What if you don't lose weight on Mounjaro?
If you are not losing weight on Mounjaro, first work out which stage of treatment you are in. Mounjaro is licensed for weight management alongside a reduced-calorie diet and increased physical activity, and individual results vary.1 Three questions sort most cases: what dose are you on, for how long, and has your eating actually changed?
Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 Many people stay on it for a month, and then the dose steps up depending on how they respond and tolerate it. If you are still in those first weeks, patience is the honest answer.

03 · Starting dose reality
The starting dose is not a weight-loss dose
The most common reason for slow early progress is simply where you are on the dose ladder. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Experiences in the first weeks differ widely. Some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal.1 Early loss also includes water weight, so the trend over weeks matters more than any single weigh-in.
Higher is not automatically better. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and your prescriber will guide the right pace.1

04 · Diet still matters
Will you lose weight on Mounjaro if you don't diet?
Mounjaro is a tool for eating less, not a substitute for it. Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite and slow how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 It is licensed for use alongside a reduced-calorie diet and increased physical activity, so someone who ignores both is working against the medicine.
In practice, a lot of stalled progress comes from small leaks in the calorie deficit rather than from the injection failing. Common culprits include:
- Liquid calories. High-calorie coffee drinks are an easy place for the deficit to leak, and plain coffee and tea are fine.1
- Alcohol. It adds calories that work against your goals, and on a reduced appetite and emptier stomach it can hit harder than usual.1
- Large portions of rich food. Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly.1
No food is banned. The aim is a sustainable deficit, not a perfect food list, with protein, vegetables and fibre prioritised at each meal and regular fluids.1
The opposite problem exists too. If your appetite is very low and you are barely eating, that can cause its own difficulties, covered in what happens if you don't eat enough.
05 · Protein and movement
Protein, muscle and activity
Eating very little is not the same as eating well. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance. The two levers are adequate protein, with a protein source at every meal, and resistance exercise two or more times a week.1 This matters more, not less, as the dose rises and appetite falls.
Increased physical activity is part of the licence, not an optional extra. Nothing special is required: the evidence-backed pattern is regular moderate activity, such as brisk walking, plus resistance work for muscle.1 Start where you are and build. On injection days with early side effects, lighter movement still counts.
Exercise also protects against regain if you later stop treatment.1 If you feel your weight is flat but your clothes fit differently, that is worth noting for your review, because the scale is only one measure of change.
06 · Plateaus and speed
Plateaus, and how to speed up progress safely
A plateau is a normal part of gradual weight loss, and the right response is to review habits and trend, not to hunt for a trick. Judge progress by trend, not individual weigh-ins, and weigh on the same day each week under similar conditions.1
Questions like how to shock your body out of a plateau attract plenty of dramatic advice online. The safe, licensed route is plain: keep injecting consistently, hold a deficit, keep protein up, stay active, sleep well, and let your prescriber decide whether a dose change is appropriate. Online "rules" about coffee have no clinical basis.1
Altering doses outside the licence means unknown dosing accuracy and no safety data.1
Check the basics of how you use the pen too. Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food, and the site should be rotated each dose.13 If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day.13
Storage matters as well. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1
07 · Resistance and response
Are some people resistant to Mounjaro?
Yes, response varies, and some people lose far less than the trial averages suggest. Individual results vary, and trial averages do not predict your own result.2 Trials such as SURMOUNT-1 report averages across large groups, which means some participants lost more and some lost less than the headline figure.
If you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1
Note the wording: the highest tolerated dose, not necessarily the highest available. If side effects stop you rising, that is a clinical conversation, and the answer may be to hold a dose rather than push on.

08 · When to get a review
What to do when Mounjaro seems to stop working
Speak to your prescriber if your progress has stalled over many weeks despite consistent injections, a stable diet and activity. Bring your dose history, your weight trend, and honest notes on appetite and eating. A good review looks at dose, tolerability, adherence, diet quality and whether other health factors need attention.
Possible outcomes include continuing at the same dose for longer, adjusting the dose within the licence, or deciding together that treatment is not the right fit. Stopping is a legitimate result of a review, not a failure. After a final dose, tirzepatide takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1
Report any new or worrying symptoms to your prescriber, and you can also report suspected side effects through the MHRA Yellow Card scheme.19 Drinking enough also matters, because reduced appetite means less fluid from food; if you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.

09 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
