Tiredness on treatment usually has more than one cause, usually a mix of the body adjusting and eating less.13 Tirzepatide activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties. In practice most people feel fuller sooner and less hungry between meals.1

That reduced appetite is the point of treatment, but it has a knock-on effect. Eating much less means less fuel for the day, and if protein, fluids or variety fall away, you can feel flat, heavy or foggy. Your body is also adjusting to a new medicine and, often, a new routine.

Several indirect factors commonly sit behind that picture:

  • Lower food intake. Smaller portions can leave you short of energy if meals are not well balanced.
  • Lower fluid intake. Reduced appetite means less fluid from food, so you may be drinking less than you realise.13
  • Digestive effects. Nausea, vomiting and diarrhoea are the most common side effects, and they can leave you drained.1

The most useful steps are practical: keep meals balanced with protein, stay hydrated and keep gentle activity going.13

Protect your protein. No food is banned on treatment, but it helps to prioritise protein at each meal, plus vegetables, fibre and regular fluids.1 Protein is harder to manage when appetite is low, so smaller, protein-led meals often work better than a big plate you cannot finish.

Drink regularly. Drinking enough matters more on Mounjaro than usual, because reduced appetite means less fluid from food. Keep water within reach through the day.1

Eat smaller meals, slowly. Large portions sit heavily because the stomach empties more slowly. Many people feel worse after very fatty, fried or very sugary food, so plain, lower-fat choices are easier while you adjust.1

Keep moving, lightly. Regular moderate activity such as brisk walking helps, and on injection days with early side effects, lighter movement still counts. Start where you are and build.1

Go easy on alcohol. There is no direct interaction with Mounjaro, but drinking on a reduced appetite and emptier stomach can hit harder than usual.1

Protein, muscle and your energy levels

Protecting muscle is part of looking after your energy, because some of any rapid weight loss comes from lean tissue. Protecting muscle protects your metabolism, strength and appearance.1

The two levers are adequate protein, with a protein source at every meal, and resistance exercise on a regular basis. This matters more, not less, as the dose rises and appetite falls.1

Increased physical activity is part of the licence, not an optional extra. Nothing special is required, since the evidence-backed pattern is regular moderate activity plus resistance work for muscle.1

The same logic explains other changes people notice. Temporary hair shedding is linked to rapid weight loss itself rather than a direct drug effect, and nutrition matters there too.1 A loss pace guided by your prescriber, rather than maximum speed, is the approach that protects lean mass.1 Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not eating as little as possible.

07 · When to get help

When tiredness needs a clinical conversation

Tell your clinical team if tiredness is severe or not settling.13 That includes tiredness that stops you doing normal daily tasks, or that arrives with other symptoms.

If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.1 Inability to keep fluids down is a contact-your-clinical-team issue, not something to push through.

Seek immediate medical help for severe, persistent stomach pain that may spread to your back, pain in the upper right abdomen with fever or yellowing of the skin or eyes, or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.1 Tiredness alongside any of these should not be put down to the medicine adjusting.

If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar, which can feel like shakiness, sweating or confusion. Your prescriber may need to adjust those medicines.1

08 · Mood and other causes

Mood, sleep and other causes of fatigue

Not all tiredness on treatment comes from the medicine. Fatigue is common in everyday life, and low mood, poor sleep, stress and other health conditions can all contribute.

On mood specifically, regulators have looked at whether GLP-1 medicines are linked to low mood or suicidal thoughts. The EMA's safety committee reviewed the evidence and concluded in 2024 that it does not support a causal link, and the MHRA keeps this under continuous review.17

If you have thoughts of harming yourself or ending your life, get help straight away. Call 999 or go to A&E if you are in immediate danger. In England you can call NHS 111 and choose the mental health option, any time. You can also call Samaritans free on 116 123, day or night. Tell your prescriber about any change in your mood while taking Mounjaro.

That said, any big change in weight, appetite and routine can affect how you feel. Tell your prescriber about any history of depression, and seek help promptly if your mood drops or you have distressing thoughts during treatment.17

Because low energy and low mood often travel together, it helps to mention both at your review. Equally, if you are sleeping badly, say so, since poor sleep is a common reason for daytime tiredness regardless of what you are taking.

Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
