A quick overview
What to know
before you start.
-
Mounjaro has no set course length; treatment continues while it helps and your prescriber agrees it is appropriate.
-
The first weeks on 2.5mg are adjustment, so judge progress over months, not days.
-
Your prescriber reviews response and discusses continuing, adjusting or stopping if progress is limited.
-
After a final dose, tirzepatide takes four to five weeks to clear, and nothing speeds this up.
-
Never change doses outside the licence to stretch a course; ask your prescriber about holding or stepping down.
In this guide
01 · The core answer
How long you stay on Mounjaro is decided by your response
You stay on Mounjaro for as long as it is helping and your prescriber agrees that continuing is appropriate. There is a structure: a gradual dose ladder, a period at a dose that suits you, and regular reviews of how you are responding.
The ladder matters because it shapes the early timeline. Everyone starts Mounjaro on 2.5mg once a week.1 That starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1 Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.
Because trials measured results at 72 weeks, a realistic mindset is to plan for many months of treatment, with progress judged by trend rather than individual weigh-ins.2
02 · What to decide
The decisions you are actually facing
| Your situation | What to do | Why |
|---|---|---|
| Just started on 2.5mg and wondering when it ends | Keep going through the ladder and judge over months | The starting dose is for adjustment; most measurable progress builds from 5mg onward |
| Reached the highest dose you tolerate with limited progress | Book a review with your prescriber | A formal response check exists for exactly this point |
| Reached a goal and wondering about stopping | Discuss it with your prescriber before changing anything | The appetite effect depends on the medicine being in your system |
| Struggling with side effects | Ask about holding a dose longer or stepping down within the licence | Your prescriber can adjust within the licence; improvising is not safe |
| Considering a break or a smaller dose to save money | Do not alter doses yourself | Altering doses outside the licence means unknown dosing accuracy and no safety data |
If you want the wider picture on staying on treatment long term, our guide on whether you need to stay on Mounjaro forever goes into that question directly.
03 · Next step
Where to go from here
If you are still deciding whether to start, our weight loss clinic page explains how treatment and reviews work. If you are ready, the Mounjaro pen page shows current doses and prices, with all six doses in stock. Your first 2.5mg pen costs £129.99 with code FIRST40 (standard price £169.99), and every later order is £25 off the standard price. Every order is reviewed by a GPhC-registered prescriber before anything is dispatched.
04 · How treatment is structured
How a Mounjaro course is structured
A Mounjaro course has no fixed length, but it does have a clear shape: a gradual dose ladder, then a period at the dose that works for you, with reviews throughout.
Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, the KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Each KwikPen contains four weekly doses, so a pen lasts roughly a month at any strength.1
The dose ladder is the stepwise climb. Everyone starts on 2.5mg once a week, and after at least 4 weeks the dose increases to 5mg. It can then step up by 2.5mg at a minimum of 4-week intervals, through 7.5mg, 10mg and 12.5mg, up to a maximum of 15mg weekly.1 Those are minimum gaps, not targets. Your prescriber guides the pace, and slower can be entirely appropriate if side effects are an issue.
The second part of the shape is maintenance. Many people settle at a maintenance dose of 5mg, 10mg or 15mg, and higher is not automatically better.1 Nobody is required to climb to the top of the ladder. The right dose is the one you respond to and tolerate.
The mechanism explains why the medicine is used as an ongoing treatment rather than a short burst. Tirzepatide activates two natural gut-hormone receptors at once, GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin. In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.1 That support is delivered by the medicine while it is in your system.
It is used alongside a reduced-calorie diet and increased physical activity, not instead of them, and individual results vary. So the length of your course is really the length of time you want that appetite support, balanced against how well it is working and how you tolerate it.
If you want to see how the first weeks in particular tend to feel, our guide to the first week on Mounjaro covers it.
05 · What the trials show
What the trials tell us about duration
The main trial evidence for weight management measured results at 72 weeks, which is why months, not weeks, is the right unit for planning.
In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them. Trial averages do not predict your own result.
The same trial reported how many people reached meaningful thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2
As a worked example only: for someone starting at 100 kg, 20.9% is about 21 kg. That is the straight arithmetic of the trial average on 15mg, not a forecast, and it says nothing about how fast any individual gets there.
What matters for duration is the pace. Weight loss on Mounjaro is gradual by design. The first weeks on 2.5mg are an adjustment phase and losses are usually modest; most measurable progress builds from 5mg onward, month by month.1 Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures: in SURMOUNT-1, average weight loss built month by month, with the full 15–21% averages measured at 72 weeks, not 12.2
This is why a short course rarely tells the whole story. Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.
Beyond the scales, weight loss on tirzepatide came with average improvements in waist size, blood pressure, cholesterol and blood-sugar measures, and most participants with prediabetes returned to normal blood-sugar levels.2 These are trial-average secondary outcomes, not promises, but they are why prescribers talk about weight management as health management rather than appearance. That framing is also why many people and prescribers think about treatment on a long horizon.
06 · Reviews and response checks
How your prescriber reviews response
Your response is formally reviewed, and that review is the nearest thing to a built-in decision point about how long to continue. It is not a countdown to stopping; it is a check on whether the current plan is working.
Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Notice the wording. The check applies once you have been on the highest dose you tolerate, which is not necessarily 15mg. Someone who tolerates 7.5mg best is judged at 7.5mg.
Between those formal checks, aftercare includes ongoing monthly reviews, dose-titration and follow-up assessments by the prescribing team, plus side-effect support. Repeat orders re-check suitability, and there is no auto-shipping, so you are never automatically carried forward without a fresh look at whether treatment still fits.
