Is the GLP-1 coffee hack relevant to you?
It is relevant if you are on a GLP-1 style medicine and finding it hard to eat enough protein, and it is largely irrelevant if you hope it will do the work of the medicine. The popular version fills most of a cup with cold brew and tops it up with a ready-to-drink protein shake instead of milk or cream. That is a food choice, nothing more.
The reason it spread is simple. Tirzepatide activates the GIP and GLP-1 receptors, which helps regulate appetite and slows how quickly the stomach empties. Most people feel fuller sooner and less hungry between meals.1 That is helpful for a reduced-calorie diet, but it makes protein harder to reach. A shake in a morning coffee is an easy way to start the day with some.
Nothing in the coffee itself copies that mechanism. "Coffee rules" circulating on social media have no clinical basis.13 So treat the hack as a practical tip. Do not treat it as a substitute for a licensed treatment, and do not expect it to change how hungry you feel.

04 · What is in the hack
What is actually in the GLP-1 coffee hack?
The hack is a cup of cold brew coffee topped up with a ready-made protein shake in place of milk or cream. That is the whole recipe. There is no secret powder, no special bean and no ingredient that acts on your hormones.
Variations exist. Some people use hot coffee and blend in a protein powder. Others add ice and a flavoured shake so it tastes like a dessert drink. Some add spices, cocoa or a splash of plant milk. Each of those is a taste choice. None has a clinical basis for weight loss.
What the version does offer is convenience. Mornings are when many people on treatment feel least like eating. A coffee they already drink can carry protein without needing a plate, a fork or an appetite. For someone who is nauseous or simply not hungry, that is a real practical advantage.
The same logic explains why the recipe is attached to GLP-1 medicines in the first place. These treatments help regulate appetite and slow how quickly the stomach empties, so most people feel fuller sooner and less hungry between meals.1 Fewer meals and smaller portions make hitting a protein target harder. The hack is a workaround for that, not a part of the treatment.
There are also questions people ask that the hack does not answer. A coffee recipe for losing weight does not exist either, because weight loss on treatment comes from the medicine working alongside a reduced-calorie diet and increased physical activity.13 Black coffee is simply a low-effort drink with no interaction with Mounjaro.
If you want to try the hack, choose a shake you can tolerate and drink it slowly. Large portions sit heavily because the stomach empties more slowly, and smaller, slower intake works better. Treat the first few attempts as an experiment and stop if it upsets your stomach. Finally, read the label. A protein shake can range from lean to very rich, and a flavoured coffee drink can quietly become a dessert. Whatever you pour in, it counts towards your day, and that is the best reason to look at it with a clear head rather than a hopeful one.
05 · Protein and muscle
Protein and muscle: the real reason people use it
Protein is the one genuinely sound reason to try the hack. Some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance.13 The two levers are adequate protein and resistance exercise. The advice is to include a protein source at every meal, which is harder when appetite is low.
