Can GLP-1 medicines be used for NAFLD?

GLP-1 medicines are not licensed specifically for NAFLD in the UK, so no pharmacy can honestly sell one as a fatty liver treatment. Mounjaro is a once-weekly injection made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1

That matters because many adults with a fatty liver also carry excess weight. Any effect on the liver is not a licensed use, so it should not be your reason for starting treatment.

What should you do if you have fatty liver?

Start with your GP, because fatty liver needs a proper diagnosis and follow-up. Ask what stage your liver is at, whether you need blood tests or a scan, and whether weight reduction is part of your plan. Do not self-diagnose from a symptom list or an online quiz.

If your GP agrees that weight management is sensible and you meet the licensed criteria, you have two routes. The NHS route exists but is phased. Mounjaro is available on the NHS, but access is being phased in over several years.4 Many people who meet the licensed criteria choose regulated private treatment instead of waiting.

03 · What NAFLD is

What NAFLD is and why weight comes up

NAFLD is a build-up of fat in the liver in people who drink little or no alcohol. It is commonly discussed alongside excess weight, raised blood sugar and other metabolic problems. That overlap is why GLP-1 medicines, which are used for weight and diabetes, keep appearing in liver research.

Many people have a fatty liver without symptoms. It is often picked up by chance on a blood test or scan. Because it can be quiet, you cannot rely on how you feel to judge whether it is improving or worsening. Only your clinician can do that with tests.

The inflammatory form is sometimes called NASH. It is more serious than simple fat build-up, which is another reason a specialist opinion matters if you have been told your liver is inflamed or scarred.

04 · How GLP-1 works

How GLP-1 treatment works

Mounjaro works on gut-hormone pathways that affect appetite and blood sugar. Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together they help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

It is worth knowing how big the weight effect can be, because weight is the likely bridge to liver health. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. This trial did not study fatty liver, so do not read those numbers as liver results.

05 · Weight loss and the liver

Why weight reduction is the usual link

The practical link between GLP-1 treatment and a fatty liver is weight. When body weight comes down, fat stored around the body generally reduces, and clinicians commonly encourage weight reduction for people with a fatty liver. Treatment is meant to sit alongside a reduced-calorie diet and increased physical activity, not replace them.2

That is why the weight route needs a realistic view. Trial averages do not predict your own result, and the SURMOUNT-1 data tell you about weight, not liver tests. Your liver response, if there is one, would need to be checked by your clinician through follow-up tests.

06 · Mounjaro and liver conditions

Mounjaro and an existing liver condition

If you already have a liver diagnosis, the prescriber needs to know before treatment starts. Mounjaro is not suitable for everyone. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Your prescriber will take extra care, or may recommend against treatment, if you have a history of pancreatitis, severe gut or digestive disease, diabetic retinopathy, or gallbladder problems. Is GLP-1 safer than metformin?

What are some signs that my liver is healing?
Fatty liver often causes no symptoms, so how you feel is not a reliable guide. Improvement is judged by your clinician using blood tests, scans or other assessments over time. If you notice new or worrying symptoms, tell your GP or prescriber; for severe or persistent stomach pain, yellowing of the skin or eyes, or swelling of the face or throat, get medical help straight away.
What removes fat from a fatty liver?
There is no single quick fix. Clinicians generally emphasise gradual weight reduction, a healthier diet, regular activity and limiting alcohol, with medical treatment where appropriate. Be cautious of supplements or products promising rapid liver cleansing. Ask your GP or specialist what is suitable for your situation.
Should I tell the prescriber about my fatty liver?

How long does Mounjaro stay in the body if I stop?
After a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 If you stop treatment for any reason, tell your prescriber and your GP, especially if you are being monitored for a liver condition.
