How to Get Mounjaro If You Don't Qualify (October 2026)
If you don't meet NHS criteria, you can ask for a private prescription: Mounjaro is licensed for a BMI of 30 or above, or 27–30 with a weight-related condition, which is wider than the NHS threshold of 35 plus a condition.1 If you fall below even that, a prescriber should not supply it. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99). Prices checked 12 October 2026.

A quick overview
What to know
before you start.
-
"Not qualifying" means two different things: missing NHS criteria (BMI 35+ with a condition, phased rollout) or missing the licensed criteria (BMI 30+, or 27–30 with a condition).
-
Many adults who miss NHS criteria still meet the licence, so a private prescription is a legitimate route for them.
-
If you fall below the licensed BMI thresholds, or have a safety reason such as pregnancy, no regulated prescriber should supply Mounjaro.
-
Your first 2.5mg pen costs £129.99 with code FIRST40; every later order is £25 off the standard price.
-
Avoid sellers offering to supply without a consultation or prescription: that is the commonest route to counterfeit pens.
In this guide
01 · The short answer
What to do if you don't qualify for Mounjaro
First work out which test you failed. Missing the NHS threshold does not mean you are unsuitable for the medicine. NICE recommends Mounjaro on the NHS for adults with a BMI of at least 35 plus at least one weight-related condition, and the rollout is being phased in over several years.4 The licence is wider. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition.1
So there are three realistic situations, and each has a different answer.
| Your situation | Can you get Mounjaro? | What to do |
|---|---|---|
| Meet the licence (BMI 30+, or 27–30 with a condition) but not NHS criteria | Yes, privately, if a prescriber judges it suitable | Complete a free online consultation reviewed by a GPhC-registered prescriber |
| Meet NHS criteria but are waiting for access | Possibly on the NHS later; privately now | Ask your GP where you sit in the phased rollout, and decide whether to wait |
| BMI below the licensed thresholds | No, it is outside the licence | Speak to your GP or pharmacist about other support; do not source it from sellers who skip the consultation |
| A safety reason applies (for example pregnancy, or an allergy to tirzepatide) | No | Discuss alternatives with your own clinician |
02 · Private route
What the private route involves and costs
A private prescription starts with a short health questionnaire, which is free and carries no obligation. A GPhC-registered prescriber reviews it. Payment is taken when you place the order, as usual. Unsuitable orders are refunded automatically. If approved, our registered pharmacy dispenses, and we dispatch the same day when you order by 3pm. Delivery is free, next-day, tracked and temperature-controlled, and Saturday delivery is £14.99.
The Mounjaro pen costs £169.99 at the 2.5mg starting dose. New customers save £40 on their first order at any dose with code FIRST40, which is how the first-order price of £129.99 for the starter pen comes about. After that, every future order carries an automatic £25 reorder discount (code DOSE25). All six doses are in stock. You can pay by Visa, Mastercard or Klarna, with pay-monthly options and no subscription.
Costs over time matter more than the first pen. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. A first year costs £2,711.88 if you settle at 5mg, up to £3,599.88 at 15mg. Those totals are before discounts: a new customer's first order is £40 less and every later order £25 less.
03 · Honest limits
Where private prescribing stops
A private prescriber works inside the same licence as the NHS. Mounjaro is for adults and must be used alongside a reduced-calorie diet and increased physical activity.1 A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable.11 That means a decline can happen privately too, and if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Answer every question accurately: the assessment exists to protect you, and the medicine is only appropriate where the licence and your history support it.
If you are still deciding, our guide to getting Mounjaro without diabetes covers a neighbouring question. For the full ordering page, see buy Mounjaro online.
04 · Why you were refused
Why you may have been refused Mounjaro
Most refusals come from one of three causes: the NHS rollout stage, the licensed criteria, or a clinical safety reason. Knowing which applies to you decides what to do next.
The first and commonest cause is the NHS phasing. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 So a GP can tell you that you meet the national recommendation and still not be in the group being treated yet. That is a timing answer, not a judgement that you are unsuitable.
The second cause is the licence. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes.1 If your BMI is below those thresholds, a prescriber has no licensed basis to supply it, whether the service is NHS or private.
The third cause is safety. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive. A prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems.13 If your GP raised one of these, it is worth asking exactly which and whether it is permanent or might change.
When you are refused, ask three things: which criterion did I miss, is it about timing or about suitability, and what do you suggest instead? The answers tell you whether to wait for the NHS, explore a private assessment, or look at a different approach altogether. A GP also holds your full record, which a private prescriber only knows what you tell them, so it is sensible to take a GP's safety concern seriously even if you disagree about eligibility.
Finally, being refused once is not a verdict. BMI changes, conditions are diagnosed and rollout stages widen, so a decision made today may differ in a year. Keep a note of the date and reason you were given so you can revisit it. Our guide on what to do if you can't get Mounjaro goes through practical next steps in more detail.
