Do you need to let Mounjaro warm up before injecting?
The leaflet does not require a warm-up. It tells you to keep pens in the fridge until first use, never freeze them and keep the cap on between doses.113

If you have already taken a pen out and are unsure whether it is still fine, read the leaflet and ask the pharmacy that supplied it. Do not rely on a day limit you half remember from the internet. Figures circulating online often come from other countries' labels.1

03 · Fridge storage rules
Fridge storage rules for Mounjaro pens
Mounjaro pens belong in the fridge at 2–8°C, and they must never be frozen.1 That is the baseline rule. Whether you let a pen warm before injecting sits on top of it and does not replace it.
A pen that has been frozen should not be used. If a pen has been pushed against the back of the fridge or left near the cooling element, check it carefully and speak to the pharmacy if you are unsure. A frozen pen is not a pen to inject cautiously and hope for the best.13
Keep the cap on to protect the pen from light. This is a good reason not to leave a pen warming on a sunny windowsill. A sensible spot is a shelf or worktop away from direct light and away from radiators, with the cap on.13
Unopened pens keep until the expiry date printed on the box, provided they have been stored in the fridge until first use.1 Always check the date before you start a new pen.
04 · In-use pens out of the fridge
How long can an in-use pen stay out of the fridge?
Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.1 This is the UK licensed figure for the KwikPen. It is useful for short trips, and it also means a pen you are using does not need to go back in the fridge between weekly doses if you would rather keep it at room temperature.
That in-use rule is why warming is not a safety step. A pen kept at room temperature not above 30°C is within its licence for up to 30 days after first use. The thing to watch is the 30-day limit and the temperature ceiling.
Each KwikPen contains four weekly doses. That fits inside the in-use window, so most people finish a pen well before the limit.
05 · Comfort and technique
Making the injection more comfortable
If you find a cold pen uncomfortable, letting it sit out before you inject is a reasonable personal choice. Treat it as comfort only.
Other comfort factors are within your control. Rotate the site with each dose.1 The licensed sites are the stomach area, the front of the thigh, or the back of the upper arm if someone else gives the injection.1 The stomach area should be at least 5cm from the belly button.
Choose a calm moment, sit or stand comfortably and relax the muscles around the site. Follow the pen's instructions for the injection itself, including how long to hold it.
Do not try to warm a pen with a heat source. The leaflet does not ask for it, the pen has a temperature ceiling, and light protection matters. If injections are painful or the site is badly bruised or swollen, tell your prescriber rather than trying home fixes.
06 · Is cold or warm more effective?
Is Mounjaro more effective when cold or warm?
Trial results depend on the dose reached and on staying on treatment alongside diet and activity, not on how long a pen sat on the worktop.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary.
The storage rules exist so the pen stays within the conditions it was tested under: fridge until first use, not above 30°C once in use, never frozen, protected from light. Staying inside those limits is what protects the medicine. Warm versus cold within them is a comfort question.
07 · Delivery and travel
Delivered pens, travel and temperature

If you are away from home, you can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription.1 In-use pens tolerate up to 30 days not above 30°C, so most holidays need no special arrangements. For hot climates use an insulated pouch.
Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. 08 · Timing, food and day
Timing: time of day, food and your injection day
There is no best time of day to inject. Mounjaro works the same morning or evening, with or without food, and what matters is the same day each week.1 Many people choose a weekend morning so early side effects fall on days off. Our page on eating before injecting goes deeper on the food question.
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 So you can inject straight after a meal if that suits you.
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day. Never take two doses within 3 days of each other.1
A long warm-up wait should never push a dose late. A dose delayed slightly for comfort is fine inside the missed-dose window, but there is no need to build the routine around it.
09 · Early weeks and results
How quickly does Mounjaro start working?
The first weeks on 2.5mg are about adjustment. Experiences differ widely: some people notice appetite change and early loss in week one, others feel little until a higher dose, and both are normal.1 Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.
Everyone starts on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
Weight loss on Mounjaro is gradual by design, and most measurable progress builds from 5mg onward, month by month. Trials measured results at 72 weeks, so think in months, not days.2 If you plateau, review portions honestly, protect protein and activity, check sleep and alcohol, and ask your prescriber about a dose step-up. A plateau is not a reason to double doses or add unlicensed products.1
10 · What to avoid
What should you avoid on Mounjaro?
Avoid freezing pens, using a frozen pen, leaving the cap off in the light, and keeping an in-use pen beyond 30 days or above 30°C.1 Those are the storage limits that actually matter.
Avoid altering doses outside the licence.

11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
