay in the fridge and keep until the expiry date printed on the box.1

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
