Benefits of Increasing Your Mounjaro Dose (October 2026)
The main benefit of increasing your Mounjaro dose is, on average, greater weight loss at higher maintenance doses, and some people notice stronger appetite control. In the SURMOUNT-1 trial, average loss rose from 15% on 5mg to 19.5% on 10mg and 20.9% on 15mg over 72 weeks; individual results vary.2 Higher is not automatically better, though, and your prescriber sets the pace. Current as of October 2026.

A quick overview
What to know
before you start.
-
A higher dose can bring stronger appetite control and, on average, greater weight loss, but trial averages do not predict your own result.
-
Everyone starts on 2.5mg, which is for tolerability. Most measurable progress builds from 5mg onward.
-
Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and a lower dose that works is a good outcome.
-
Stepping up can also bring stronger side effects, so the benefit has to be weighed against how well you tolerate each step.
In this guide
01 · The short answer
What you gain from a higher Mounjaro dose
A higher dose can give, across a trial population, a larger average weight loss, and some people feel fuller sooner and less hungry between meals. That is the honest headline, and it comes with a caveat: the benefit is an average, and it is not the same for everybody.
The licensed ladder is built to let your body adjust first. This starting dose is about letting your body adjust, not about weight loss itself.1 So if you feel little on 2.5mg, that is expected rather than a sign the medicine is not working for you.
The trial figures show why people look forward to stepping up. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.
02 · Your decision
The decisions you are actually facing
Most readers are weighing one of two situations. Either you are on a lower dose and wondering whether to ask for a step up, or you are on a dose that is working and wondering whether you are missing out by not going higher.
If appetite control is already good and your weight is trending in the right direction, there is usually no benefit in going higher just for the sake of it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better. Your prescriber will guide the right pace.1
If hunger is returning before your next weekly injection, or progress has flattened over a period of months on a dose you tolerate well, that is a good moment to raise it at your review. Our guide to whether you have to keep increasing covers the opposite question.
03 · At a glance
Benefit versus trade-off, step by step
| Stage | What it is for | What to expect |
|---|---|---|
| 2.5mg | Adjustment and tolerability | Modest change; some people feel nothing yet |
| 5mg | First maintenance option | Most measurable progress starts to build |
| 7.5mg to 12.5mg | Intermediate steps if more effect is needed | Possibly stronger appetite control; side effects may return after each step |
| 15mg | Maximum licensed dose | Highest average trial result; not needed by everyone |
Dose increases are part of an ongoing review rather than a one-off purchase. Our prescribing team handles dose-titration and follow-up as part of aftercare, and you can see how to start on the Mounjaro pen page. A new customer gets £40 off their first order at any dose with code FIRST40, and every later order is £25 off the standard price.

04 · How the dose ladder works
How the Mounjaro dose ladder works
The Mounjaro dose ladder is a stepwise route from 2.5mg to a maximum of 15mg, and understanding it explains most of the benefits and limits of increasing. Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. That is the earliest a step is allowed, not a target, and nobody is obliged to climb at that speed or to reach the top.
Each KwikPen contains four weekly doses, so a pen normally lasts one month at a single strength. This matters when you change strength, because each new strength arrives as its own pen.
Why it starts so low
The starting dose exists for tolerability. The body needs time to adjust to a medicine that slows how quickly the stomach empties and changes appetite signals. Starting at a dose that is not expected to deliver full benefit is therefore intentional.
What you gain at each step
The relationship between dose and effect is not a straight line, and the trial averages show gains that get smaller as the dose rises.
The practical benefit of the ladder is flexibility. If a lower dose is doing the job, you can stay. If it is not, there is room to move. You can read our overview of the Mounjaro stages for a month-by-month picture of how people typically move through the ladder.
Who decides the pace? You and your prescriber, together, at your monthly review. That is the right model: your response, your tolerance and your goals decide the dose, not a fixed schedule on a leaflet.
05 · What the trial data show
What the trial data show at each dose
The clearest evidence for the benefit of a higher dose comes from SURMOUNT-1, and it supports a modest but real extra average effect at higher strengths. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages, and individual results vary. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 The average hides a wide spread of experience: some people do very well on a lower dose, others need more.
How many people reached meaningful loss
The trial also reports how many participants crossed meaningful thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Notice how close those figures are. Most people on the lowest maintenance dose reached that threshold, so a higher dose does not decide whether you respond at all.
The difference shows more clearly at the upper end. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 That is where a higher dose appears to earn its place: it raises the chance of a larger loss, particularly for people who have a lot to lose and tolerate the dose well.
A worked example, with its limits
For someone starting at 100 kg, 20.9% is about 21 kg. That is the straight arithmetic of the trial average on 15mg, not a forecast for you, and it says nothing about how quickly it would happen. Trials measured results at 72 weeks, so think in months rather than days.
Reading the numbers honestly
Three points keep this in proportion. First, the figures come from a single trial population with structured support. Second, you cannot read a larger average as a promise of a larger personal result. Third, a bigger number is not automatically a better outcome if the side effects at that dose make life harder. The benefit of increasing is real on average, but it is a benefit you confirm in your own body, over months, with your prescriber.
If you want to know whether your own trend justifies a step, the most useful tool is a steady record, not a single weigh-in, and our free GLPTrackr companion app is designed for exactly that kind of tracking.
06 · Appetite and the mechanism
Why a higher dose can feel different
Tirzepatide acts on appetite and digestion through gut-hormone pathways. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1
In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic).
What stronger appetite control means in daily life
For many people, the day-to-day benefit of a step up is not a number on the scales. It is feeling satisfied by a normal portion, finding it easier to leave food on the plate, and noticing that thoughts about the next meal are less intrusive. Those changes make a reduced-calorie diet easier to sustain, which is what the medicine is designed to support.
