How Mounjaro acts on appetite
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1
That matters for how you read your own experience. The medicine does not work like a stimulant or a caffeine-style pill that you feel within minutes. It keeps working in the background across the week. What most people describe is not a sudden absence of hunger but a change in how quickly they feel satisfied and how often food occupies their thoughts.
Why fullness and hunger are different effects
Slower stomach emptying tends to show up as feeling full sooner at a meal, and sometimes as a feeling of food sitting for longer. Appetite regulation is the quieter effect: less drive to snack, fewer cravings between meals. These can arrive at different times. Why it is not instant like a stimulant
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears. It also means a single missed day of appetite control does not mean the medicine has stopped working.

What the first days commonly feel like
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.13 Altered appetite can mean eating less at meals, losing interest in snacks, or simply feeling that food is less central. Some people find their usual portions feel too large. Others notice taste preferences shifting. These are descriptions of ordinary adjustment, not predictions of your outcome.
Early weight changes are partly water
Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.13 If the scales fall quickly in the first days, do not assume that pace will continue. If they do not move, do not assume nothing is happening. Weigh yourself at a consistent time and look at the direction across several weeks.
A practical first-week approach
- Eat when you are hungry and stop when you feel satisfied, rather than forcing a full plate.
- Keep meals simple and avoid very rich or greasy food if you feel queasy.
- Note how you feel in the days after the injection, since the effects are most noticeable then.

Why you might still be hungry on Mounjaro
The starter dose is not designed to deliver the full effect. It is designed to let your body adapt.
Reasons hunger can persist
The most likely explanation is simply dose. If you are at the beginning of the ladder, you may not yet be on the dose that suits you. Other factors sit alongside it. Portions can drift back up as the early novelty fades. Poor sleep, regular alcohol and high stress all push appetite up independently of any medicine. Meals low in protein can leave you feeling less satisfied.
What to check, in order
- Food intake. Review it honestly. Portions drift back up without us noticing.
- Protein and activity. Protect both. They support how you feel and how your body responds.
- The basics. Look at sleep and alcohol.
- Dose. Discuss with your prescriber whether a step-up is appropriate if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products. Doses should only ever move on your prescriber's advice, within the licence.
Hunger versus habit
Some of what feels like hunger is habit: the biscuit with the tea, the evening snack in front of the television. Mounjaro can make these cues quieter, but it does not erase them. Naming which kind of hunger you are feeling, physical or habitual, is often more useful than any single tweak.

Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.13 Appetite change often comes before visible weight change, and the two should not be confused.
Think in months, not days
Trials measured results at 72 weeks — think in months, not days, and judge progress by trend, not individual weigh-ins.13 If you feel your appetite has dropped but the scales have not moved this week, the two are not in conflict. Early loss includes water weight, and the real signal appears across weeks.
What to expect in the first few months
Most people are still on the lower rungs of the dose ladder at this stage, so trial-average losses are modest compared with the headline figures. In SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2 A sensible 3-month expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.2 Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
How your prescriber reviews response
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.13 That review is a conversation, not a verdict. It is there so that treatment is adjusted to you.
Plateaus are normal
Weight loss on Mounjaro is not linear, and plateaus of a few weeks are normal and expected as the body adapts to a lower weight.13 A stall in the middle of treatment does not mean the appetite effect has gone. Review your intake, protect protein and activity, check sleep and alcohol, and speak to your prescriber.

08 · What the trials show
What the trial evidence tells you
The headline figures come from SURMOUNT-1, and they describe weight, not appetite timing. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
How to read those averages
These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result. Notice too that the trial measured outcomes at 72 weeks. It tells you where treatment can get to over the long term, not how you will feel in the first week.
The distribution matters as well as the average
At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those numbers show that the majority responded at each dose studied, and that higher doses were associated with larger responses. They also show that the dose ladder matters, which is why the first weeks on 2.5mg should not be treated as the whole story.
Beyond the scales
In trials, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels. These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance.

Why supplements are a different category
Products marketed as natural appetite suppressants, including those nicknamed after prescription medicines, are available cheaply without assessment. They are not licensed in the same way, and dramatic weight-loss claims on their packaging should be treated with caution.
Speed is not the same as suitability
If something promises very fast appetite suppression, ask what it is, who has reviewed it, and what happens if you react badly. A regulated pharmacy service should offer a prescriber review, aftercare and a route to report problems. Eating well while your appetite changes
As appetite falls, the goal is to eat well rather than simply less. Protect protein and activity, check the basics such as sleep and alcohol, and review food intake honestly.13 Those principles apply from the first week, not only when progress stalls.
Practical habits that fit the guidance
- Build meals around protein. It supports how satisfied you feel and protects what you want to keep.
- Eat smaller, regular meals if large portions make you queasy, and stop when comfortably full.
- Keep drinking fluids through the day, especially if you eat less.
- Stay active. Increased physical activity is part of how this treatment is used, not an optional extra.
- Be sensible with alcohol, which can affect both appetite and sleep.
Should you fast?

When to be careful
Mild nausea is among the commonest early effects and usually settles.13 If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away. If you have severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of the skin or eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing, seek immediate medical help. A good appetite response should never mean skipping meals entirely or feeling unwell.
Using other medicines
If you take other medicines, including insulin or tablets for diabetes, tell your prescriber, because doses may need adjusting when appetite and eating patterns change. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1

11 · NHS, private and ordering
NHS, private treatment and how to start
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
