GLP-1 is a natural gut hormone, and it is one of two hormone receptors that tirzepatide activates, the other being GIP.1 Together these help regulate appetite, slow how quickly the stomach empties, and improve how the body responds to insulin.1

Foods do not contain GLP-1 in any useful form. Your own gut produces the hormone, and the question is whether what you eat changes that in a way that matters.

What a medicine adds is a sustained signal. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 Food choices then work alongside that effect rather than creating it.

The foods that make treatment easier

The most useful pattern is to prioritise protein at each meal, vegetables and fibre, and regular fluids.1

Protein sources. Eggs, chicken, turkey, white fish, salmon, Greek yoghurt, cottage cheese, tofu, beans and lentils all count. Protein matters because some of any rapid weight loss comes from lean tissue, and protecting muscle protects your metabolism, strength and appearance.1

Vegetables and fibre. Broccoli, spinach, cabbage, carrots, peppers, courgette, oats, pulses and wholegrain bread give volume and fibre without large calorie loads. Soups and stews are gentle options when appetite is low.

Healthy fats in small amounts. A little olive oil, avocado, nuts or oily fish can sit well in a meal. Keep portions modest, because very fatty food is one of the things many people feel worse after.1

Fluids. Water, herbal tea, clear broth and diluted squash all help. Reduced appetite means less fluid from food, so drinking deliberately matters.1

Many people feel worse after very fatty, fried or very sugary food, and large portions sit heavily because the stomach empties more slowly.1 Think chip-shop meals, creamy sauces, pastries, sweets and large takeaway portions. Smaller, slower meals work better.1

Treatment is licensed alongside a reduced-calorie diet, so the aim is a sustainable deficit, not a perfect food list.1 A liquid-calorie habit such as sweet coffees, juices or milkshakes can quietly erase that deficit, because high-calorie coffee drinks are an easy place for it to leak.1

Practically, if a food leaves you nauseous or bloated, eat less of it, eat it more slowly or leave it for another day. That is a comfort decision, not a rule.

07 · Protein and muscle

Protein, muscle and why it matters more as appetite falls

Protecting muscle is a good reason to plan your meals on treatment. Some of any rapid weight loss comes from lean tissue.1 The two levers are adequate protein — aim to include a protein source at every meal, which is harder when appetite is low — and resistance exercise two or more times a week.1

This matters more, not less, as the dose rises and appetite falls.1 A practical way to manage it is to start each meal with the protein item, then add vegetables, then add starch if you still have room. When you can only manage a few mouthfuls, you will have eaten the most valuable part first.

Increased physical activity is part of the licence, not an optional extra. Regular moderate activity such as brisk walking plus resistance work for muscle is the evidence-backed pattern, and lighter movement still counts on injection days with early side effects.1

08 · Fluids and caffeine

Hydration, coffee and alcohol

Drinking enough matters more on Mounjaro than usual. Reduced appetite means less fluid from food, and vomiting or diarrhoea can dehydrate you quickly, which is the main route to the rare kidney problems reported with this class.1 Keep water within reach through the day. If you cannot keep fluids down, contact your clinical team — dehydration needs medical help straight away.

Coffee and tea are fine, as there is no interaction between Mounjaro and caffeine. Strong coffee on an empty, slower-emptying stomach can aggravate nausea or reflux for some people.1

Alcohol has no direct interaction with Mounjaro, but drinking on a reduced appetite and emptier stomach can hit harder than usual, and alcohol adds calories that work against your goals.1 If you also take insulin or a sulfonylurea, alcohol increases the risk of low blood sugar.1 Moderation is the sensible rule.

Supplements marketed as "natural GLP-1 boosters" are not equivalents and are not substitutes for prescribed treatment.1 They are low-cost products with no clinical oversight, and their dosing is not standardised. Dramatic weight-loss claims attached to them should make you cautious.

No supplement is required with Mounjaro, but eating much less can leave gaps, so a standard daily multivitamin is a reasonable safety net while losing weight. Discuss anything beyond that with a pharmacist.1 Oral supplements, like other oral products, may be absorbed more slowly when you start treatment.1

Trial results and how to access treatment

Trial evidence for tirzepatide is alongside diet and exercise. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.2

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.5
