Do GLP-1 medicines affect bone density?
Whether a GLP-1 medicine affects bone density is a question with two separate parts. The first is whether a medicine acts on bone tissue directly. The second is whether the weight change itself, and the eating pattern that goes with it, has knock-on effects for the skeleton. Many pages blur these together. Is bone health a reason to avoid treatment?
For most adults it is a reason to have a more informed conversation, not an automatic reason to stop. Whether treatment suits you is a clinical judgement made on your whole history. That is why every order is reviewed by a GPhC-registered prescriber rather than a scoring script, and why unsuitable orders are refunded automatically.
Some people should be more deliberate than others. If you already know you have thinning bones, have broken a bone easily in the past, take long-term medicines that you have been told affect bone, or have a close relative with osteoporosis, say so clearly in your consultation. The prescriber can then weigh it up with you, and your GP can advise on scans and bone-protecting treatment.

04 · How the question is framed
How the bone question is framed
The bone question has two parts, and keeping them apart makes it easier to read any headline you meet. The first part is direct action: does a medicine act on bone-building or bone-clearing cells? The second is indirect: does losing a lot of weight, and eating less to do so, change how the skeleton is loaded and nourished?
Websites tend to mix these up, which is why the same topic can look alarming on one page and reassuring on another. Those are different questions, with different answers, for different drugs.
That variety is the key point. The term GLP-1 covers several medicines with different structures, doses and trial populations. A result for one drug cannot be assumed to carry across to another, and a result in people with type 2 diabetes cannot be assumed to apply to a healthy adult using a weight-management dose.
The people best placed to answer this for you are your prescriber and GP, who can see your history.
A good habit when you read any bone claim online is to ask three things. Which medicine was studied? In which kind of patient? And does the writer give a source you can open? If any answer is missing, treat the claim as unproven.
05 · What Mounjaro does
What Mounjaro does in the body
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.
Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
This matters for the bone discussion because the effect on appetite is the main way the medicine works. A person who eats noticeably less needs to think about the quality of what they do eat. That is a general principle of any reduced-calorie plan, and it is one reason monthly reviews are part of our aftercare.
This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy/Ozempic). If you are comparing the two, our Wegovy pen page sets out that product separately.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears. If you stop treatment because of a bone concern, your prescriber can talk you through what to expect.
06 · Scale of weight change
Why the scale of weight change matters
The size of the weight change is part of any bone discussion, so it helps to know what the main Mounjaro trial reported. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2
These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2 Trial averages do not predict your own result, so nobody can tell you in advance how much you will lose or what that will mean for your bones.
It is also worth noting how many people reached large losses. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
A substantial change deserves a sensible, reviewed plan.
07 · Protecting bone health
How to look after your bones during treatment
Looking after yourself during GLP-1 treatment starts with doing what the licence already asks. Mounjaro is a prescription-only medicine, supplied after an online consultation reviewed by a GPhC-registered prescriber. It is used alongside a reduced-calorie diet and increased physical activity.
That wording gives you two practical levers. The first is food. A reduced-calorie diet should still be a varied and balanced diet, because eating less makes every meal count for more. If your appetite has fallen sharply, say so at your monthly review so that your prescriber can talk through whether you are eating well enough.
The second lever is activity. Increased physical activity is part of the licensed use, and it is a sensible habit to build while your body is changing. Choose activity you can keep up and that your GP is happy with, particularly if you already have a bone or joint condition.
08 · Osteopenia and osteoporosis questions
Osteopenia and osteoporosis questions we get asked
We are a pharmacy, not a bone clinic, and for diagnosis and bone treatment your GP is the right route.
Can bone loss be reversed? It depends on the cause, your age and your health, and only a clinician who has your scan results can say. Can you rebuild bone with osteopenia? Some people can improve, and some need medicine. What is the new treatment for osteopenia? Treatment choices change, and your GP or a specialist will know current options.
What are the early warning signs of osteoporosis? The condition often causes no symptoms until a bone breaks, which is why people with risk factors are encouraged to speak to their GP rather than wait. A fracture from a minor fall is worth raising.
Who is most at risk? Risk depends on many factors, including age, sex, family history, hormone changes, other medicines and previous fractures. Rather than label a type of person, we would say: if you recognise any of these in your own life, mention it in your consultation.
What about the one vitamin that rebuilds bone, or what people in other countries take? Ask your GP or pharmacist before taking any supplement. Bone density after 70 is likewise a question for a GP, not a pharmacy web page.

09 · Monitoring and reporting
Monitoring, reviews and reporting new symptoms
Treatment with us is not a one-off transaction. Our aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team, plus side-effect support. Repeat orders re-check suitability, and nothing is auto-shipped, so there is a natural point each month to mention anything about your health, including bone or joint concerns. Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing.
Use that review well. Mention any fall or fracture, any new pain you are worried about, any change in appetite or eating, and any new medicine from your GP. Short, specific notes help a prescriber far more than a general worry.
For any new symptom, the right step is simple. We advise you to discuss it with your prescriber and, if you suspect a medicine is involved, to report it through the MHRA Yellow Card scheme.19
Yellow Card reports help regulators spot patterns across the whole country. You can report as a patient, and you do not need to be certain that the medicine is to blame.

10 · NHS and private routes
NHS and private routes, and where bone history fits
Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.
On either route, bone history matters in the same way: it should be disclosed and weighed by a clinician. If you are already under a GP for osteoporosis, ask them how a weight-loss medicine fits with your current care. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full.
