Monthly price alone is a poor guide. The medicines differ in effect, dosing frequency and how many months you may need them, so cost per month and cost per outcome are not the same thing.

In SURMOUNT-5, the first large head-to-head trial, adults on tirzepatide lost on average 20.2% of body weight over 72 weeks versus 13.7% on semaglutide 2.4mg.15 Both are effective, licensed treatments, and which suits you depends on your health profile, tolerance and prescriber advice, not the headline number alone.15 For someone starting at 100 kg, 20.2% is about 20 kg and 13.7% is about 14 kg. These are trial averages and individual results vary.

Mounjaro prices vary widely between providers, even for the same pen. In EverydayMeds' own index of the UK pharmacies we track (checked 11 October 2026), Mounjaro pens cost between £143.96 and £339.00 per month across all doses and providers. The 2.5mg starting pen alone ranges from £143.96 to £179.99.

For any single dose, the gap between the lowest and highest provider runs from £27 to £41. Boots' Mounjaro dose prices run from £179.00 to £339.00 per month.

How to compare providers like for like

Planning a year of treatment

A first year of Mounjaro costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. These totals cover 52 weeks, or 13 pens, at standard prices and before discounts. A new customer's first order is £40 less, and every later order is £25 less.

If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices. If you settle at another maintenance dose, work from the monthly price: £279.99 at 7.5mg, £290.99 at 10mg or £319.99 at 12.5mg.

07 · NHS versus private

NHS access versus private treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.

NHS access to Wegovy runs through specialist weight-management services; ask your GP. If you meet the NHS criteria, ask your GP about your place in the rollout. If you do not, or prefer not to wait, private prescribing is the alternative.

Privately, Mounjaro is licensed for adults with a BMI of 30 or above, or between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It is used alongside a reduced-calorie diet and increased physical activity, and a prescriber assesses suitability first.1

08 · Cheaper alternatives

Cheaper alternatives to Mounjaro

There are licensed alternatives to Mounjaro, but none is a like-for-like substitute. There is no tablet form of Mounjaro; tirzepatide is injection-only in the UK.1 Two GLP-1 weight-loss tablets are now licensed: Wegovy tablets (oral semaglutide), approved by the MHRA in June 2026, and Foundayo (orforglipron), approved in August 2026.2021 They are different medicines with their own dosing, eligibility checks and trial results, and both are prescription-only.

Orlistat blocks some dietary fat absorption and produces smaller average losses. Wegovy and Saxenda (liraglutide) are other licensed injectables; liraglutide is a daily injection supplied at 6mg per millilitre, unlike once-weekly Mounjaro and Wegovy.

09 · Supplements and false savings

Shortcuts that are not savings

"Natural GLP-1" supplements such as berberine, sometimes called the "poor man's Ozempic", are not medicines, are not regulated for this use and have no comparable trial evidence. They are low-cost supplements with no clinical oversight, and dosing is not standardised. Their lower price reflects that gap, not an equivalent product.

Each KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it.

Finally, beware prices that look too good. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites.12 Genuine treatment always involves a prescription and a clinical consultation.

10 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