This matters more, not less, as the dose rises and appetite falls. Early on, on the 2.5mg starter pen, many people still eat fairly normally. As treatment steps up through 5mg, 7.5mg and beyond, appetite can fall further, and it becomes easy to eat very little and very little protein. A protein-containing drink is a low-effort safety net.
That said, a shake in coffee is not the only way, and often not the best. Whole foods bring fibre, texture and other nutrients. Prioritise protein at each meal, vegetables and fibre, and regular fluids, and favour smaller, slower meals.13 If you can manage eggs, yoghurt, fish, poultry, beans or lean meat, those do the same job. The hack is useful on the mornings when you cannot.
Do not overlook the second lever. Protein without resistance work only goes so far. Resistance exercise two or more times a week is part of the picture, and it can be simple: bodyweight moves, bands or weights at home. Increased physical activity is part of the licence, not an optional extra, and the evidence-backed pattern is regular moderate activity such as brisk walking plus resistance work for muscle.13
It is tempting to think a protein coffee substitutes for all of this. It does not. A morning shake with no movement and thin meals the rest of the day is not a muscle-protection plan. Think of it as one small piece.
Because trial weight-loss results are averages, individual results vary. In the SURMOUNT-1 trial, treatment worked alongside diet and exercise, not instead of them.2 The same logic applies to your muscles. The medicine reduces appetite, and what you do with the food you do eat is still up to you.
If protein shakes upset your stomach, say so to your prescribing team at a monthly review. There are other ways to get protein, and side effects should be discussed rather than pushed through.
06 · Coffee, nausea and reflux
Coffee, nausea and reflux on treatment
Coffee is fine on Mounjaro, but strong coffee on an empty stomach can make some people feel worse. Two practical notes apply. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people, and high-calorie coffee drinks are an easy place for the calorie deficit to leak.13
That second point is where the hack can help or hurt. A cup of black coffee adds almost nothing to your day. A large flavoured latte with syrup can add a meal's worth. Swapping cream and sugar for a protein shake can lower the load if the shake is modest, but a rich, sweetened shake can raise it. Check the label rather than assuming the hack is automatically light.
The viral claim that cold brew is easier on reflux is plausible to some but untested. If hot, strong coffee gives you heartburn or nausea, trying a milder or cooler drink, or having it with food, is a sensible experiment. If symptoms persist, mention them to your prescriber. If you have severe, persistent stomach pain, get medical help straight away.
Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly.13 A big, creamy, sweet coffee drink can do the same. If you notice that your morning drink leaves you queasy, the fix may be a smaller portion, less sugar or drinking it more slowly.
Hydration deserves a mention too. Drinking enough matters more on Mounjaro than usual: reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class. Keep water within reach through the day.13 Coffee counts as fluid in everyday terms, but it should not be your only drink. If you cannot keep fluids down, get medical help straight away rather than pushing through.
Alcohol is a separate matter, and some people pair a coffee routine with an evening drink. There is no direct interaction between Mounjaro and alcohol, but drinking on a reduced appetite and an emptier stomach can hit harder than usual, and alcohol adds calories.13 Moderation is the sensible rule.

