Why do you get gas on a GLP-1 medicine?

Gas is linked to the slower stomach emptying that these medicines cause. Tirzepatide activates two gut-hormone receptors, GIP and GLP-1, which together help regulate appetite and slow how quickly the stomach empties.1

Most people feel fuller sooner and less hungry between meals.1 Because digestion is different, the way your gut feels after eating can change too. That includes how food moves, how full you feel and how your abdomen feels afterwards.

The UK Mounjaro patient leaflet lists indigestion, bloating, burping, wind and reflux as common side effects, affecting up to 1 in 10 people.13

05 · Dose pace and gas

Can slowing your dose pace help?

Your prescriber can hold your current dose longer before stepping up if a dose is hard to tolerate.

The licensed ladder is built to be gradual. Everyone starts on 2.5mg weekly. After at least 4 weeks the dose rises to 5mg, then can step up by 2.5mg at minimum 4-week intervals (7.5mg, 10mg, 12.5mg) to a maximum of 15mg weekly, depending on how you respond and tolerate it.111

Many people settle at 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.1

06 · Injection routine

Does your injection routine matter?

A steady routine makes symptoms easier to interpret, though there is no best time of day. Mounjaro works the same morning or evening, with or without food. What matters is the same day each week.1

Many people choose a weekend morning so early side effects fall on days off.1 If you are not sure which day suits you, you can change your injection day, provided at least 3 days (72 hours) have passed since your last dose.1

Inject under the skin of the stomach area (at least 5cm from the belly button), the front of the thigh, or the back of the upper arm if someone else gives the injection. Rotate the site with each dose.1

If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.1 Never double up to "catch up", as that can make how you feel harder to read.

07 · What is normal early on

What is normal in the first weeks?

The 2.5mg starting weeks are about adjustment, and experiences differ widely. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects, and they usually settle.1

Most side effects ease within days to a few weeks as the body adjusts. Your own log is the better guide. If symptoms are steady or fading over a few weeks, mention it at your next review. If they are getting worse, do not wait.

Early weight change is also variable. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1

What about "quick debloat" tips and drinks?

Be especially cautious with low-cost supplements sold as gentle alternatives to prescription treatment, as they are not substitutes for prescribed treatment. They come with no clinical oversight, and their dosing is not standardised. Check with a pharmacist before combining anything with your treatment.

If you have a symptom that is not on the patient leaflet's list of side effects, tell your prescriber. You can also report any suspected side effect through the MHRA Yellow Card scheme.19 Reports help regulators monitor safety.

09 · When to seek advice

When should you speak to someone about gas?

Speak to your prescriber if gas is persistent, painful, getting worse or comes with other new symptoms.

Get medical help straight away, by calling 999 or going to A&E, for severe or persistent stomach pain, or if you feel seriously unwell. Do not wait for a routine review in those situations.

You should also mention any change in your other medicines. Insulin or sulfonylureas may need dose adjustments, so tell your prescriber about them. If you take an oral contraceptive, note that Mounjaro slows stomach emptying, which can reduce how well the pill is absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

Putting digestive effects in context

Digestive symptoms are one side of the picture, and results are the other. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

For someone starting at 100 kg, 20.9% is about 21 kg. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

Progress is gradual by design. Think in months, not days, and judge progress by trend rather than individual weigh-ins. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1

Mounjaro is licensed for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition. Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
