So the figure that matters is what you will pay across your whole course, not the advertised price of the 2.5mg starter pen alone.

The table shows why the starter price is a poor guide. You will not stay on 2.5mg, because it is a starting dose. Your prescriber decides when to move up, and the dose where you settle decides most of your yearly cost. Many programs advertise only the lowest dose, so ask for the full dose ladder before comparing.

04 · Comparing total cost

How to compare GLP-1 programs on total cost

The fairest comparison is the total you will pay for the doses you will actually take, so build the comparison around three numbers. The first is the price of each dose step. The second is how many months you are likely to spend at each step. The third is everything else the provider charges. Mounjaro is licensed for weight management as a once-weekly injectable in six strengths: 2.5mg, 5mg, 7.5mg, 10mg, 12.5mg and 15mg.1 A program that quotes only one of them is not giving you enough to compare.

Start by asking for the whole dose ladder. A low starter price can hide a steep climb later. Our GLP-1 program guide explains how the stages fit together. Next, look at the fixed costs. Does the consultation cost anything? Is delivery free? Are needles and a sharps bin included? Do monthly reviews cost extra? Finally, compare like with like on the date. Prices move, so a comparison is only useful when it is dated. Our index figures were checked 10 October 2026. That spread is why it pays to compare the dose you will actually be on, rather than whichever price headlines a page.

A short checklist helps. Write down the dose ladder prices, the first-order price, the repeat-order price, the delivery terms and the review arrangements. If another provider's total comes out lower for the same course and the same genuine product, that is a fair reason to choose them. If it is lower only because the dose ladder is missing, it is not a real comparison.

05 · Why prices changed

Why Mounjaro prices rose in September 2025

Mounjaro prices rose because Eli Lilly increased the UK list price. On 14 August 2025 Lilly announced an agreement to raise it, effective 1 September 2025. That change explains why older articles quoting very low monthly prices are out of date.

Pharmacy sell prices still vary, because pharmacies have confidential discounts and set their own margins. This is why you see a spread between providers for the same dose. Across the UK pharmacies we track, the cheapest and dearest Mounjaro pens run from £143.96 to £339.00 per month, with the starting 2.5mg pen between £143.96 and £179.99. We show our own prices live, checked 10 October 2026, so what you see is current.

What does this mean for someone hunting for the cheapest program? First, treat any price you find in an old blog post, video or social post with caution; it may predate the change. Second, the gap between low and high doses is now wide, so your maintenance dose matters even more than it did. Third, do not assume the lowest-looking price is a bargain if it comes from a seller you cannot verify. A price far below the rest of the market is a reason to check the pharmacy on the GPhC register.

It is also worth separating the list price from what you pay. The list price is what Lilly charges the supply chain. What you pay includes the pharmacy's costs: the prescriber review, dispensing, cold-chain packaging, delivery and support. Two pharmacies buying the same pen can end up with different sell prices, and a program that includes reviews and delivery in the price may suit you better than one that adds them on.

06 · Dose, results and cost

How your dose changes what you pay

Your dose is the biggest lever on your monthly bill, so it helps to know what each dose is for.

In the SURMOUNT-1 trial of 2,539 adults, people taking tirzepatide alongside diet and exercise lost on average 15% of their body weight on 5mg, 19.5% on 10mg and 20.9% on 15mg over 72 weeks, compared with 3.1% with lifestyle changes alone.2 These are trial averages, and individual results vary. For someone starting at 100 kg, 20.9% is about 21 kg. Treatment works alongside a reduced-calorie diet and increased physical activity, not instead of them.2

At 72 weeks, 85% of participants on 5mg, 89% on 10mg and 91% on 15mg had lost at least 5% of their body weight. Half of participants on 10mg and 57% on 15mg lost at least 20% of their body weight.2 Those figures describe groups, and trial averages do not predict your own result.

What does this mean for cost? Do not choose a dose by price. Your prescriber chooses the dose that suits you, based on your response and how you tolerate it. A higher dose is not automatically better for you, and a lower dose is not automatically a waste. If you settle at 5mg, a first year costs £2,711.88 at standard prices, and if you settle at 15mg, up to £3,599.88. In between, 7.5mg is £279.99 per month, 10mg is £290.99 and 12.5mg is £319.99.

07 · Unsafe shortcuts

Cost-cutting shortcuts that are not worth the risk

Mounjaro is licensed as a once-weekly medicine, and its blood levels are designed around a 7-day cycle.

Each KwikPen is designed and licensed to deliver exactly four weekly doses. Use the pen as designed, then dispose of it. A sharps bin is included with every order.

Talk to your clinical team rather than self-adjusting. Because tirzepatide has a half-life of about five days, it takes roughly 4–5 weeks to clear the system after a final dose, and there is no way to speed this up.1 That is one more reason to plan any change with a prescriber instead of improvising.