07 · The cortisol claim
The cortisol claim: is there anything in it?
The cortisol angle is marketing language, not something with a clinical basis. Some versions of the hack are pitched as lowering stress hormones so that belly fat melts away.
It helps to separate what the medicine does from what the internet attaches to it. Tirzepatide activates the GIP and GLP-1 receptors, which help regulate appetite, slow stomach emptying and improve the way the body responds to insulin.1 That is the documented mechanism. A stress-hormone story is not part of it, and a coffee cup cannot add to it.
If you notice a new symptom while on treatment, such as jitteriness, poor sleep or an upset stomach, discuss it with your prescriber. If you have palpitations or a racing heart, get medical help straight away. You can also report suspected side effects through the Yellow Card scheme.19
People who ask about this are often under real stress, and food, sleep and routines do affect how we feel. That is a general life point, not a claim about the hack. Sleeping well, moving regularly and eating regular meals are sensible for many reasons, and none of them needs a branded recipe.
Caffeine can make some people feel wired or anxious, particularly in large amounts or late in the day. That is a general observation about how people feel after strong coffee, and it is another reason to be moderate rather than to chase a trend. If you find coffee unsettling, there is no requirement to drink it on treatment.

The same caution applies to posts promising that a tiny daily trick unlocks fat loss. Treat dramatic weight-loss claims with suspicion. Real treatment is a licensed medicine, prescribed after a consultation, alongside diet and activity, and its trial results are averages that do not predict your own result.
08 · Natural GLP-1 boosters
"Natural GLP-1 boosters" and what they are not
Supplements sold as natural GLP-1 boosters are not equivalents and are not substitutes for prescribed treatment. The coffee hack is often sold alongside powders, mushroom blends, fibres and similar products with big promises, so it is worth being clear about the gap.
Consider what the real medicine does. Tirzepatide is a prescription injection that activates two receptors at once, GIP and GLP-1. It is licensed for weight management in adults and for type 2 diabetes.1 A low-cost supplement with no clinical oversight cannot be assumed to do anything comparable, and dosing for such products is not standardised.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
A drink or supplement with no trial results of this kind cannot be placed next to a licensed medicine.
There is one sensible supplement note. No supplement is required with Mounjaro, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist. Oral supplements, like other oral products, may be absorbed more slowly when you start treatment.13
Be careful about where you buy. Medicines of this kind should only come from a registered pharmacy, and the GPhC register lets you check one. Anyone offering to supply an injectable without a consultation or prescription is a warning sign, and the MHRA publishes counterfeit medicine alerts and runs the Yellow Card scheme.7
If a hack makes you feel you are doing something about weight, that is fine as a motivator. Just do not let it stand in for the steps that carry real evidence: a licensed treatment where suitable, a reduced-calorie diet, protein, movement and regular follow-up.
09 · Building a routine
Building a routine that actually helps
A routine that works is built on meals, movement and follow-up, with coffee as an optional extra. No food is banned on Mounjaro, including bananas, but some choices make treatment easier and more effective. Prioritise protein at each meal, vegetables and fibre, and regular fluids.13
Start with the morning. If you like coffee, have it, and if you struggle to eat early, a protein-containing drink can bridge the gap. Pair it with something small and solid when you can. Smaller, slower meals work better than big ones, because the stomach empties more slowly on treatment.
Through the day, aim for a protein source at each meal. Add vegetables and fibre, and keep water within reach. If appetite is very low, small frequent portions are often easier than a plate you cannot finish. Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.13
Add movement. Nothing special is required: start where you are and build. On injection days with early side effects, lighter movement still counts. Exercise also protects against regain if you later stop treatment.13 Walking most days and some resistance work is a good foundation for nearly everyone.
Watch for leaks. The easiest place for a deficit to disappear is in liquid calories: sweetened coffee drinks, rich shakes and alcohol. If you use the hack, read the label and treat the shake as part of your day's food, not a free addition.

10 · Getting real treatment

If the hack appeals because you want help with weight, the better step is to find out whether licensed treatment suits you. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

11 · Storage and safety
Safety, side effects and reporting
Whether or not you use any coffee hack, the same basic safety rules apply to treatment. Mounjaro is a prescription-only medicine and should be used exactly as prescribed, at the licensed dose, never stretched, split or combined with unlicensed products. Off-label tricks are not safe or supported.
Keep the pen cold until you start it. Store unopened pens in the fridge between 2–8°C until first use, and they keep until the printed expiry date. Once in use, a KwikPen can be kept for up to 30 days (not above 30°C), as the patient information leaflet explains.13 For anything else about storage, such as unopened pens left out of the fridge, check the leaflet and ask the pharmacy rather than guessing.
Know how long the medicine stays. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 No coffee, supplement or drink changes that, and the same is true of claims about "flushing" the medicine out.
Hydration remains central. Drinking enough matters more on treatment than usual, and inability to keep fluids down is a reason to contact your clinical team.13 This is more important than any drink recipe.
Oral contraception is another point many people miss. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 The medicine can reduce absorption of oral contraceptives because stomach emptying slows, so ask your prescriber or pharmacist about what is right for you.7 If you take insulin or a sulfonylurea, doses may need adjusting because of the risk of low blood sugar, so tell the prescriber about every medicine you take.
If you notice a new symptom, whether or not you think it is connected to a drink, discuss it with your prescriber. You can also report a suspected side effect through the MHRA Yellow Card scheme.19
Be sceptical of shortcuts. If a post, seller or video promises specific results from a coffee, powder or pen trick, treat it as marketing rather than medicine. A regulated pharmacy will ask you questions, involve a prescriber and give you clear instructions, including needles, a sharps bin and injection guidance with every order. That is what safe treatment looks like, and it is far more reliable than any viral hack.
12 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
