It can, if your contraception is a pill you swallow. Mounjaro (tirzepatide) slows how quickly the stomach empties, and UK guidance on GLP-1 and GIP medicines says this can reduce absorption of oral contraceptives.713 A pill that is absorbed less well may protect less well.

The reason is delayed gastric emptying. A pill you swallow has to leave the stomach, pass into the intestine and be absorbed into the blood. Mounjaro slows the first step.1 If the hormone is absorbed more slowly or less completely, the protection it gives can be weaker than intended.

The effect on stomach emptying is the one most relevant when you begin treatment and when your dose goes up. Your contraceptive cover needs to be reviewed at those points, and your prescriber can explain how the leaflet applies to you.13

Saying Mounjaro interferes with birth control or cancels it out overstates the issue. Oral methods may be less reliable and you should take precautions, but not every method stops working.

04 · Oral and non-oral methods

Oral versus non-oral contraception

The concern centres on contraceptive pills taken by mouth. Methods that do not pass through the digestive system are not affected by slower stomach emptying in the same way.7

In practice, the options people discuss with their prescriber include:

  • Combined and progestogen-only pills: taken by mouth, so the absorption concern applies and a non-oral or barrier method is advised.
  • Contraceptive injection, patch and vaginal ring: delivered without relying on the stomach, so they are commonly raised as alternatives. Implants, coils and injections are unaffected.
  • Coil or implant: long-acting methods fitted by a clinician, often considered by people who want to avoid daily tablets.
  • Condoms: a barrier method that does not depend on absorption and can be used as backup protection.

This list is a conversation starter, not a recommendation. The right method depends on your health, your history and what you want from contraception. Your GP, sexual health clinic or pharmacist can go through it with you.

05 · Starting and dose increases

Starting Mounjaro and increasing the dose

Mounjaro is a once-weekly injection supplied in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 Treatment starts at the lowest strength and your prescriber increases it step by step.

Extra protection is needed for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase, so a single check at the start is not enough.7

A practical approach is to put the dose-increase dates in your calendar next to your contraception plan. When a new strength is due, ask yourself whether your protection still covers that period. If you ever stop Mounjaro, the medicine does not vanish immediately. Tirzepatide has a half-life of about five days, so after a final dose it takes roughly four to five weeks to clear the system.1 Ask your prescriber how that affects any decision to change contraception when you finish treatment.

06 · Pregnancy and planning

Can you get pregnant while on Mounjaro?

Yes, it is possible to become pregnant while using Mounjaro, which is exactly why contraception reliability matters. Mounjaro must not be used during pregnancy, and it should be stopped at least one month before trying to conceive. If you become pregnant during treatment, stop the medicine and tell your prescriber promptly.

If you suspect you are pregnant, stop Mounjaro, take a test and tell your prescriber promptly. Do not wait for your next scheduled review. Tell them which contraception you were using and when your last dose and last dose increase were.

If you want to conceive in future, raise it before starting rather than afterwards, as Mounjaro should be stopped at least one month before trying to conceive. Because tirzepatide takes roughly four to five weeks to clear after a final dose, your prescriber can help you plan the timing around that.1

A pregnancy that happens despite contraception while on Mounjaro is worth reporting through the Yellow Card scheme, which is how suspected side effects and unexpected events are monitored in the UK.19
