Does Mounjaro interfere with HRT? The short answer
No interaction between Mounjaro and HRT is known, and Mounjaro can be prescribed alongside HRT.1 The same guidance notes that oral medicines in general can be absorbed more slowly when you start treatment.13 That is a general point about tablets, not a finding that HRT stops working. It is the reason your prescriber needs to know everything you take.
Some claim that Mounjaro lowers the absorption of HRT tablets and that the womb lining may then be left unprotected. That is a theoretical worry, not an established interaction. If you take oestrogen with a progestogen and notice unexpected bleeding after starting Mounjaro, do not wait. Speak to the clinician who prescribes your HRT.
Mounjaro slows how quickly the stomach empties, and that is the mechanism behind the general absorption point.1 Where a specific medicine is affected, the label says so. For oral contraceptives it does: the medicine can reduce absorption because stomach emptying slows.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 Do not confuse the two. Tell the prescriber about your HRT before you start. The consultation asks what you take, and the answer shapes the review. Include tablets, patches, gels, sprays, implants and any vaginal oestrogen. Do not stop your HRT to start Mounjaro. Nothing in the product information suggests you should.

03 · How Mounjaro works
How Mounjaro works and why digestion matters
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes. It comes as a pre-filled multi-dose pen (KwikPen) in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1
Tirzepatide activates two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.
The slower stomach emptying is the detail that matters for the HRT question. Anything you swallow leaves the stomach more gradually while the medicine is working. That is the root of the general statement that oral medicines can be absorbed more slowly when you start treatment.13 It means the timing can shift, particularly around the start of treatment and when the dose goes up.
Mounjaro's action is about gut hormones, appetite, stomach emptying and insulin response. It describes no direct action on oestrogen, progesterone or testosterone. When people ask whether Mounjaro messes with hormones, they usually mean one of two things. Either they worry about their sex hormones, or they notice the medicine acts through hormones in the gut. The second is true.
This distinction helps you read headlines. A statement that a medicine slows digestion is not the same as a statement that it blocks a specific drug. Where the label does state a specific effect on another medicine, such as oral contraceptives, you will find it written plainly.7 For HRT, there is no known interaction.1
Dosing is gradual by design. Your prescriber decides the pace. Because the absorption point is strongest at the start and at dose changes, those are the moments to be most attentive to any change in how your other medicines seem to work.
04 · Oral absorption explained
Oral medicines and slower absorption: what it means for you
Oral medicines in general can be absorbed more slowly when you start Mounjaro, and that is the point that applies to tablets and HRT.13 It is worth understanding what that does and does not imply, because the internet tends to inflate it.
Slower absorption is a timing effect. For a few, the label asks you to take extra care. Oral contraceptives are the example the guidance spells out: the medicine can reduce absorption of oral contraceptives because stomach emptying slows.7 If you take the pill, switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. That statement is specific to contraception. We do not stretch it to HRT, and neither should you.
Some online summaries argue that because HRT tablets are swallowed, they must be affected in the same way. They then add that this could leave the womb lining under-protected in women who take a progestogen. That is a chain of inference, not a documented finding. There is no known interaction.1
That does not mean you should ignore your body. If you take HRT that includes a progestogen to protect the womb lining and you see unexpected or heavy bleeding after starting Mounjaro, tell your HRT prescriber promptly. This applies whatever the cause turns out to be. Bleeding that is new for you deserves attention regardless of your other medicines.
Practical points follow from this:
- Keep taking your HRT exactly as prescribed. Do not skip or double doses to compensate for something you have read.
- Declare the form of HRT you use, so the prescriber can judge relevance.
- Mention any change in symptoms, such as hot flushes returning, in your monthly review.
- Report suspected side effects through the Yellow Card scheme, which is open to patients as well as clinicians.19

05 · Menopause and weight
Menopause, weight gain and Mounjaro eligibility
Weight gain around menopause is common and driven by hormonal and metabolic change, which can make diet-only approaches slower.1 If you have noticed that your usual habits no longer work as they once did, you are not imagining it, and you are not failing.
Mounjaro's eligibility criteria are the same at any adult age.1 Being perimenopausal, menopausal or postmenopausal neither disqualifies nor entitles you. What counts is whether you meet the licensed criteria for weight management and whether a GPhC-registered prescriber judges treatment appropriate for you after reviewing your history.
That brings us to several questions people raise about HRT itself. Will HRT make belly fat go away? Why is my stomach so big on HRT? If your abdomen has changed since starting HRT, raise it with the clinician who prescribes it, who can look at your regimen and your overall health.
Similar caution applies to questions such as what age is too late to take oestrogen, what disqualifies you from HRT, whether you can stay on HRT for life, and whether you age faster without it. These are decisions between you and your HRT prescriber, based on your individual risks and benefits. If you are weighing both treatments, make the HRT decision with the clinician who manages it.
A sensible sequence for many people is simple. Keep your menopause care in the hands of the clinician who already provides it. Add Mounjaro only if you meet the criteria and your prescriber agrees. Keep both informed as things change.

