The trial also reported how many people reached particular thresholds. At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Notice how close those three figures sit. Most people on every maintenance dose reached a meaningful loss. The higher doses added a few more people to that group, but the lower dose already worked for the large majority.
The deeper thresholds tell a different story. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Here the higher dose does more to push people into the very large loss category.
What does this mean in plain terms? Dose matters, and it matters most for people aiming at the largest reductions. But the lower strengths are not weak. They produced substantial average losses in the same trial, which is why many people never need to go to the top of the ladder.

Everyone starts Mounjaro on 2.5mg once a week. This starting dose is about letting your body adjust, not about weight loss itself.1 That detail answers a question many people have: you cannot begin on a high dose to get faster results. After at least 4 weeks the dose increases to 5mg, and can then step up by 2.5mg at a minimum of 4-week intervals (7.5mg, 10mg, 12.5mg) up to a maximum of 15mg weekly, depending on how you respond and tolerate it.1
The six strengths

Three months in, most people are still on the lower rungs of the dose ladder, so trial-average losses at this stage are modest compared with the headline figures.1 In SURMOUNT-1, average weight loss built month by month, and the full averages were measured at 72 weeks, not at three months.2 A sensible expectation at this stage on a standard titration is mid single digits as a percentage of body weight, with wide individual variation.1
Slower early progress does not predict failure. The dose you respond to may still be ahead of you.1 That sentence is worth remembering if you feel behind. The ladder is designed so that the larger changes tend to arrive as the dose rises, month by month.
Why speed is the wrong question
Think in months, not days.
Rushing the ladder is not an option in any case. The licence sets a minimum interval between steps, and the starting dose is about letting your body adjust. Skipping ahead is not supported.
The formal review point
Your prescriber reviews how treatment is going. Treatment response is reviewed by your prescriber: if you have lost less than 5% of your starting weight after 6 months on the highest dose you can tolerate, your prescriber will discuss whether continuing or stopping is right for you.1 Note the wording: the highest tolerated dose, not the highest licensed dose. This is also where NICE guidance sits in the background. NICE recommends tirzepatide for adults with a BMI of at least 35 plus at least one weight-related condition, as part of a defined care pathway.4 Whether you are treated on the NHS or privately, the principle of a structured review is the same.

Weight is not the only thing that changes. What not to do

Missed doses and changing your day
Plateaus are sometimes blamed on irregular dosing, so keep the routine tidy. If you miss a dose, take it as soon as you can within 4 days (96 hours) of the scheduled time, then carry on with your usual day. If more than 4 days have passed, skip the missed dose entirely and take the next one on your normal day — never take two doses within 3 days of each other.13

10 · Tolerability at higher doses
Dose, side effects and tolerability
Dose and tolerability are linked, which is why the ladder is gradual. The licence says the dose can step up depending on how you respond and tolerate it.1 So tolerability is not an afterthought; it is one of the two things that decide your dose.
The commonest early effects are mild. Because tirzepatide slows how quickly the stomach empties, it can reduce absorption of oral contraceptives.7 The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1 If you take insulin or sulfonylureas, doses may need adjusting because of the risk of low blood sugar, so tell your prescriber about everything you take.1 These considerations apply at every dose, and a step up is a natural moment to review them with your prescriber.
Stopping and clearance
Tirzepatide has a half-life of about five days, so after a final dose it takes roughly 4–5 weeks to clear the system — there is no way to speed this up, and nothing needs doing while it clears.1 This is why changes to your plan are made calmly with your prescriber rather than abruptly.
Storage across doses
Keep Mounjaro pens in a fridge at 2–8°C and never freeze them. Once in use, a pen can be kept out of the fridge not above 30°C for up to 30 days, after which it should be discarded.13 Keep the cap on to protect the pen from light. Higher-strength pens are stored exactly like lower ones.
Tolerability is personal, and it often improves with time on a dose. That is a good reason to give each step a fair period before deciding whether to move on.

Serious side effects are uncommon but need urgent attention. Seek immediate medical help for: severe, persistent stomach pain that may spread to your back, with or without vomiting (possible pancreatitis); pain in the upper right abdomen, fever or yellowing of skin/eyes (possible gallbladder problems); or signs of a severe allergic reaction such as swelling of the face or throat and difficulty breathing. If you take insulin or a sulfonylurea alongside Mounjaro, watch for low blood sugar (shakiness, sweating, confusion) — your prescriber may need to adjust those medicines.1
