Does GLP-1 affect cortisol?

The honest answer is that this is not established for the medicines we supply. Mounjaro (tirzepatide) is a once-weekly injectable prescription medicine licensed in the UK for weight management in adults and for type 2 diabetes.1 Cortisol lowering is not among its licensed purposes.

What the product information does describe is the mechanism. Tirzepatide activates GIP and GLP-1 receptors, which help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 Weight loss is the intended outcome. Any change in stress hormones would be a separate question that the label does not answer.

Is cortisol relevant to your treatment?

For most people considering treatment, cortisol is a side issue. Eligibility depends on weight, health conditions and a prescriber's review, not on a hormone test. NICE recommends Mounjaro for adults with a BMI of at least 35 plus at least one weight-related condition, and NHS access is being phased in over several years.4

How stress can work against appetite control

Stress can make dieting harder even when medication is working as intended. Many people notice stronger cravings for comfort food, more evening snacking and poorer sleep when life is demanding. Those patterns are behavioural and familiar, and they matter because Mounjaro works alongside a reduced-calorie diet and increased physical activity, not instead of them.

The mechanism of the medicine is about fullness. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 If stress eating is driven by emotion rather than hunger, a medicine that turns down hunger may help less than you expect.

That is not proof that cortisol "blocks" a GLP-1 medicine. Some websites state that as fact. It is better described as a plausible practical obstacle than as an established drug interaction.

What the weight-loss trials show

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.

Those results were achieved by people living ordinary lives with ordinary stress. Trial averages do not predict your own result.

If your weight is not moving as you hoped, the practical review is about dose, diet, sleep, activity, other medicines and side effects. Your prescribing team can go through these at your monthly review. Hormone testing, if it is needed at all, is a GP decision.

05 · Signs and weight gain

Signs of high cortisol and weight gain

You cannot reliably tell at home whether weight gain comes from cortisol. Ordinary weight gain, medication effects, thyroid problems, sleep loss and hormonal changes can look similar. Self-diagnosis from a checklist is how people end up buying supplements they do not need.

A medical cause of raised cortisol is a recognised condition that a GP can investigate. The signs worth raising are those that are new, persistent and unexplained.

If you have concerns, speak to your GP before starting or changing any weight-loss treatment. If you have new symptoms such as chest pain, a racing heart, fainting or difficulty breathing, get medical help straight away. Tell the prescriber about any hormone condition when you complete a consultation.

06 · Lowering stress safely

Lowering stress without gimmicks

The question "how do I flush cortisol out of my body?" has no quick answer, and anything promising one deserves suspicion. Cortisol is a normal hormone that rises and falls through the day. There is no detox, drink or supplement that we can recommend for "flushing" it.

What helps in general terms is dull but sensible: regular sleep, movement you enjoy, regular meals, time away from screens and talking to someone when pressure builds.

08 · Safety and next steps

Safety, side effects and reporting

If you start Mounjaro and notice a new symptom you cannot explain, tell your prescriber. Whether a symptom links to stress, hormones or the medicine is a clinical judgement, not a guess to make online. The most common side effects, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort, and they are most likely when starting treatment or moving up a dose.13 If you could become pregnant, do not use Mounjaro in pregnancy and stop it at least one month before trying to conceive; if you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed: switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.

You can report any suspected side effect through the MHRA Yellow Card scheme.19 Reports help regulators monitor medicines. They are not a source of proven effects.

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

Does GLP-1 reduce cortisol?
GLP-1 medicines are not licensed to reduce cortisol, and the Mounjaro product information does not list it as an effect. Claims that they do should be treated cautiously. If you are worried about your hormone levels, your GP can arrange appropriate tests and advise on the right next step.
Does GLP-1 work on cortisol levels?
Mounjaro works by activating GIP and GLP-1 receptors to regulate appetite, slow stomach emptying and improve the body's response to insulin. Cortisol is not part of that licensed description, so any link remains unproven for the medicines we supply.
Can high cortisol stop GLP-1 medication working?
