Yes. Leptin lowers hunger over the longer term by telling the brain that the body holds enough stored energy. It is not a meal-by-meal fullness signal in the way some gut hormones are. Think of it as a background gauge of how much fuel is in reserve, not a switch that stops you eating a second helping.

When body fat falls, leptin falls with it, and the brain reads that as a sign that energy is running short. Hunger tends to rise and the body becomes more efficient with energy. This is one reason weight that has been lost can feel so hard to keep off, and why willpower alone is often the wrong explanation for regain.

When body fat is high, leptin is usually high too. In theory that should keep appetite in check. In practice, many people with obesity still feel hungry, even though leptin is high.

How leptin works in the body

Leptin is a hormone made mainly by fat tissue. The more fat tissue you carry, the more leptin enters the bloodstream. It travels to the brain and acts on the hypothalamus, the region that coordinates hunger, energy use and body weight.

There it does two broad things. It quietens the signals that drive you to seek and eat food, and it supports signals that promote fullness. It also influences how much energy the body spends, so leptin is as much about energy balance as about appetite.

Leptin does not work alone. Other hormones from the gut and pancreas report on each meal, while leptin reports on long-term stores. The brain combines all these messages. That is why describing leptin as 'the' appetite hormone oversimplifies the picture: it is a major contributor in a crowded system.

Sleep, stress and illness also change how hungry you feel, independently of leptin. So a simple 'fix your leptin' message ignores most of what shapes appetite day to day.

04 · Leptin resistance

Why high leptin does not always suppress appetite

In many people with obesity, leptin levels are high but the brain does not respond as strongly. This reduced response is often called leptin resistance. The message is being sent, but it is not fully received, so the brain behaves as if energy stores were lower than they are.

This helps explain why 'high leptin' is not the same as 'good appetite control'. To the question 'is it better to have high leptin or low leptin?', the honest answer is that neither number is the goal on its own. What matters is whether the brain is sensing the signal properly, and that is not something an everyday blood test can tell you.

The idea of leptin resistance is still being researched, and it is not a diagnosis your GP or pharmacist makes in routine care. Be cautious of websites that claim to diagnose it from your symptoms, or that sell a product to 'reverse' it.

What clinicians can assess is your overall picture: weight, BMI, related health conditions, medicines, and previous attempts to lose weight. Those factors guide treatment far more usefully than a hormone label.

05 · Signs of low leptin

What are the signs of low leptin?

Low leptin generally follows low body fat, so the signs are those of the body sensing an energy shortage: strong hunger, preoccupation with food, low energy, feeling cold, and disrupted menstrual cycles in some women. These are not specific to leptin and have many other causes.

Very rarely, people are born with a genetic problem that means they make little or no leptin. This usually shows up in early childhood as intense, relentless hunger and severe weight gain. It is a specialist matter and not what most adults are experiencing.

If you have symptoms such as unexplained fatigue, missed periods or a very restricted diet, speak to your GP. Do not try to self-diagnose from an online list. For adults carrying excess weight, low leptin is usually not the issue; the more common pattern is a weakened response to the signal.

06 · Leptin diets and resets

Leptin diets, 'resets' and what to avoid

There is no evidence-based set of leptin diet rules, list of foods to avoid or way to reset leptin, because the claims behind these plans are not supported by clinical evidence.

Plans of this kind usually share a few features: timed eating windows, cutting certain food groups, and a promise of a hormonal 'reset'. Some of the habits, such as eating regular meals, favouring minimally processed food and sleeping well, are sensible for anyone. The problem is the claim that they repair a specific hormone pathway on a set schedule.

Treat any plan that promises dramatic results, relies on expensive supplements, or tells you to avoid whole categories of food without explanation with caution. A balanced, reduced-calorie diet with more physical activity remains the foundation alongside any licensed treatment, and Mounjaro itself is used in exactly that way.

If you have tried restrictive plans repeatedly and regained weight, that history is useful information for a prescriber, not a failure on your part.

07 · Can you buy leptin?

Can you buy leptin or take it to lose weight?

Leptin is not a licensed weight-loss treatment for ordinary obesity, and we do not supply it. Because most people with obesity already have plenty of circulating leptin, simply adding more is not how treatment is approached.

Products sold online as 'leptin' supplements or 'leptin support' are a different matter. They are low-cost supplements with no clinical oversight, their contents and dosing are not standardised, and they do not replace leptin the hormone. Anything offered to you as the real hormone without a consultation or prescription should be treated as a red flag.

If you want a regulated route, the safer step is a consultation with a prescriber who can discuss licensed options. Counterfeit and unregulated weight-loss products are a recognised risk, and the MHRA publishes alerts and runs the Yellow Card scheme for reporting problems.7

08 · Ozempic, Mounjaro and leptin

Is Ozempic like leptin? How Mounjaro works instead

No. Ozempic (semaglutide) and Mounjaro (tirzepatide) are not leptin and do not replace it. Ozempic is semaglutide licensed for type 2 diabetes — it is not a UK weight-loss medicine, and prescribing it privately for weight loss is off-label. They copy gut hormones released after eating, which act on appetite and digestion through different receptors from leptin's.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1

Other things that suppress appetite

Leptin is only one route to a smaller appetite. Different medicines, drugs and habits reduce hunger through very different mechanisms, and not all are safe or appropriate. The practical point is that suppressing appetite is not automatically a good thing. The aim of a licensed treatment is to bring hunger down to a manageable level so a reduced-calorie diet is sustainable, under a prescriber's review, not to switch it off.

10 · NHS and private access

NHS and private access to treatment

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first (initially BMI 40+ with four or more qualifying conditions).46
