What a higher dose does to your appetite and weight

A higher Mounjaro dose strengthens the same two actions you already feel on a lower dose. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

That mechanism is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. The medicine works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The same trial reports how many people reached meaningful thresholds: At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Notice what that tells you: the large majority of people already reached a worthwhile loss on the lower strengths, and the higher strengths added to the proportion reaching the largest losses. It does not mean everyone needs the top dose.

An increase may also affect more than the scales. In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

Equally, a higher dose also means a higher chance of feeling the medicine. 05 · The dose ladder

The Mounjaro dose ladder, step by step

The Mounjaro dose ladder has six strengths, and every patient climbs it in the same order. Mounjaro comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 You cannot skip the bottom rung, because everyone starts Mounjaro on 2.5mg once a week.

The 2.5mg starting phase is deliberately modest. The first weeks are an adjustment phase and losses are usually modest. Experiences differ widely: some people notice appetite change and early loss in the first week, others feel little until a higher dose, and both are normal. The next move is to 5mg. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Two things in that sentence matter. The word minimum means four weeks is the earliest, not the target. And the phrase depending on how you respond and tolerate it means the decision is individual.

Where people end up varies. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1

06 · How long before increasing

How long before increasing your Mounjaro dose

You should stay on each Mounjaro strength for at least four weeks before increasing, and often longer if you are still adjusting. The licence sets the minimum: after at least 4 weeks the dose increases to 5mg, and further steps come at a minimum of 4-week intervals.1 Each KwikPen is designed around that rhythm, because each KwikPen contains four weekly doses.1 In other words, you normally finish one pen at one strength before moving to the next pen.

Minimum is not the same as automatic. A good test is to ask three practical questions before your review. Are side effects settled, or are you still feeling nausea or tiredness after injections? Is your appetite control still working, or has it faded? And has your overall trend slowed over several weeks, rather than just a single weigh-in? If the answer to the first is that you are still struggling, there is no benefit in rushing.

If side effects are the concern, your prescriber can hold the current dose longer before stepping up. That is a normal part of treatment, not a failure, and it is the reason ongoing review matters. Timing also matters for how you judge progress. Weight loss on Mounjaro is gradual by design, and trials measured results at 72 weeks, so think in months, not days, and judge progress by trend rather than individual weigh-ins.4 A sensible expectation at about three months on a standard titration is modest, because most people are still on the lower rungs. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.4

There is also a formal checkpoint. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4 Note the wording: the highest tolerated dose, not the highest dose on the ladder.

07 · Do you need the top dose

Do you need to reach 15mg?

No, you do not need to reach 15mg, and higher is not automatically better. The licensed maximum is 15mg weekly, but the licence describes a ceiling, not a goal. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1

The trial data support that flexibility. In SURMOUNT-1, people on 5mg lost on average 15% of their body weight over 72 weeks, compared with 19.5% on 10mg and 20.9% on 15mg, against 3.1% with lifestyle changes alone.2 Those are averages and individual results vary. What they show is a real, meaningful result on the lower strengths, with a gradual additional average effect as the dose rises. For a worked example, someone starting at 100 kg would lose about 15 kg at the 5mg average and about 21 kg at the 15mg average, though your own result may differ.

Whether the extra step is worth it depends on you. Consider what you are trying to achieve, how you tolerate the current dose, and whether your progress has stalled for reasons a dose change will not fix. If your food intake has drifted, a bigger pen will not replace the basics. If your appetite has returned and you are comfortable on your current strength, the next rung may be sensible. Check the basics, which means sleep and alcohol. It is also important to know what not to do. A plateau is not a reason to double doses or add unlicensed products.4

Expectations matter here too. Three months in, most people are still on the lower rungs, so trial-average losses at this stage are modest compared with the headline figures. 09 · Injecting at a new dose

Injecting at a new dose: the practical rules

Injecting at a higher strength follows exactly the same routine as before. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food. Rotate the site with each dose.1 Licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection. Needles, a sharps bin and injection guidance are included with every order.

Checking the dose is simple and the same at every strength. When you open a new pen at a new strength, check the number on the pen label and in the window before you inject. Each KwikPen contains four weekly doses.

Timing questions come up around step-ups. There is no best time of day: Mounjaro works the same morning or evening, with or without food, so what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off, which can be a sensible choice when you begin a new strength.1 If you need to move your day, you can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1

If you miss a dose, the rule is this: take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 Do not use a missed dose as an excuse to catch up with a bigger one.

Storage does not change with strength. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1

Travelling after a step-up is straightforward. You can fly with Mounjaro, carrying pens in hand luggage, ideally in a cool bag with the prescription label or a copy of your prescription.1 Check destination rules before you fly.

10 · Stopping and holding

Holding, stepping back or stopping

Increasing is one of several things your prescriber can decide at review; holding and stepping back are equally valid. That is not going backwards. It is how the ladder is designed to be used.

Sometimes the right decision is to stay on the dose that gave you the most benefit for the least discomfort. Because the dose you tolerate and respond to is the dose that counts, the licence refers to the highest tolerated dose rather than the highest available. If you reach your weight goal or feel well supported at a lower strength, your prescriber can discuss whether maintaining there makes sense.

Many people ask whether they can stop altogether once they have lost weight. That is a decision for you and your prescriber, based on your health, your goals and how you are managing without the medicine. What is useful to know is the practical side: tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1

If you are not responding, the review framework applies. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4 That conversation is about fit, not blame.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

12 · NHS, private and ordering

NHS or private: who manages your dose increases

Dose increases are managed by whoever prescribes your Mounjaro, whether that is the NHS or a private prescriber. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

Practically, tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears. Can I increase my dose faster than every four weeks?
No. The licensed interval is a minimum of four weeks between steps, so moving up sooner is outside the licence. Taking extra doses, mixing strengths or changing the amount the pen delivers means unknown dosing accuracy and no safety data. Can side effects come back when I increase my dose?
They can. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle, and they may return for a short spell after a step-up.
