04 · Cost of titration

What the first year costs

Your costs depend on how quickly you move up the dose ladder and where you settle. Everyone starts on 2.5mg once a week. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.

05 · NHS or private

Do you have to pay for Mounjaro, or is it on the NHS?

Mounjaro is available on the NHS, but access is being phased in over several years.6 NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

If you are not yet reachable through the NHS, self-pay is the alternative. Choosing it is a personal decision about timing and budget, and you can still talk to your GP at any point. Private treatment does not stop you joining an NHS pathway later.

06 · Why prices differ

Why do Mounjaro prices differ between pharmacies?

Pharmacies buy at a list price set by the manufacturer, and Eli Lilly raised its UK list price for Mounjaro in 2025, with the largest rise on the highest dose.10 Pharmacy sell prices still vary because of confidential discounts, so what a pharmacy pays is not public, and you cannot work out its margin from the shelf price.

That is why the answer to "how much do pharmacies pay for Mounjaro" is not a single published figure. What you can compare is the price you are charged, per dose, and what comes with it. The price index is dated so you can see how current it is (10 October 2026).

When you compare, check the following rather than only the headline price:

  • Whether the price is for the same strength of pen you will actually use.
  • Whether a prescriber review is included and what happens if you are declined.
  • Whether delivery is free and temperature-controlled.

08 · Avoiding scams

How to avoid paying for fake Mounjaro

A very low price is a warning sign, not a bargain. Mounjaro is a prescription-only medicine, supplied only after an online consultation reviewed by a GPhC-registered prescriber. Anyone offering to supply without a consultation or prescription is not operating legitimately.

The MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme for reporting problems.7 Social media sellers, marketplaces and unregulated websites are common routes for counterfeit pens. Before you pay anywhere, confirm the seller on the GPhC register, and be wary of anyone who asks for payment by bank transfer to an individual.

If you notice a side effect after any purchase, you can report it through the Yellow Card scheme.19 Retail marketplaces such as Amazon are not a licensed route to a prescription weight-loss injection, and over-the-counter products are not equivalent to Mounjaro.

09 · Value for money

What do you get for what you pay?

Value depends on what treatment can realistically deliver alongside diet and activity. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages; individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month. Treatment response is reviewed by your prescriber, so you are not paying indefinitely for a dose that is not working for you.

Also consider the cost of stopping. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 Nothing needs doing while it clears, but plan how you will maintain habits once treatment ends.

How to keep the cost down safely

Avoid waste too. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use and must then be thrown away.