The other cost-cutting trap is a suspiciously cheap pen from an unverified seller. The MHRA has seized thousands of fake weight-loss pens in the UK and has issued alerts after falsified Mounjaro pens were identified. Fake pens have caused hospitalisations, and some contained insulin rather than tirzepatide.7 Only buy from a pharmacy you can verify on the GPhC register, never from social media, beauty salons or unverified websites. Genuine treatment always involves a prescription and a clinical consultation, so anyone offering to supply without a consultation or prescription should be avoided.

Saving on a pen only counts if the pen is real. A pharmacy-supplied pen carries a batch number and expiry date you can check, and arrives in cold-chain packaging. If a low price depends on skipping the clinical steps, the true cost could be a hospital visit. The legitimate ways to pay less are discounts, spreading payment through Klarna or pay-monthly options, and comparing dated prices.

08 · NHS or private

NHS or private: which route costs less

The NHS route costs the least out of pocket if you qualify, but access is being phased in over several years. Mounjaro is available on the NHS, and NICE recommends it for adults with a BMI of at least 35 plus at least one weight-related condition.4 The NHS primary-care rollout began in June 2025 with the highest-need groups first, initially BMI 40+ with four or more qualifying conditions.6 Full rollout is expected to take years.

That staging means many people who meet the licensed criteria choose regulated private treatment rather than waiting. It is a personal choice, and the two routes are not in competition. If you are in a priority group, ask your GP what the local position is. If you are not yet, or would prefer to start sooner, a private prescription from a registered pharmacy is the alternative. The NHS guide to the medicine is a useful read either way.5

The licence for private prescribing is wider than the NHS criteria. Mounjaro is licensed for weight management in adults with a BMI of 30 or above, or a BMI between 27 and 30 with at least one weight-related condition such as high blood pressure, high cholesterol, obstructive sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It must be used alongside a reduced-calorie diet and increased physical activity. A prescriber will assess your BMI, medical history and current medicines before deciding whether it is suitable for you.1

So how do you decide? Think about three things. First, do you currently qualify for the NHS pathway in your area, and how long might you wait? Second, what can you afford each month at the dose you are likely to need? Third, would you prefer a defined private service with monthly reviews included? An NHS prescription in England carries the standard charge of £9.90 for a normal prescription item, but whether weight-management injections are supplied that way depends on the service, so ask your GP.

If you go private, the cheapest program is not the one with the lowest headline. It is the one you can sustain at the dose you need, from a pharmacy you can verify, with clinical oversight. 09 · Other GLP-1 options

Wegovy, tablets and liraglutide: do they cost less?

Other GLP-1 products exist, but cheaper on paper does not mean cheaper overall, and the products work differently. Mounjaro (tirzepatide) activates two receptors, GIP and GLP-1, while semaglutide (Wegovy and Ozempic) is a single-action GLP-1 medicine.1 They are different treatments, so price should not be the only factor in choosing between them.

Wegovy is a once-weekly semaglutide pen. Its dose ladder is 0.25mg, 0.5mg, 1mg, 1.7mg and 2.4mg, stepping up after at least four weeks and reaching the 2.4mg maintenance dose from week 17. The 2.4mg dose is the licensed maintenance dose for weight management.18 Our first Wegovy pen is £99.99 as a first-order price; see the Wegovy pen page for the live ladder. In the STEP 1 trial, semaglutide was studied in adults with overweight or obesity,9 but trial results for different medicines should not be set side by side as though they were a head-to-head test.

Tablets are the other category. Wegovy tablets (oral semaglutide) and Foundayo (orforglipron) are licensed as film-coated or oral products with their own product information.2021

Liraglutide (sold as Saxenda, and in products such as Erly) is supplied at 6mg per millilitre and is a daily injection, unlike once-weekly Mounjaro and Wegovy. A daily injection is a different routine, and figures for liraglutide vary, so your prescriber can advise on how it compares for you.

The practical rule: choose the medicine with your prescriber on suitability, tolerance and routine, then compare prices for that medicine. Switching to a different product only because its monthly figure looks lower can cost more in the end if it suits you less or needs a different dose pathway. Ordering safely takes three steps, and the whole process is built around a clinical review rather than a checkout. When the pen arrives, check it. A genuine UK Mounjaro pen comes from a registered pharmacy with a batch number and expiry date, arrives cold-chain packaged, and the solution inside is clear and colourless. Do not use a pen that has been frozen, looks cloudy or discoloured, has particles in it, is past its expiry, or has been above 30°C. An in-use pen can be kept out of the fridge at not above 30°C for up to 30 days after first use, and must then be thrown away. If supply or packaging ever seems wrong, check the pharmacy on the GPhC register and contact the pharmacy before injecting.

For storage, keep unopened pens in the fridge until first use, and they keep until the printed expiry date. Follow the pen's instructions each time you inject.13 Needles, a sharps bin and injection guidance are included with every order.