05 · NHS criteria and timing
How you qualify for Mounjaro on the NHS
You qualify on the NHS if you are an adult with a BMI of at least 35 and at least one weight-related condition, and you fall in a group the local rollout has reached. Both parts matter.
NICE sets the national recommendation: a BMI of at least 35 plus at least one weight-related condition.4 NHS England then phases access so that services are not overwhelmed. The primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions, and full rollout is expected to take years.6
That is why "how quickly can you get Mounjaro on the NHS" has no single answer. Someone in the first group may be offered it now; someone who meets the NICE threshold but sits in a later group may wait for years. Your GP practice or integrated care board is the right source for where you stand. The NHS also publishes general information on the medicine, which is useful background before you speak to them.5
Mounjaro is available on the NHS without a private route being needed, but the phasing is the reason many people who meet the licensed criteria choose regulated private treatment rather than waiting. That is a personal decision involving cost, urgency and health priorities. Waiting is a reasonable choice for some people, particularly where a high-need group is likely to be reached soon.
If you are weighing it up, a few practical steps help:
- Ask your GP to record your BMI and any weight-related conditions accurately, because both feed into eligibility.
- Ask whether your practice has started offering weight-management injections and which group is being offered them first.
- Ask about specialist weight-management services, which can be a separate route from primary care.
- Keep copies of recent readings, such as blood pressure, so a private prescriber can see the same information.
None of this prevents you from seeking a private assessment in parallel. If you later become eligible on the NHS, you can discuss moving over with your GP. Our guide to finding GLP-1 treatment when you don't qualify covers the wider picture beyond Mounjaro.
06 · Licensed private criteria
The licensed criteria for a private prescription
A private prescriber can consider Mounjaro if your BMI is 30 or above, or between 27 and 30 with at least one weight-related condition. That is a lower bar than the NHS recommendation of BMI 35 plus a condition, which is why the private route exists for many people.1
The weight-related conditions named in the licence information include high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes and type 2 diabetes. Mounjaro must be used alongside a reduced-calorie diet and increased physical activity. It is a treatment that supports changes to how you eat and move, not a replacement for them.1
People from South Asian, Chinese, other Asian, Middle Eastern, Black African or African-Caribbean backgrounds are at higher risk of weight-related health problems at a lower BMI, so clinical thresholds are usually reduced by 2.5 kg/m² for these groups. If that applies to you, a BMI just under the standard threshold may still be assessed differently, and you should say so in your consultation.
What the prescriber actually does matters more than the number alone. They assess your BMI, medical history and current medicines before deciding whether Mounjaro is suitable.11 Share your full history, including everything you take, because interactions and conditions can change the answer. In particular, tell them if you or a close relative have had medullary thyroid cancer or the inherited condition MEN 2.
| Measure | NHS recommendation | Licence (private prescribing) |
|---|---|---|
| BMI alone | 35 or above, with a condition | 30 or above |
| BMI with a condition | 35 or above | 27 to 30 with at least one condition |
| Access timing | Phased over several years | Subject to prescriber assessment |
If you sit just below a threshold, resist the temptation to adjust the figures you give. The prescriber's judgement protects you, a decline is refunded automatically, and the decision rests on accurate information. If you are unsure which category you fall into, the free consultation is a no-obligation way to find out, and our guide on getting Mounjaro without diabetes explains why a diabetes diagnosis is not required for weight management.
07 · When the answer is no
When the answer is genuinely no
The answer is no if your BMI is below the licensed thresholds, or if a safety reason applies. In those cases no regulated prescriber should supply Mounjaro, and we would rather say so plainly than imply a workaround.
The safety reasons are these. You should not take Mounjaro if you are allergic to tirzepatide or any of its ingredients, and it should not be used during pregnancy or while trying to conceive.13 A prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. They will also want your full medicine list.
Thyroid history is weighed in the assessment. In rodent studies tirzepatide caused thyroid C-cell tumours; it is not known whether this applies to humans, and no increase has been shown in human trials. As a precaution, you should tell your prescriber if you or a close relative have had medullary thyroid cancer or MEN 2. Routine thyroid monitoring is not required for most people.
If you fall below the licensed BMI, you may be tempted by routes that skip the clinical step: sellers offering to supply without a consultation or prescription. That is unsafe.
What can you do instead? A decline is a prompt to look at other options with a clinician who knows you:
- Ask your GP or a pharmacist about weight and lifestyle support available locally.
- Ask your prescriber whether a different licensed medicine could be appropriate for your situation.
- Revisit the question if your BMI or health conditions change, since eligibility is reassessed against the same criteria.
