03 · Results at 3 months

What to expect by month three

Three months in, most people are still on the lower rungs of the dose ladder, so losses are modest compared with headline figures. A sensible expectation on a standard titration is mid single digits as a percentage of body weight, with wide individual variation. In SURMOUNT-1, losses built month by month, with the full averages of 15% on 5mg, 19.5% on 10mg and 20.9% on 15mg measured at 72 weeks, not 12.² Individual results vary.

Slower early progress does not predict failure, because the dose you respond to may still be ahead of you.

04 · Stopping after 3 months

Can you stop after three months?

You can stop at any point, and your prescriber can talk it through. Tirzepatide has a half-life of about five days, so it takes roughly 4–5 weeks to clear the system after a final dose.¹ Trial results were measured at 72 weeks, so three months is better seen as the start of a longer plan.

05 · Early weeks and plateaus

First weeks and plateaus

The 2.5mg weeks are about adjustment. Some people notice appetite change in week one, others feel little until a higher dose, and both are normal. Mild nausea, altered appetite and tiredness in the first days after each injection are the commonest early effects and usually settle. Plateaus of a few weeks are also expected. Review portions, protect protein and activity, check sleep and alcohol, and ask your prescriber about a step-up. A plateau is not a reason to double doses or add unlicensed products.

Who can use Mounjaro

Mounjaro is licensed for adults with a BMI of 30 or above, or 27 to 30 with at least one weight-related condition such as high blood pressure, high cholesterol, sleep apnoea, cardiovascular disease, prediabetes or type 2 diabetes. It is used alongside a reduced-calorie diet and more physical activity.¹ NHS access is being phased in over several years, with NICE recommending it for a BMI of at least 35 plus a weight-related condition.⁴
