Digestive effects are the most common side effects of Mounjaro. Nausea, diarrhoea, vomiting, constipation and abdominal discomfort affect more than 1 in 10 people. Indigestion, reflux and burping are common, affecting up to 1 in 10.1 Bloating is listed as a common side effect in the patient leaflet, so tell your prescriber about any new or lasting symptoms.13

For context on how common digestive effects are, in the weight-management trials nausea was reported by 24.6% of people on 5mg, 29.0% on 10mg and 28.0% on 15mg, compared with 8.5% on placebo. Diarrhoea was reported by 18.7% on 5mg, 20.8% on 10mg and 22.5% on 15mg, compared with 7.8% on placebo.1 These are trial averages and individual experience varies.

For most people, digestive side effects ease within days to a few weeks as the body adjusts.1 They are most likely when starting treatment or moving up a dose, which is why you may notice a pattern: worse for a few days after each increase, then calmer.

Side effects usually appear in the first days after starting or increasing a dose, peak early and settle as your body adapts. This is one reason Mounjaro uses a gradual dose ladder.1 Nausea, vomiting and diarrhoea were more common while the dose was being increased and became less frequent over time in the trials.1

If a new dose is hard to tolerate, prescribers can hold the current dose longer before stepping up. Speak to your clinical team rather than pushing through severe symptoms.1 Everyone starts on 2.5mg weekly, and the dose can increase after at least 4 weeks, in steps of 2.5mg at a minimum of 4-week intervals, up to 15mg. Higher is not automatically better, and many people settle at 5mg, 10mg or 15mg.1

Mounjaro is injected once a week on the same day each week, at any time of day, with or without food, so there is no best time of day for digestion.1 Choose a day that suits you. Some people prefer a day when they can eat lightly in the following 24–48 hours, but that is a personal routine rather than a rule.

A few habits that tie together:

  • Plan lighter, smaller meals for the first days after any dose increase.
  • Rotate injection sites each week: the stomach area at least 5cm from the belly button, the front of the thigh, or the back of the upper arm if someone else injects.1
  • Keep gentle daily movement going, since it supports bowel regularity.
  • Keep a simple note of when symptoms start, and share it with your prescriber.

Missed a dose? Take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip it and take the next one on your normal day. Never take two doses within 3 days of each other.1

Tell your prescriber about every medicine and supplement you take; they assess your medical history and current medicines before deciding whether Mounjaro is suitable.1

First, if you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion). Your prescriber may need to adjust those medicines.1

Second, Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

Most Mounjaro bloating is mild and temporary, but certain symptoms need urgent attention. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting, which could be pancreatitis.1

Also seek immediate help for pain in the upper right abdomen, fever or yellowing of the skin or eyes, which could signal gallbladder problems, or for signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1

Get medical help straight away for bloating that comes with severe or persistent pain. Get medical help straight away if you have gone more than a few days without a bowel movement or cannot keep fluids down — dehydration needs medical help straight away.1 You can also report any suspected side effect through the Yellow Card scheme.19