06 · Hormones and Mounjaro
Can Mounjaro mess with your hormones?
Mounjaro acts on two gut-hormone receptors, GIP and GLP-1, which regulate appetite, stomach emptying and the body's response to insulin.1 In that sense it works through hormones. Mounjaro is not described as acting on oestrogen or other sex hormones.
People often ask the question for practical reasons. They have noticed that their periods are different, or that their menopause symptoms have shifted, since starting. Many things can change at once when your weight, diet and routine change. Discuss any new symptom with your prescriber and report it through the Yellow Card scheme.19

There is one area where the guidance is explicit and relevant to hormones: contraception. Mounjaro can reduce absorption of oral contraceptives because stomach emptying slows.7 This matters for anyone who could still become pregnant, including in perimenopause, when periods can be irregular but pregnancy remains possible. If that applies to you and you take the pill, switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly. Do not assume HRT acts as contraception. That is a separate matter to settle with the clinician who prescribes your HRT.
Why do we separate these cases? Because they are easy to blur. The statements are different:
- Oral contraceptives: the medicine can reduce absorption, so take advice on contraception.
- HRT: no known interaction, though oral medicines in general can be absorbed more slowly at the start.
- Diabetes medicines such as insulin or sulfonylureas: doses may need adjusting because of the risk of low blood sugar.

07 · Medicines to flag
Which medicines should you tell your prescriber about?
Tell your prescriber about every medicine, supplement and product you use, because that is the only way they can judge what matters. The question of what cannot be taken with Mounjaro has no short list. The answer depends on your full picture, which is why the consultation asks.
Oral medicines in general can be absorbed more slowly at the start of treatment.13 Oral contraceptives can be less well absorbed, so switch to a non-oral method, or add a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.7 Insulin and sulfonylureas may need dose adjustment because of the risk of low blood sugar, which is not the same as being forbidden from combining them with Mounjaro. HRT can be prescribed alongside Mounjaro, with no known interaction.1
What should you include when listing HRT? Be specific:
- The product name and the form you use, such as tablet, patch, gel, spray or implant.
- Whether it contains oestrogen only or a combination with a progestogen.
- Any vaginal preparations or other hormone products.
- Any recent changes to the dose or brand.
Brand changes are worth mentioning, because a switch around the time you start Mounjaro can muddy the picture if your symptoms change. If your HRT and your Mounjaro both change in the same period, it is hard to tell which is responsible for anything you notice. Where possible, avoid changing several things together, and agree timing with your prescribers.

08 · Results and NHS access
What to expect, and NHS versus private access
Trial results show what Mounjaro can do alongside diet and exercise, though your own result will differ. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages: individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2
A worked example makes the percentage concrete. For someone starting at 100 kg, 20.9% is about 21 kg. That is arithmetic on a trial average, not a forecast for you. Trial averages do not predict your own result.
On access, Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting.6
If you are already receiving HRT on the NHS, that does not stop you seeking Mounjaro privately, and it does not oblige you to. It does mean your NHS clinicians should know about it. Telling your GP keeps your record complete and helps them spot anything unexpected.
Treatment is used alongside a reduced-calorie diet and increased physical activity.13 Consider how that fits your life at menopause. Sleep disruption and joint aches can make activity harder, and a realistic plan serves you better than an ambitious one. Your monthly review is the place to adjust it.
09 · Starting and monitoring
Starting Mounjaro while on HRT: monitoring and when to speak up
Starting Mounjaro while on HRT works best when you plan to watch, record and report. Your prescriber sets the pace and may hold you on a dose for longer.
Because absorption of oral medicines can be slower at the start, the early weeks and each dose increase are when to pay closest attention. Speak to a clinician promptly if you notice any of the following:
- Unexpected or heavy bleeding, especially if you take HRT with a progestogen.
- A clear return of menopausal symptoms that had been well controlled.
- Severe or persistent side effects: for severe, persistent stomach pain, yellowing of the skin or eyes, or swelling of the face or throat, get medical help straight away.
- Anything new that you cannot explain.

What happens if you stop? Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system. There is no way to speed this up, and nothing needs doing while it clears.1 Mounjaro manages weight; it does not cure the biology behind it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average.14 Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load. You do not need to alter your HRT because you stop.
Storage is straightforward. Keep unopened pens in the fridge at 2–8°C until first use; they keep until the printed expiry date. Once in use, a KwikPen can be kept out of the fridge at a temperature not above 30°C for up to 30 days after first use, and must then be thrown away. Follow the pen's instructions for injecting.13 Needles, a sharps bin and injection guidance come with every order.

10 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