Some people notice a clear change at each step. Others find the change between two neighbouring doses is small and the main difference is side effects.
Side effects after a step up
Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks as the body adjusts. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 Expect some of them to return briefly after a step up, then ease. Our guide to side effects when increasing your dose goes through this in detail.
Why timing within the week matters
If your appetite returns towards the end of the week, that pattern is worth mentioning at your review. It is useful information, because it tells your prescriber how the dose is performing across the whole interval rather than only on the days after injection. It is a reason to discuss a change, not a reason to alter the schedule yourself. Never take two doses within 3 days of each other.
07 · Plateaus and slow progress
Plateaus: when stepping up helps
A step up can help when progress has genuinely stalled on a dose you tolerate well, but many apparent plateaus are normal and need patience rather than more medicine. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1
Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins.2 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1
Signs a step up may be worth discussing
- Your trend has been flat across several weeks on a dose you tolerate comfortably.
- Hunger or food noise is returning before your next weekly injection.
- You have been on the current dose for a good while and side effects have settled.
- You are still following a reduced-calorie diet and staying active, and results have not moved.
Signs to wait or hold
- You have only just stepped up and side effects are still settling.
- Your scale weight moved around but the overall trend is still downward.
- You are happy with your appetite control and your progress.
- You are finding the current dose hard to tolerate.
The formal review point
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Notice the phrase highest tolerated dose. The aim is not the highest dose on the ladder, it is the highest dose that suits you.
Before you ask for more
Check the basics first. Are you injecting on the same day each week? Have you missed doses? Has your eating drifted back to old patterns? Has anything else in your health or medicines changed? A dose increase will not fix a problem that has another cause, and your prescriber will ask about all of these. For a wider discussion of what stages feel like in practice, see our overview of the stages of Mounjaro treatment.
08 · Why higher is not always better
Why higher is not always better
Higher is not automatically better, because the extra benefit has to be weighed against how well you tolerate the dose and what you need from treatment.
The trial figures support this
Look again at the SURMOUNT-1 results. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 A lower maintenance dose already helped the large majority reach that point. Whether a step to a higher dose adds value for you depends on your goal. If you want a larger loss and tolerate the dose well, it may. If you are already at a weight and appetite level you are happy with, it likely will not.
Tolerability and quality of life
A dose that you can live with week after week is worth more than a higher one you dread. Nausea, fullness that makes eating unpleasant, or constant tiredness all count against a step up. That is an entirely normal outcome of dose-titration, not a failure.
Cost across a year
Each pen strength has its own standard price. At standard prices, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. A new customer's first order is £40 less with code FIRST40, and every later order is £25 off the standard price. Monthly prices as of 12 October 2026 are £218.99 at 5mg, £290.99 at 10mg and £329.99 at 15mg.
Reporting problems
If a new dose brings a symptom that worries you, speak to your prescriber, and you can report suspected side effects through the Yellow Card scheme.19 Reporting helps regulators spot patterns, and it does no harm to do it alongside a conversation with your prescriber.
A good question to ask yourself
Rather than asking whether you can go higher, ask whether you need to. If your appetite is controlled, your trend is good and you feel well, the benefit of the next step may be small. If one of those is missing, a conversation about a step up is reasonable.
09 · Injecting and pen changes
Practicalities when you change dose
When you move to a higher strength, the main practical change is a new pen, so keep your routine the same and follow the instructions for the new one. Mounjaro is injected under the skin (subcutaneously) once a week, on the same day each week, at any time of day, with or without food.1
There is no best time of day. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week. Many people choose a weekend morning so early side effects fall on days off, which is a sensible choice around a step up.1
Where to inject
Licensed injection sites are the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1
If you miss a dose around a step up
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day, and never take two doses within 3 days of each other.1 If you are unsure which strength a missed dose belongs to, contact your prescriber instead of guessing.
Changing your injection day
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 A step up is a convenient moment to settle on a day that works, but avoid shortening the gap between doses.
Storage with a new pen
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, which is useful for short trips, and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13 Unopened pens stay in the fridge until first use and keep until the expiry date printed on the box.
If you are travelling, carry pens in hand luggage, ideally with the prescription label, and check destination rules for prescription injectables before you fly. Needles, a sharps bin and injection guidance come with every order.
10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
11 · NHS and private routes
Getting a dose increase: NHS and private routes
Dose increases are managed by whichever prescriber is looking after your treatment, so the route you use to start decides who reviews your progress. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46
Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
How a private review works with us
Everything starts with a free online consultation: a short health questionnaire, with no charge and no obligation. It is reviewed by a GPhC-registered prescriber, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Repeat orders re-check suitability, and there is no auto-shipping, so a dose increase is always a considered decision rather than a default.
Aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team, plus side-effect support. That structure is what a dose conversation needs: someone who can see your history and respond to how you are doing.
Supply and delivery
We supply Mounjaro through official LillyDirect supply, direct from Eli Lilly's UK distribution, as genuine KwikPens. All six doses are in stock. Delivery is free next-day tracked in temperature-controlled, discreet packaging, with same-day dispatch when ordered by 3pm. Saturday delivery is £14.99. Our pharmacy is registered with the GPhC, number 9012878.
Switching to us at a higher dose
If you are already at a higher strength elsewhere, a new customer's first order is £40 off at any dose with code FIRST40, and every future order is £25 off with code DOSE25. You can start from the Mounjaro pen page.
Which route suits a dose question?
If you are on the NHS pathway, raise your dose question with your NHS prescriber or service. If you use a private service, raise it at your review. In both cases, the right question is not whether you can go up, but whether going up is likely to give you a benefit worth the trade-off. Payment options include Visa, Mastercard, Klarna and pay-monthly plans, with no subscription lock-in.