Plateaus are the most common reason people wonder whether to stop. Weight loss on Mounjaro is not linear; plateaus of a few weeks are normal and expected as the body adapts to a lower weight.2 Within the licence, the responses are practical:
- Review food intake honestly, because portions drift back up.
- Protect protein and activity.
- Check the basics, such as sleep and alcohol.
- Discuss with your prescriber whether a dose step-up is appropriate if you are not yet at maximum.
A plateau is not a reason to double doses or add unlicensed products. It is also not, on its own, a reason to stop. It is a reason to review.
When judging progress, use the trend. Early loss includes water weight, so the trend over weeks matters more than any single weigh-in.1 Some people notice appetite change and early loss in the first week, others feel little until a higher dose, and both are normal.
In short, the length of your course is shaped by three questions at each review: is it working, are you tolerating it, and do you still want the support it gives. Your prescriber helps answer all three. For a related angle, see our page on how long treatment is usually taken.
07 · What happens when you stop
What happens when you stop Mounjaro
When you stop, the medicine clears over several weeks and its appetite-regulating action fades with it. In SURMOUNT-4, people who stopped tirzepatide after 36 weeks regained on average 14% of body weight over the following year, while those who continued treatment lost a further 5.5% on average. Stopping should always be planned with your prescriber, with diet and activity habits ready to carry the load.
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 In plain terms, you will not feel an abrupt switch-off on the day after your last injection. The change is gradual, over the four to five weeks the medicine takes to leave.
The reason stopping deserves thought is the mechanism. The medicine helps regulate appetite, slows how quickly the stomach empties and improves how the body responds to insulin.1 Once it has cleared, those effects are no longer there to support a reduced-calorie diet. Habits you have built, such as portion awareness, protein and activity, are what carry forward, which is why they are worth protecting throughout treatment rather than only at the end.
Practical points for stopping well:
- Talk to your prescriber first, so the decision is shared rather than abrupt.
- Ask whether stepping down within the licence suits you better than stopping at a high dose.
- Keep the habits that made treatment work: regular meals, protein, activity, sleep.
- Keep monitoring the trend of your weight so you notice changes early and can seek advice.
Stopping is not a failure, and restarting later is a conversation to have with a prescriber rather than something to do from leftover pens. If you have been on treatment for a while and are unsure, our related guide on how long to use Mounjaro covers the planning side.
08 · Breaks and missed doses
Breaks, missed doses and changing your day
If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1 That rule is for genuine accidents, not for planning a gap.
You can change your weekly injection day if needed, as long as at least 3 days (72 hours) have passed since your last dose.1 That is useful if your routine changes, and it does not require stopping.
Mounjaro is injected under the skin once a week, on the same day each week, at any time of day, with or without food.1 There is no best time of day. Mounjaro works the same morning or evening. What matters is the same day each week, so pick a day and time you will remember. Many people choose a weekend morning so early side effects fall on days off.
People sometimes try to make a pen last longer or take smaller amounts to ease side effects or cost. It is not licensed, not supported by the pen's design and not endorsed by any UK regulator.1 Altering doses outside the licence means unknown dosing accuracy and no safety data.
If you think you need a break, for illness, surgery or another reason, speak to your prescriber rather than deciding alone. That is a clinical decision with consequences for your dose plan.
09 · Cost of staying on treatment
What staying on Mounjaro costs, and the NHS alternative
Cost is a real part of how long people stay on treatment, so it is worth planning. If you step up after 4 weeks on each dose, your first four pens (2.5mg to 10mg) cost £959.96 at standard prices, before discounts.
For a longer view, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. Those are standard prices before discounts. A new customer's first order is £40 less with code FIRST40, and an automatic £25 reorder discount applies to every future order (code DOSE25). If you settle at another maintenance dose, work from that dose's monthly price: £279.99 for 7.5mg, £290.99 for 10mg and £319.99 for 12.5mg, each a standard price for one pen.
Prices were checked 12 October 2026, and you can see live figures on the Mounjaro pen page.
Payment options include Visa and Mastercard, Klarna and pay-monthly options, and there are no subscriptions. That means you are not locked into continuing: each repeat order re-checks suitability and nothing ships automatically, so you can pause or stop ordering when you and your prescriber decide to.
The NHS route is the other path. Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition, and the NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.4 Full rollout is expected to take years, which is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you are in a priority group, ask your GP about NHS access first.
10 · Safety over a longer course
Safety, side effects and storage on a longer course
The side effects most people notice are early ones, and they usually settle.
Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1 If something new appears later, or an effect does not settle, tell your prescriber. You can also report suspected side effects through the MHRA Yellow Card scheme.19
Storage matters across many months of pens. Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, useful for short trips, after which it should be discarded. Keep the pen cap on to protect it from light, and never use a pen that has been frozen.1 Our guide on whether Mounjaro needs to stay in the fridge goes into more detail.
Travel is manageable on a long course. You can fly with Mounjaro. Carry pens in hand luggage, because hold luggage can fall below freezing, ideally in a cool bag with the prescription label or a copy of your prescription. Unopened pens should be kept in the fridge and keep until the expiry date printed on the box. If an unopened pen has been out of the fridge, check with the pharmacy before using it rather than relying on a remembered day limit, since figures circulating online often come from other countries' labels.13
Finally, supply matters for continuity. Our Mounjaro comes through official LillyDirect supply, direct from Eli Lilly's UK distribution, as genuine KwikPens, with free next-day tracked delivery in temperature-controlled packaging. Needles, sharps bin and injection guidance are included with every order, and the GPhC-registered pharmacy (GPhC 9012878) can be checked on the public register.
11 · Serious side effects
Serious side effects: when to get help straight away
Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