If you have started something and develop a side effect, report it through the MHRA Yellow Card scheme as well as telling your prescriber.19 If you are pregnant or become pregnant during treatment, stop the medicine and tell your prescriber promptly; Mounjaro should be stopped at least one month before trying to conceive. Seek immediate medical help for severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
08 · Costs over a year
What private Mounjaro costs over a year
Planning the cost before you start avoids surprises, because the dose you settle at changes the monthly price. All figures here are standard prices before discounts, and each pen is four weeks of treatment.
Everyone starts at the 2.5mg dose. The Mounjaro pen at 2.5mg costs £169.99, and your first 2.5mg pen costs £129.99 with code FIRST40. If you are switching to us from another provider at a higher dose, your first order at 15mg is £289.99, because the new-customer discount applies at any dose.
| Period | Pens | Standard cost |
|---|---|---|
| First year if you settle at 5mg | 13 | £2,711.88 |
| First year if you settle at 15mg | 13 | £3,599.88 |
Those totals are before discounts. A new customer's first order is £40 less, and every later order is £25 less than the standard price. For other maintenance doses, the monthly prices are £279.99 at 7.5mg, £290.99 at 10mg and £319.99 at 12.5mg. The weekly equivalents at the standard prices are £42.50 at 2.5mg and £54.75 at 5mg.
How does that compare with elsewhere? Across the UK pharmacies we track, Mounjaro pens ranged from £143.96 to £339.00 per month across all doses and providers, as checked 12 October 2026. For the same dose, the gap between providers ran from £27 to £41. Boots' Mounjaro dose prices ran from £179.00 to £339.00 per month in that index. Those are indexed snapshots, so check current prices before you decide.
The consultation is free, the price shown is the price paid and there are no hidden fees. There is no subscription and no auto-shipping: repeat orders re-check your suitability, and ongoing monthly reviews are part of the service. If cost is the reason you are looking at the private route, work through the year totals first and decide whether you can sustain the dose you are likely to need. Because treatment has to be maintained alongside diet and activity, it is worth planning beyond the first pen.
Eli Lilly raised the UK list price in September 2025 and pharmacies sell at varying prices because of confidential discounts, which is why live prices differ by provider.10
09 · What treatment looks like
What treatment looks like once prescribed
If a prescriber approves you, treatment follows a gradual dose ladder and results build gradually over months. Knowing this helps you decide whether the commitment suits you.
Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it. Many people settle at a maintenance dose of 5mg, 10mg or 15mg; higher is not automatically better, and your prescriber guides the pace.1
The pen is injected under the skin once a week, on the same day each week, at any time of day, with or without food. Each KwikPen contains four weekly doses. Needles, a sharps bin and injection guidance come with every order.13
On results, the SURMOUNT-1 trial of 2,539 adults found that people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. For someone starting at 100 kg, 20.9% is about 21 kg. These are trial averages, individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
Pace is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Think in months, not days, and judge progress by trend rather than individual weigh-ins. If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Early effects are usually mild. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Some people notice appetite change in week one, others feel little until a higher dose, and both are normal.
This matters when you are borderline on eligibility. Treatment is a months-long commitment with reviews, not a quick fix, and a prescriber is more likely to be comfortable where you are ready for that.
10 · Buying safely
How to buy Mounjaro safely and avoid counterfeits
Buy only from a registered pharmacy that requires a consultation and prescription. Counterfeit and unregulated pens are a real risk for people who feel locked out of the official routes, and the MHRA publishes counterfeit medicine alerts.7
Three checks help. First, confirm the pharmacy is on the GPhC public register: ours is registration 9012878, and you can look it up on the GPhC register.12 Second, be wary of anyone offering to supply without a consultation or prescription. Mounjaro is a prescription-only medicine, and a legitimate supplier reviews your suitability first. Third, check how the product is sourced. Our Mounjaro comes through official LillyDirect supply, direct from Eli Lilly's UK distribution, as genuine KwikPens, with all six doses stocked.
- Free online consultation. A short health questionnaire, with no charge and no obligation.
- Prescriber review. A GPhC-registered prescriber reviews it, not a scoring script. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
- Dispensing and delivery. Our registered pharmacy dispenses, with same-day dispatch when ordered by 3pm and free next-day tracked delivery in temperature-controlled, discreet packaging.
Once your pen arrives, store it properly. Keep pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.13 If you want the detail, read what happens if Mounjaro isn't kept in the fridge.
You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Unopened pens should stay in the fridge and keep until the expiry date on the box; if one has been out of the fridge, check with the pharmacy before using it.13
Hydration is also part of staying well on treatment, and our article on drinking enough water on Mounjaro covers it. If you notice a side effect, tell your prescriber and report it via the MHRA Yellow Card scheme.19 Our support team is available Monday to Friday, 9am to 5pm, on 0333 207 0413 or support@everydaymeds.co.uk, and the prescribing team provides ongoing monthly reviews.
Mounjaro (tirzepatide) is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity. Individual results vary. This page is information, not a substitute for medical advice. Explore the full range on our weight loss treatments page.
