Thinking in months rather than only the first pen is the most useful habit when you are chasing the lowest cost. A pen lasts four weeks, so a year is 52 weeks and 13 pens. At standard prices, a first year costs £2,711.88 if you stay on 5mg after your 2.5mg starting pen, up to £3,599.88 if you step up through every dose to 15mg. Those are standard-price totals, so your real figure is lower once the first-order and reorder discounts apply.

What Mounjaro is and how it works

Mounjaro is the brand name for tirzepatide, a once-weekly injectable prescription medicine made by Eli Lilly. In the UK it is licensed for weight management in adults and for type 2 diabetes.1 It comes as a pre-filled multi-dose pen, called a KwikPen, in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.13

Knowing this helps when you compare prices. Every pen is a four-week supply, and the strength you pay for depends on the dose your prescriber chooses. A pen is not a flexible quantity that you can stretch or split. It is built to deliver four weekly doses.

Tirzepatide is the first treatment of its kind to activate two natural gut-hormone receptors at once: GIP and GLP-1. Together these help regulate appetite, slow how quickly the stomach empties, and improve the way the body responds to insulin.1 In practice, most people feel fuller sooner and less hungry between meals, which supports a reduced-calorie diet.

This dual action is what distinguishes Mounjaro from single-action GLP-1 medicines such as semaglutide (Wegovy and Ozempic). That is a difference in mechanism, not a claim that one is right for everyone. Your prescriber decides which is appropriate for you.

Tirzepatide has a half-life of about five days. After a final dose it takes roughly four to five weeks to clear the system.1 There is no way to speed this up, and nothing needs doing while it clears. This matters for the cost question too. If you stop treatment, the medicine does not disappear overnight, and stopping should always be planned with your prescriber.

Why does eligibility matter when you are comparing prices? Because the cheapest pen is no use if it is not suitable for you. A genuine pharmacy will not supply on price alone. A prescriber will assess your BMI, medical history and current medicines before deciding whether Mounjaro is suitable.

  • BMI of 30 or above. You may be considered for weight management with Mounjaro, subject to the prescriber's review of your history and medicines.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages. Individual results vary, and treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.

Cost only makes sense against what you are paying for. Knowing the trial figures helps you set realistic expectations, so you can judge whether a year of treatment is worth it for you.

At 72 weeks in SURMOUNT-1, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight.2 Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2

To put a percentage in concrete terms, take a worked example. For someone starting at 100 kg, 20.9% is about 21 kg. That is straight arithmetic from the trial average, not a prediction for you. Trial averages do not predict your own result, and some people lose less while others lose more.

The trial also tells you something about dose. The average loss was higher at 10mg and 15mg than at 5mg. But that does not mean you should aim for a higher dose to get more value. Your prescriber chooses your dose based on how you respond and tolerate treatment.

Mounjaro is available on the NHS, but access is being phased in over several years. NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.

That is why many people who meet the licensed criteria choose regulated private treatment rather than waiting. If you qualify on the NHS and can access it, that is a route to consider, and your GP is the person to ask. If you cannot access it yet, a regulated private pharmacy is a safer option than an unverified seller.

A common question is whether people with diabetes can get Mounjaro on the NHS. Mounjaro is licensed for type 2 diabetes as well as weight management, and type 2 diabetes is also one of the weight-related conditions that can support eligibility for weight management. Yes, semaglutide is sold in the UK under brand names such as Wegovy and Ozempic, but it is prescription-only, so it needs a consultation and prescriber approval. Wegovy pens step up after at least four weeks from 0.25mg to the 2.4mg maintenance dose, reached from week 17. NHS access to Wegovy runs through specialist weight-management services, so ask your GP. What should I avoid while taking Mounjaro?
Avoid any pen that looks cloudy, has particles, has been frozen, is past expiry, has been above 30°C, or has been out of the fridge for more than 30 days in use. Tell your prescriber about all your medicines. Mounjaro slows stomach emptying, which can reduce how well oral contraceptive pills are absorbed. The SmPC advises switching to a non-oral method, or adding a barrier method (such as condoms), for 4 weeks after starting Mounjaro and for 4 weeks after each dose increase.1
What is known as the "poor man's Ozempic"?
The nickname is applied to low-cost supplements such as berberine, which are marketed with dramatic weight-loss claims. They are available cheaply without assessment, have no clinical oversight, and their dosing is not standardised. They are not the same as a licensed prescription medicine.
Is the new-customer discount only for the 2.5mg dose?
No. A new customer gets £40 off their first order at any dose with code FIRST40, including people switching to us from another provider at a higher strength. After that, every future order gets an automatic £25 off the standard price (code DOSE25). If you are switching to us at 15mg, your first order is £289.99, which is a first-order price rather than a usual monthly one.
Will I be charged if the prescriber says no?
No. Payment is taken when you place the order; if the prescriber decides the treatment is not suitable, the order is refunded automatically in full. Once a prescription medicine has been dispensed it cannot be returned, except if it is faulty, damaged or the wrong item, which you should report within 3 days.
