You need a higher dose only if your current one is no longer doing enough and you are tolerating it well. If your weight is trending down and your appetite feels controlled, staying where you are is a perfectly valid plan. The licensed dose ladder is a set of options, not a target to reach.1

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.111 Many people settle at a maintenance dose of 5mg, 10mg or 15mg. Higher is not automatically better, and your prescriber will guide the right pace.

So the real question is not "how high can I go?" but "is my current dose working, and is it comfortable?"

Why some people need a higher dose and others do not

People respond differently to the same dose, which is why the ladder exists. Some notice appetite change and early loss straight away, while others feel little until a higher dose. Both are normal.1

Because response varies, no schedule can tell you in advance where you will settle. Your prescriber looks at how you are tolerating treatment, how your appetite behaves across the week and how your weight is trending, then decides whether to hold or step up.

03 · How titration works

How Mounjaro titration works

Titration is the gradual stepping up of your dose, and it is built into the licence. The pen is supplied in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg. Each KwikPen contains four weekly doses, so a pen at one strength normally covers a month.1

The early part of the ladder is deliberately slow. The 2.5mg start exists for tolerability, and the minimum gap between steps protects you from moving too fast. Moving up at the earliest permitted point is allowed, not required. If you are still settling in, your prescriber can keep you on a dose for longer.11

04 · What the trials show

What the trial results show about dose

In the main trial, average results rose with dose, but every dose group lost meaningful weight. In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages — individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Those averages come from groups of people, so they do not predict your own result. For someone starting at 100 kg, 15% is about 15 kg and 20.9% is about 21 kg, but that is simple arithmetic on a trial average, not a forecast.2

05 · Plateaus and hunger

Plateaus, hunger and when to raise it

A plateau or returning hunger is the most common reason a prescriber considers a step up. Weight loss on Mounjaro is gradual by design. The early weeks on 2.5mg are an adjustment phase and losses are usually modest. Most measurable progress builds from 5mg onward, month by month.1

Trials measured results at 72 weeks, so think in months, not days, and judge progress by trend, not individual weigh-ins. A quiet stretch in the scales is not automatically a failure of the dose. Water, hormones, sleep, salt and bowel habits all move the number around.

Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.4

Before asking for more, it is worth checking the basics: are you injecting on the same day each week, eating enough protein and fibre, staying active and sleeping reasonably? A higher dose works alongside a reduced-calorie diet and more activity, not instead of them.

Staying put is often the right call when side effects rise faster than benefits. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle.1

Other good reasons to stay where you are include: you are happy with your rate of progress, your appetite is well managed, or you are close to your goal and want to maintain. Maintenance on a modest dose is common and fully within the licence.

07 · What not to do

What not to do if you feel you need more

Do not adjust the dose yourself.

Altering doses outside the licence means unknown dosing accuracy and no safety data.1

Equally, do not add an extra injection because the appetite has crept back. Missing a dose is handled by a clear rule. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.1

Serious side effects: when to get help straight away

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1

What should I do if I get side effects after a dose increase?
Side effects are most likely when starting treatment or moving up a dose, and for most people they ease within days to a few weeks.
