03 · How Mounjaro affects fluids

How Mounjaro can affect what you drink

Mounjaro can affect how much you drink indirectly, through appetite and stomach emptying rather than through a known thirst pathway. Tirzepatide activates two natural gut-hormone receptors, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties and improve the way the body responds to insulin.1

In practice, most people feel fuller sooner and less hungry between meals.1 That is the intended effect, and it supports a reduced-calorie diet. A side effect of a smaller appetite can be a general drop in everything you take in, including drinks and water-rich foods such as soups, fruit and vegetables.

This is worth separating clearly from a claim about thirst itself. Feeling full is a different sensation from feeling thirsty. A person can feel full and still need fluids, and that is the reason a routine is more reliable than waiting for a cue. Slower stomach emptying also means large drinks taken quickly with a meal may feel uncomfortable, so smaller, regular sips are often easier. That is a comfort tip rather than a medical instruction.

04 · Not drinking enough

What happens if you don't drink enough water on Mounjaro?

Not drinking enough fluid is a problem on any medicine. Severe vomiting or diarrhoea can cause dehydration, which can strain the kidneys, so keep fluids up and get medical help straight away if you cannot keep fluids down.

Three things help:

  • Read the section of the patient information leaflet on side effects and warnings, which is the authoritative list.13
  • Tell your prescriber at your monthly review how you are eating and drinking. Dose-titration decisions are made with that information.
  • Report any new or worrying symptom through the Yellow Card scheme as well as to your prescriber.19

If you feel seriously unwell, faint, confused or unable to keep fluids down, do not wait for a review: get medical help straight away.

Because dose increases can bring new gut symptoms, titration is a time to pay attention. Your prescriber decides whether and when to move up, and eating and drinking are part of that conversation.

05 · Sickness and diarrhoea

When fluids matter most: sickness and diarrhoea

Fluids matter most when you are being sick or have loose stools, because that is when you lose more than you take in. The most common side effects of Mounjaro, affecting more than 1 in 10 people, are nausea, diarrhoea, vomiting, constipation and abdominal discomfort.13

06 · Dry mouth and teeth

Dry mouth, "Ozempic tongue" and your teeth

Dry mouth is not on the list of side effects in the UK Mounjaro patient leaflet.13 If you develop it, it is still worth telling your prescriber.

Why does GLP-1 cause dry mouth?

Dry mouth has many ordinary causes, including breathing through the mouth, other medicines, drinking less than usual, caffeine, alcohol and some health conditions. Your pharmacist can look at your whole medicine list if you ask.

Can Mounjaro cause dry mouth at night?

Night-time dry mouth is common for reasons unrelated to any medicine, such as a blocked nose or sleeping with the mouth open. If it began after you started treatment, note when it began and tell your prescriber. Do not assume the cause either way.

What is "Ozempic tongue"?

It is a social-media label, not a term used in the product information. Different people use it for different things, so it does not point to a defined condition. Ozempic itself is semaglutide licensed for type 2 diabetes. It is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. If you have a sore, coated or dry-feeling tongue or mouth, describe your actual symptoms to a prescriber, pharmacist or dentist rather than relying on the nickname.

What fixes dry mouth immediately?

Practical comfort measures are widely used: sipping water through the day, avoiding things that dry the mouth further, and asking a pharmacist about sugar-free products designed for dry mouth. If it is persistent, a dentist or prescriber should look at the cause.

Can Mounjaro affect your teeth?

Dental problems are not listed as a side effect in the UK Mounjaro patient leaflet, but a dry mouth from any cause can make teeth less comfortable to look after, because saliva helps protect them. Keep up regular brushing and dental check-ups, and mention your treatment to your dentist so they have the full picture. Any new symptom can be reported through the Yellow Card scheme.19

07 · Electrolytes

Are electrolytes worth taking on GLP-1 medication?

Whether electrolytes help you is a question for your prescriber or pharmacist. Many products sold as hydration boosters are marketed with strong claims.

There are three sensible questions to ask before buying anything:

  • Do I take other medicines, such as for blood pressure or the kidneys, where extra salts or minerals could matter?
  • Do I have a health condition where a pharmacist would want to check first?
  • Is my problem really a lack of electrolytes, or is it simply that I am not drinking or eating enough?

In many cases, ordinary food and plain water do the job, but your own circumstances decide that. A pharmacist can check suitability alongside everything else you take.

Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed, so raise it with your prescriber if it applies to you.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you also take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so tell your prescriber.

Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them, and individual results vary.2 In the SURMOUNT-1 trial of adults, people taking tirzepatide lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone. These are trial averages, so they do not predict your own result.2 Good habits around food and drink are part of how that lifestyle component works in real life.

09 · Other GLP-1 products

Is this the same for other GLP-1 medicines?

Wegovy, liraglutide and newer medicines each have their own product information, and each is a separate conversation with a prescriber. Do not assume one medicine's leaflet answers questions about another.

Remember also that Mounjaro stays in the body for a while. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system, and nothing needs doing while it clears.1 If you suspect a symptom is linked to treatment, the effect of stopping will not be instant.

When to speak to someone

Speak to your prescriber if thirst, dry mouth or reduced drinking is new, persistent or worrying. You do not need to wait for a scheduled review, and you can contact our support team Monday to Friday during office hours by email, account portal chat or phone.

11 · Serious side effects

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
